R. Ashburn, director and D. Vines, assistant director assisted me with today’s visit. Your program currently operates GS-110 notice of compliance, issued 1/18/19. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety one percent as of 5/26/26. The NC Secretary of State website was reviewed on 5/26/26 and Opendoor CDC, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and the facility email address was updated. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. I completed a walk-through of the facility, monitoring all indoor and outdoor areas. Nine groups of children were observed in both the indoor and outdoor learning environments. Children were participating in indoor and outdoor free play and whole group time. Children enrolled in spaces #1 and #6 received care according the their individual needs, including tummy time, diapering, feedings and naps. Safe sleep checks were completed and documented as required. Files for new staff were reviewed. A fire inspection and sanitation inspection remain current. The following violations were observed during the visit. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. In space #5, a screw was exposed in the bottom shelf of the kitchen set. .0601(c) 843 A drug or medicine was administered after its expiration date. One tube of A&D Ointment in space #6, expired 3/2026. 10A NCAC 09 .0803(1)(d) Comments Section: Compliance Verification: No violations were cited during today’s visit. No further action is required at this time. Medication-Child care staff must monitor medication expiration dates to prevent the administration of ineffective or potentially harmful treatments. Expired medications lose potency, posing serious risks, especially for emergency and life-saving treatments (e.g., EpiPens or inhalers). There was one tube of A&D Ointment in space #6 that expired March 2026, however the authorization form expires on December 22, 2026. You shared that you have recently implemented a monthly check of medication and that you mistakenly overlooked this ointment. You removed the medication and notified the child’s parents during the visit. You will keep the medication in the office for pick-up by parents at their request. Reviewing medication expiration dates and authorization dates is a key safety measure for facilities. We reviewed your current checking procedures performed by the administrative staff. We discussed how classroom staff can assist with monitoring by checking the medications, at least monthly, against the authorization forms. Additionally, when completing the authorization forms, you may consider having parents make the authorization document to expire at the same time as the medication for less tracking and easier monitoring. Furnishings- equipment and furniture provided by a child care center shall be kept clean and in good repair. Today, there was a exposed screw on the kitchen set in space #5. The screw was removed during the visit. We discussed the types of materials that you have throughout the facility. We discussed investing in more sturdy, commercial use furnishings for the spaces as it seems you have made multiple replacements of the kitchen set over time. We discussed the daily checks that are completed, prior to caring for children. Adding a list of potential hazards for staff to look for and training staff on classroom monitoring and reporting may help you to become of potential hazards more immediately. Additional Comments: For the latest information on child care rules and regulations, please visit the Division of Child Development and Early Education's website at https://ncchildcare.ncdhhs.gov/ Criminal Background Check-Today you shared with me that you have some staff that are seasonal and some that may take a leave of absence. If an applicant or staff member has not been employed in child care within the last 180 days, a new criminal background check application must be completed. An individual may have their criminal background recheck completed as early as six months prior to the expiration date that is noted on the qualification letter. Outdoor Environment-During our walkthrough of the outdoor environment, we noticed a empty wasp nest. You shared that it was recently active and facilities maintenance staff had strayed and gotten rid of the wasp. You shared your current practices for monitoring insect/pest activity and your current pest control methods. I also noticed the mulch has become compacted near the stationary structures, and it was noted on the recent playground inspection. While the mulch does measure at 6 inches in the fall zone, the compact nature of the material does affect its resiliency during a potential fall. You were able to put a work order in and facilities maintance staff confirmed the work order and there is a plan to have it fluffed with in the next few days. Transitions for Infants and Toddlers- Transition means that something is changing. Not all transitions are equal, and everyone experiences them differently. Every day there are many small transitions for infants and toddlers. Think about how many times a day very young children start an activity, stop an activity, wash hands, eat, go outside, go inside, go to sleep, wake up, and so much more! Because transitions are such a huge part of every infant and toddler’s day, it’s important that we make the best of them. When transitioning from the outdoor to the indoor space that was a wait time for infants as staff transitioned them from the stroller to hand washing and then to indoor free play. This wait time can make children have big feelings and become upset. We discussed having an transition bin of toys. These would be toys that are cleaned and sanitized that would be given to the children as they wait for handwashing and immediately removed when they wash their hands and placed in the dirty toy bin for cleaning at a later time. This will give the children things to do and keep the classroom materials clean. Additionally, staff can talk with and sing to the children as they wait. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you. Your program currently operates with a GS 110-106 Notice of Compliance, issued January 18, 2019. The last annual compliance visit was conducted December 4, 2024. The sanitation inspection was completed May 29, 2025, with a “Superior” classification. The last fire inspection was conducted January 16, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety three percent as of November 14, 2025. The NC Secretary of State website was reviewed on November 14, 2025 and Opendoor CDC, LLC was listed as current- active. Lunch consisted of chicken nuggets, sweet potatoes, applesauce, and milk. The following violations were documented. Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Today, I observed Expo White Board Care spray and Zevo Insect Killer spray located on the bottom shelf of an unlocked closet in space #2. During the visit, you moved the items to locked storage and locked the closet door. We discussed sharing proper storage practices with your staff. You may get a copy of the Hazardous Items Storage poster from the Division’s website to share with your staff. Children’s Health Records- Ensuring that children are current with vaccinations reduces the number of communicable diseases that can be fatal to children both in your care and nationwide. It not only protects the child, it also protects the rest of the children in care. Unless there is a religious waiver on file, all children enrolled in child care are required to have documentation of current immunization records on file within 30 days of enrollment. A current health assessment help to identify thing that will help caregivers in meeting each child's physical, emotional, cognitive and social needs. One of fifteen children’s records reviewed contained an immunization record dated beyond thirty days of enrollment. Two of fifteen children’s records reviewed contained a medical report dated beyond thirty days of enrollment. You shared with me that as of March 2025, you require a current medical report and immunization record to be on file upon enrollment. Continue to review children’s records regularly to ensure the required documents are on file and received within the required timeframe. Staff Records- Staff employed on 10/7/25 had a medical report of file dated 6/14/23 and 11/5/25. You stated that the staff’s records were turned in to you prior to employment and the date on the form was misread. You shared with me that you are working to get all the staff on the same cycle for updating the forms to help you better track this information. Set aside time to review staff and children’s records regularly. We discussed ways to build this into your daily/weekly routine. Pathways to the Stars: During the visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. You informed me that your facility will not pursue a pathway and will remain operating with the Notice of Compliance. Reminders: Be knowledgeable of the child care laws and rules, review the with your center staff, and assist your staff with maintaining compliance with the laws and rules. During the visit, you moved the items to locked storage and locked the closet door. We discussed sharing proper storage practices with your staff. You may get a copy of the Hazardous Items Storage poster from the Division’s website to share with your staff. Children’s Health Records- Ensuring that children are current with vaccinations reduces the number of communicable diseases that can be fatal to children both in your care and nationwide. It not only protects the child, it also protects the rest of the children in care. Unless there is a religious waiver on file, all children enrolled in child care are required to have documentation of current immunization records on file within 30 days of enrollment. A current health assessment help to identify thing that will help caregivers in meeting each child's physical, emotional, cognitive and social needs. One of fifteen children’s records reviewed contained an immunization record dated beyond thirty days of enrollment. Two of fifteen children’s records reviewed contained a medical report dated beyond thirty days of enrollment. You shared with me that as of March 2025, you require a current medical report and immunization record to be on file upon enrollment. Continue to review children’s records regularly to ensure the required documents are on file and received within the required timeframe. Staff Records- Staff employed on 10/7/25 had a medical report of file dated 6/14/23 and 11/5/25. You stated that the staff’s records were turned in to you prior to employment and the date on the form was misread. When you realized the documents were not valid, the staff was suspended until the current TB Screener and Medical Statement was received. Cautiously review all documents received before filing them to make sure they are complete, current, and contain all the required information. Five of thirty staff files reviewed contained a health questionnaire and emergency information form that was dated beyond one year. You were able to have three of the five staff update these forms during the visit. You shared with me that you are working to get all the staff on the same cycle for updating the forms to help you better track this information. During the visit reminder was set on the digital calendar for the director and assistant director to update staff health questionnaire and emergency information forms in September and October 2026. Caregivers/teachers must be diligent in their adherence to the medication administration policy and procedures to prevent any inadvertent medication errors, which may be harmful to the child. There is always a risk that a child may have a negative reaction to a medication, and children should be monitored for serious side effects that may require an emergency response. Medications may change chemically beyond their expiration date and should not be used. Today, I observed one tube of Aquaphor Healing Ointment that expired on 3/2025, located in space #6. One canister of Aquaphor Healing Ointment was located on the changing table of space #5. In space #9, there was one twin pack box of epinephrine injector pins, that did not have a pharmacy label on it. We discussed your current monitoring system for tracking medications and authorization forms. Currently medications and authorizations must be reviewed by administrative staff upon acceptance into the facility. You reported that medications and authorizations were logged on a spreadsheet and periodically reviewed. Today, we discussed making a tracking tool for each classroom and have scheduled times, at least monthly, where a staff member looks at all medications and their authorization forms to ensure forms are current and complete and neither has expired. Outdoor Environment- Today, I observed a panel of the exterior wall of the building, on outdoor space #2, had the lower right corner missing and the lower half of the panel was not attached to be building. You stated that there had been some previous repairs made to the exterior wall, but you were unsure if it was that panel. You were able to put in a work order during the visit. When completing your daily outdoor inspections, be sure to observe all areas for hazards, including areas that are not the climbing structure or ride on toys. When a hazard is observed, it should be addressed immediately. Review your facility’s requirements and process for reporting damages and the procedures are to ensure the safety of the children when using the outdoor areas. In the meantime, this area needs to be made inaccessible to children.
