T. McGhee, Director, assisted me with the visit. Your program currently operates with a five-star license, issued 10/11/22, earning 7 points in the education component, 5 points in the program standards component (meeting enhanced space) and 1 quality point for the programmatic option of staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted June 18, 2025. The sanitation inspection was completed January 23, 2026 with a “Superior” classification. The last fire inspection was conducted August 4, 2025 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety nine percent as of June 2, 2026. The NC Secretary of State website was reviewed on June 2, 2026 and McGhee Family Enterprises, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Six groups of children were observed in the indoor space and outdoor learning environments. Children enrolled in space #3 received care according to their individual needs, including tummy time and bottle feeding. Safe sleep checks were documented as required. Throughout the facility, children were engaged in indoor free play, whole group and transitions. Staff were observed leading whole group, asking opened questions and assisting children with learning materials. Lunch was observed and consisted of corndogs, peas, oranges, and milk. The following violation was documented. In space #9, one Ventolin HFA inhaler had an authorization form on file that expired on January 23, 2026, and one Ventolin HFA inhaler did not have an authorization form on file. 10A NCAC 09 .0803(1)(a & b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. You shared that families sometime request to take the medications home and return them with items missing. We discussed how you receive, track, and monitor medications. Consider limiting families from removing medications from the facility in non-emergency situations. You stated you will return the medication with no box and have the other family sign new authorization at pick-up. Parents were notified during the visit. Health and Safety Trainings - Child care administrators and staff members must complete health and safety training within one year of employment unless the staff member has completed the training within the year prior to beginning employment. Additionally, health and safety training must be completed as part of on-going training so that every five years, all required topic areas will have been covered. Staff employed on 5/27/26 did not complete training on all the required topic areas within one year of employment. You stated you became aware the staff did not complete all the trainings while reviewing files and had them start. Have the staff complete the Medication in Childcare training to complete the training series. I shared with you the steps to complete the health and safety training via DCDEE Moodle. Please provide documentation of successful completion of training with your compliance letter. Rated License Information: Your rated license re-assessment using the new QRIS rated license rules and procedures was due by December 31, 2026. You submitted your application for re-assessment May 27, 2026. A new license is based on the following: Education – Staff Education and Information Form was submitted on February 17, 2026, however it is no longer current. Having a “math rich” environment that includes print numbers helps children learn numbers, number order, matching, grouping, etc. Encourage children to play with these types of materials and talk positively about numbers and help them understand the relationship between a written number and the quantity it represents. “What number is on your card? The Child Care Commission is currently in the rulemaking process to spell out and implement the changes it recommended in the QRIS Legislative Report. Today, I completed a walk-through of the facility, monitoring all indoor and outdoor areas. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Children enrolled in space #3 received care based on their individual needs, including feeding and diapering. Staff moved about the space talking to and playing with the children. In space #4 and #9 engaged in indoor free play in learning centers including blocks, art, dramatic play and tabletop manipulatives. Children’s artwork hang on the walls of the classroom. The outdoor areas offered stationary climbing structures, tricycles, cozy cars, balls and hula hoops to encourage gross motor play. Files for new staff were reviewed. Fire and sanitation inspections have not occurred since the last visit and remain current. Authorization for all prescription medication needs to be documented on the “Medication Authorization Permission Form,” even if it is a topical medication. You stated that it was an oversite that the medication was a prescription and it is not currently being used and you will return it to the parent. We discussed your current system to monitor medications. Please be reminded to double check medication and their original packaging to ensure the proper form and information is provided. Safe Sleep- The frequency of visual checks should be included in your Safe Sleep Policy and followed. Best practice is to check on sleeping infants every 15 minutes. Visual checks must be documented. Safe Sleep checks were not documented for 1/23/24 for four of four infants in attendance on that date. It was reported that staff did complete the check and documented the sleep time and wake times in ProCare, but not on the Safe Sleep log. We discussed having staff record the checks as it happens and completed periodic checks to ensure documentation is correct and current. T. McGhee, Director, assisted me with today’s visit. Your program currently operates with a five-star license, issued 10/11/22, earning 7 points in the education component, 5 points in the program standards component (meeting enhanced ratios) and 1 quality point for the programmatic option of staff benefits package and infrastructure of parent involvement. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent as of 1/22/24. The NC Secretary of State website was reviewed on 1/22/24 and McGhee Family Enterprises, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. Today, I completed a walk-through of the facility, monitoring all indoor and outdoor areas. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Children enrolled in space #3 received care based on their individual needs, including feeding and diapering. Staff moved about the space talking to and playing with the children. In space #4 and #9 engaged in indoor free play in learning centers including blocks, art, dramatic play and tabletop manipulatives. Children’s artwork hang on the walls of the classroom. The outdoor areas offered stationary climbing structures, tricycles, cozy cars, balls and hula hoops to encourage gross motor play. Files for new staff were reviewed. Fire and sanitation inspections have not occurred since the last visit and remain current. We discussed your current system to monitor medications. Please be reminded to double check medication and their original packaging to ensure the proper form and information is provided. Safe Sleep- The frequency of visual checks should be included in your Safe Sleep Policy and followed. Best practice is to check on sleeping infants every 15 minutes. Visual checks must be documented. Safe Sleep checks were not documented for 1/23/24 for four of four infants in attendance on that date. It was reported that staff did complete the check and documented the sleep time and wake times in ProCare, but not on the Safe Sleep log. We discussed having staff record the checks as it happens and completed periodic checks to ensure documentation is correct and current.
