B. Grinder, director, assisted me with the visit. Your program currently operates with a Notice of Compliance issued July 22, 2022. The last annual compliance visit was conducted July 30, 2025. The sanitation inspection was completed May 6, 2026, with a “Superior” classification. The last fire inspection was conducted on September 5, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-four percent as of July 7, 2026. The NC Secretary of State website was reviewed on July 7, 2026, and Christ’s Church Kids, LLC was listed as current-active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. Four groups of children in the indoor and outdoor spaces. Children enrolled in space #1 were observed bottle feeding, engaging with materials on Exersaucers and moving freely about the space. Children enrolled in spaces #2, #3 and #6 were observed engaged in chapel activities, transitions, and snack. In the outdoor space, children moved freely, engaging with ride on toys and the stationary structure. Staff were observed talking with the children, redirecting them, and engaging in play. The following violations were documented. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. On outdoor space #1, a spaceship shaped playhouse had a hole in the roof, where mold and mildew was observed. On outdoor space #2, the dome on the climbing structure had a hole in it, creating sharp edges. .0601(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One canister of Microdot Bleach Wipes was stored in an unlocked cabinet, above five feet, in space #1. .2820(b) 847 Parent's medication authorization did not include required information. Authorization for one tube of Nystatin Ointment USP, located in space #1, was completed on the Medication Administration Permission Form for Over-The-Counter Topical Medications and Fluoridated Toothpaste form, therefore missing the dosage, possible reactions/side effects and acknowledgment that the child had received the medication without reactions or side effects. 10A NCAC 09 .0803(4)(6-9) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. Staff employed on 9/6/23, had an ITS-SIDS training certificate that expired on 9/6/25. .1102(f) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 22, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. One canister of Microdot Bleach Wipes was stored in an unlocked cabinet near the refrigerator. Thank you for moving it to locked storage in space #4. You shared that staff were aware the wipe should not be stored in the classroom and the classrooms are also shared with the church, so it is possible they were left there by non-childcare staff. Prior to providing care for children, staff should inspect their classrooms for potential hazards, not just at the child’s level, but in the entire space including bins, closets, cabinets, and other storage spaces. Inspecting prior to caring for children gives staff a chance to address any concerns and make necessary accommodation to provide a safe environment. Medication Authorization- Administering medication requires skill, knowledge and careful attention to detail. Parents/guardians and prescribing health professionals must give a caregiver/teacher clear written authorization to administer medication to the child. While Nystatin is a topical ointment, it is prescribed by a physician, therefore needs to be on the Medication Administration Permission form. You were able to provide the correct form to the classroom staff with the correct form and speak with them about the two forms during the visit. Providing the staff with the Medication Administration Poster located at https://healthychildcare.unc.edu/wp-content/uploads/sites/17234/2023/09/Medication-Administration-Poster.pdf may be helpful to staff to know which form to use when receiving medications. Review the Accepting, Documenting, Storing Medication section of the poster with the staff to ensure they know what to look for when accepting and documenting medications. Outdoor Equipment- Proper maintenance is a key factor when trying to ensure a safe play environment for children. Equipment must be sturdy, stable, and free of hazards that are accessible to children during normal supervised play including splinters. Any hazardous equipment should be made inaccessible to children by barriers, or removed until rendered safe or replaced. Today, on outdoor space #1, a spaceship shaped playhouse had a hole in the roof, where mold and mildew was observed. On outdoor space #2, the dome on the climbing structure had a hole in it, creating sharp edges. These hazards were not documented on the June 2026 playground inspection. You stated you were aware of the hole in the dome of the climbing structure and repairs had been made but did not know it was in need of repair again. You removed the portable structure on space #1 and repaired the dome on space #2. You plan to look for a replacement dome or panel as a long-term fix. When doing daily inspections of the outdoor space, staff need to report any needs immediately and limit access to hazardous equipment to prevent injuries to children and staff in child care. ITS-SIDS- One of the most important ways to reduce infant deaths in child care settings is for caregivers to implement (and model for parents/guardians and families) safe sleep practices in their programs. ITS-SIDS training must be completed withing 2 months of caring for infants and again every 3 years from the completion of previous ITS-SIDS training. Staff employed on 9/6/23 had an ITS-SIDS training certificate that expired on 9/6/25. In review of the Staff training worksheet, the year of the training was documented as 2023, causing you to understand the expiration to be 9/6/2026. As of 6/30/26 you had the staff member registered for ITS-SIDS training on 7/28/26. We discussed checking your tracking tool against the documents on file to ensure accuracy. B. Grinder, director, assisted me with today’s visit. Your program currently operates with a GS-110 notice of compliance, issued 7/22/22. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety four percent as of 12/5/25. The NC Secretary of State website was reviewed on 12/5/25 and Christ Church Kids, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. Today I completed a walk-though of the program, monitoring all indoor and outdoor spaces. Four groups of children were observed in the indoor environment. Children throughout the program were engaged in indoor free play with materials including magnet tiles, crayons and paper, and wooden blocks. Teachers were observed moving about the spaces, assisting children with the learning materials and transitioning into play. The following violation was observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The September 5, 2025 fire inspection report was not submitted to DCDEE within one week of the approved inspection. 10A NCAC 09 .0304(a) Compliance Verification- Violations documented were corrected during the visit. No further action is required at this time. Technical assistance: Fire Inspection- A fire inspection was completed in September 2025 as required, however a copy was not submitted to the Division. You stated that you were not aware that you needed to submit it, as you had not submitted it in the past due to the previous fire marshal emailing it to the facility and the consultant when the inspection was completed. During today’s visit, a copy of the latest inspection was provided and you added a reminder to your inspections check list to email a copy to the consultant, to ensure you do not forget it in the future. Pathways to the Stars During the visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Additionally, we discussed the Education Standards. You shared that you do not wish to pursue a pathway at this time and wish to continue to operate under the Notice of Compliance. Resources • The QRIS Modernization page outlines the three pathways to demonstrate quality and earn a 2-5 Star Rated License: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. We recognize that the transition to the new QRIS is a big change. Consultation: Outdoor area-The large shade tarp has come detached from the posted and is currently tied to the fence to help ensure children have proper shade. You shared the facility is aware that the tarp is not attached properly, as you stated the anchoring line snaped during a recent strong wind. You stated you have placed a work order to have the repairs done soon. We discussed staff’s positioning during outdoor play, so children do not try to grab or pull on the tarp as it is currently attached to the fencing. During your observations of outdoor play, you may see a need to have additional staff assist on the playground in that area to ensure safety. Indoor spaces-Today you shared that the classrooms are used by the church, outside of the typical child care hours. With the classroom spaces having dual use, sometimes things are rearranged or misplaced. We discussed the checklists and practices used by both groups to ensure the spaces are ready for use. You shared that the checklist have been in use for over a year and seem to work pretty well, however there are still times when things aren’t ideal. Consider revising the checklist and/or creating a brief orientation for volunteers that use the space during non-child care hours. North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Four groups were observed in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children enrolled in space #1 received care based on their individual needs. Children engaged in free play, moving about the spaces freely and playing with soft while others napped in cribs. Safe sleep checks were completed and recorded as required. Children enrolled in space #2 participated in small group activities with their teachers, counting monkeys and practicing lacing on cardboard cutouts. Children enrolled in space #5 engaged in gross motor play with tricycles and tag games in the auxiliary space. Teachers moved about the space monitoring the children and engaging in conversation with them. In space #6, children had a morning snack. Teachers assisted children with personal care needs, handwashing, opening packages. Proper handwashing techniques were observed. A wipe should be used to clean the teacher's hands, even when gloves are used, and then another wipe to clean the child's hands after removing the dirty diaper and prior to putting on the clean diaper. You reported you had been in communication with the Child Care Health Consultant about completing a diapering training with staff, but the training had be postponed. Review the diaper changing procedure with staff in the classrooms where diapers and pull-ups are used. Consider rescheduling the training with the Child Care Health Consultant. Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. The resource room is set up for use by children and stores additional learning materials for each classroom. Today I observed the room being used by four children for gross motor play. A storage container, containing Airwick Stickup air fresheners, was stored on a shelf below five feet. Thank you for moving them to a locked storage closet. It is important to perform safety checks at arrival each morning and throughout the day. Having conversations about safety requirements with staff will help as you work to keep the children safe. A checklist for storage of hazardous products was provided today. Use the checklist to train staff on proper storage of products. Heating Elements- To prevent burns and potential fires, items with heating elements must not be accessible to children. During today’s visit, a portable laminator and a container of hot glue guns was stored below five feet on a shelf in the resource room. A common cause of fires and burns result from very hot heating elements being too close to flammable objects and people. You reported these items have a designated place to be stored and they were not put in the correct location. Thank you for relocating the laminator to a space above five feet in height and the hot glue guns to a locked closet. Consider designating a location for items with heating elements to cool before being stored. Remind staff of the importance of properly storing potentially hazardous items. Recognizing & Responding to Suspicions of Child Maltreatment - Caregivers/teachers are mandatory reporters of child abuse and neglect. Proper education is important and can increase the likelihood of appropriate reports to child protection and law enforcement agencies. One staff member did not have a training certificate on file for Recognizing & Responding to Suspicions of Child Maltreatment training. You stated you had requested the staff member complete the training and was told it was started but not completed. Please have the staff member visit the Prevent Child Abuse NC website www.preventchildabusenc.org to complete the training and provide the certificate with your compliance letter. Consider having staff complete the training on-site during orientation to ensure it is completed within 90 days of employment. Additional Comments: Wait Time Activities- There are many times throughout the day when children have to wait as a part of a transition such as waiting for meals, diapering/toileting, or transitioning form one activity to another. During this time children may become restless, and you may see undesired behavior. Planning ahead can help minimize those undesired behaviors and keep children active and engaged. Participating in reading books, singing songs, fingerplays, and games help to keep children engaged while waiting. Teachers can build specific songs or fingerplays into daily transitions to help children remember what activity will come next. B. Grinder, director, assisted me with the visit. Your program currently operates with a Notice of Compliance issued 7/22/22. The last annual compliance visit was conducted 9/27/22. The sanitation inspection was completed 6/8/23 with a “Superior” classification. The last fire inspection was conducted 6/21/22 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty two percent as of 9/5/23. The NC Secretary of State website was reviewed on 9/5/23 and Christ’s Church Kids, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and the facility email was updated. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. You assisted me with the visit. Children enrolled in space #1 were having indoor free play including playing with farm animals, stacking cups, blocks, and linking toys. Children moved about the space freely. Teacher sat near the children, engaged in conversation with them and caring for their individual needs including diapering and bottle feeding. In space #2, children used crayons and paper to decorate an apple for the apple discussion. Children enrolled in space #3 participated in music and movement and routine care before transitioning to whole group time with their teacher. In space #4, children read books and play with dinosaurs. Proper hand washing techniques were observed. The outdoor learning areas offer stationary and mobile climbing structures, trucks, trikes, and ride toys to encourage vigorous gross motor play. The following violations were documented. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. The surface of the table in space #1 was cracked, creating a sharp edge. .0601(c) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. There is no fire drill documented for the June 2023. .0604(t); .0302(d)(5) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Three cans of Barbasol shaving cream were stored in an unlocked cabinet above five feet. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Staff employed on 6/19/23 had a medical report on file dated 7/18/23. 10A NCAC 09 .0701(a) 1867 The depth of the loose surfacing was not based on critical height of the equipment. On outdoor space #1, the surfacing in the fall zone of the climbing structure measures less than 1 inch. .0605(k)(1-4) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. In space #1, an albuterol inhaler was stored without the original packaging. 10A NCAC 09 .0803(2)(b)(i-v) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 9/20/23, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance: Storage of Hazardous Items – All aerosol cans must be kept in locked storage. Today I observed three cans of Barbasol shaving cream in an unlocked cabinet, above 5 feet in space #2. The cans were moved to locked storage. You stated the lock was broken and were able to have it repaired during the visit. Acceptable locks include magnetic safety locks, combination locks, and padlocks. Child-safety latches are not acceptable to prevent access. Medication- Medicines can be crucial to the health and wellness of children. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. In space #1, there was an albuterol inhaler not stored in its original packaging. Original packaging and pharmacy labels provide you with the necessary information needed to provide the utmost care, including: the child’s name, administration instructions, and possible side effects. When accepting all medication, please be sure to also received the manufactures packaging. You were able to contact the parent to provide you with the packaging. Furnishings- All equipment and furnishings shall be sturdy, stable, and free of hazards that may injure children including sharp edges, lead based or peeling paint, rust, loose nails, splinters, protrusion, and pinch and crush points. In space #1, I observed a cracked tabletop, creating a sharp edge. We discussed your current system for staff to report broken or damaged items. Have the table repaired or replaced. Surfacing- All stationary outdoor equipment more than 18 inches high must be over protective surfacing. Protective surfacing shall cover the area under and around fall zone and be at a depth of at least 6 inches. Today, mulch in the fall zone near the slide in outdoor space #1 measured 1 to 2 inch. You stated it had been a over a year since mulch was last replaced and will work on getting more installed. When conducting playground inspection, be observant of the amount of surfacing present. Have protective surfacing installed under and around playground equipment to help reduce the risk of serious injury. Fire Drill-Even though emergencies are unpredictable and unexpected, we should do our best to prepare as generally as possible. Then, when a real emergency happens, we can rely on our knowledge and training, and adjust according to the situation in front of us. Practicing what to do in an emergency before one happens can help staff and children feel more confident in their ability to handle an emergency and can help reduce stress. There was no record of a fire drill for June 2023. You stated the June drill was overlooked by mistake. Fire drills are required to be conducted at least monthly to ensure that all staff and children, are aware of what actions to take in the event of a life-threatening emergency. Fire Inspection- A fire inspection is required to be conducted annually. You stated the month of the annual inspection changed due to adding additional classrooms last year and you did not change the reminder date. During the visit you were able to contact fire inspector to have an inspection completed. Moving forward, set aside a time to review the annual inspection dates at least quarterly to help track upcoming due dates and to allow time to have appointments scheduled prior to the date of the previous inspection. Staff Medical Report- Staff qualifications are essential for adults who interact with and teach children. requirements protect children by ensuring safe and healthy environments where developmental needs of children are met. All staff are required to obtain a medical report is due on prior to the date of hire. Staff employed on 6/19/23 had a medical report of file dated 7/18/23. When bringing on a new staff member, be sure to have the medical report on file prior to hire.
