The last Annual Compliance visit was conducted on August 13, 2025. The facility currently operates with a Notice of Compliance Permit that was issued on April 7, 2003. The last Sanitation Inspection was completed on May 20, 2026, with a "Superior" classification and received four (4) demerits. The last Fire Inspection was conducted on August 19, 2025, and the facility was approved for daytime care only. The last Fire Drill was conducted on June 16, 2026. The last Lock Down/ Shelter-in-Place was on April 29, 2026. The last Playground Inspection was conducted on June 9, 2026. Upon arrival, I was greeted by the director, Tracy Fortune. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children were attending a puppet show. A checklist was used to note the requirements I monitored today. The North Carolina Department of the Secretary’s website was used to check the status of (Vernon Forest Baptist Church) corporation on July 2, 2026. This corporation is current and active. Today four (4) children’s files were monitored and found to be in compliance. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. Today two vehicles were monitored for general safety. A Chevrolet Express G3500 24 capacity bus, tag number JLJ-1980 expiring on January 6, 2027. This vehicle had a mounted fire extinguisher and a first aid kit. Dodge Ram Wagon DZ15349 expires 3/6/27. Each vehicle is insured through Church Mutual Insurance and coverage expires 3/6/27. Today three new staff files were monitored for compliance. The following violations were observed during today’s visit and technical assistance was offered to maintain compliance. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The fence had a tree fall on it from a storm and is in need of repair. G.S. 110-91(6); .0601(b) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. All staff were not recorded in the ABCMS portal. G.S. 110-90.2 & .2703(r) 1898 Staff did not complete the health and safety training within one year of employment. New staff C. Gerber did not complete this training within one year of hire date. .1102(a) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 pm July 20, 2026. The letter should include: Each item number of noncompliance cited. Describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received to my office by 5:00 PM July 20, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. Technical Assistance: Please make sure that all new staff complete their Health and Safety Trainings within one year of hire date. The director stated that a tree fell on it and the church has ordered a new fence and its coming. Childcare Centers: The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Today five children's files were monitored and found to be in compliance. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. Today three vehicles on site were monitored for general safety. 1.Dodge Ram 3500, tag number XZJ-5349, registration good through August/2025. The tires on this vehicle met safety standards and all seatbelts were in good working order. This vehicle was equipped with a mounted fire extinguisher and first aid kit. 2. Chevrolet 15 passenger van, tag number JLJ-1980, registration good through January 2025. This vehicle had one container of "Prestone" antifreeze/Coolant behind the front driver’s seat. This is a repeat violation. This vehicle had a mounted fire extinguisher and first aid box. 3.A Chevrolet 24 capacity bus, tag number PNL-3680 with tags expiring on July 2025, had one container of “O Riley’s windshield wiper cleaner” behind the driver’s seat. The back tires are starting to be worn and will need to be changed soon. This vehicle had a mounted fire extinguisher and a first aid kit. The following violations were observed during today’s visit and technical assistance was offered. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. On the playground for the younger children a sandbox has a wooden boarder that is broken and needs to be replaced. G.S. 110-91(6); .0601(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Today I found Prestone anti/freeze coolant behind the drivers seat in the 2000 Chevrolet tag number JLJ-1980 and windshield wiper cleaner in the 2001 Chevrolet bus by the passenger door. I found a teachers backpack under her desk in the classroom. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. The following staff did not have a medical prior to their start date. Ms. Fortune, Windsor and Novack. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The following staff did not have a TB test or screening completed prior to their start date. Fortune, Windsor and Novack. .0701(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. All staff did not have an annual health questionnaire completed. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Ms. Brooke S. first aid expired on 9/24/22. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Ms. Brooke S. CPR training expired on 9/24/22. .1102(d) 1757 A valid qualification letter was not on file and available to review at the facility. The following staff di not have a qualifying letter in their file. J. Salgado, A. Skinner, T. Fortune and K. Novack. Ms. Salgado is not allowed to return to the facility until she has a qualifying letter. G.S. 110-90.2(b) & (d) & .2703(e) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. The following staff did not complete this training within 90 days of hire. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. Technical Assistance: Today we discussed that all staff must have a qualifying letter before they start working and a medical and TB test or screening prior to the start date. Every year all staff need to complete a new health questionnaire and emergency form. Each staff member has 90 days to complete a “Recognizing and Responding to Suspicions of Maltreatment" from the start date. All staff have one year from their hire date to complete the Health and Safety trainings. Several staff were not up to date on their first aid and CPR trainings and this is vital to all children’s safety that staff know how to react to a medical emergency. All purses should be 5 feet off the ground and in a locked cabinet away from children. There should be no chemicals around young children at any time. Today I found anti-freeze/coolant and windshield wiper fluid on the vans where children are present. This is a repeat violation from my last visit on 9/18/23. Today I monitored the playground area for the younger children and there is a sand box that has a wooden boarder around it that is broken and needs to be replaced. Reminder a Lock down/shelter in place is required to be practiced every three months and one will need to be practiced by the end of October. Note: Ms. J. Salgado cannot return to the facility until she obtains a qualifying letter. UPDATES: -CBC qualifying letters are now valid for five (5) years regardless of the date of issue. The last Lock Down/ Shelter-in-Place was on July 10, 2024. The last Playground Inspection was conducted on September 9, 2024. Upon arrival, I was greeted by the director, Tracy Fortune. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children were observed participating in daily lessons and having music time. A checklist was used to note the requirements I monitored today. The Department of the Secretary’s website was used to check the status of (Vernon Forest Baptist Church) corporation on September 6, 2024. This corporation is current and active. Today five children's files were monitored and found to be in compliance. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. Today three vehicles on site were monitored for general safety. 1.Dodge Ram 3500, tag number XZJ-5349, registration good through August/2025. The tires on this vehicle met safety standards and all seatbelts were in good working order. This vehicle was equipped with a mounted fire extinguisher and first aid kit. 2. Chevrolet 15 passenger van, tag number JLJ-1980, registration good through January 2025. This vehicle had one container of "Prestone" antifreeze/Coolant behind the front driver’s seat. This is a repeat violation. This vehicle had a mounted fire extinguisher and first aid box. 3.A Chevrolet 24 capacity bus, tag number PNL-3680 with tags expiring on July 2025, had one container of “O Riley’s windshield wiper cleaner” behind the driver’s seat. The back tires are starting to be worn and will need to be changed soon. This vehicle had a mounted fire extinguisher and a first aid kit. I found a teachers backpack under her desk in the classroom. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. The following staff did not have a medical prior to their start date. Every year all staff need to complete a new health questionnaire and emergency form. Each staff member has 90 days to complete a “Recognizing and Responding to Suspicions of Maltreatment" from the start date. All staff have one year from their hire date to complete the Health and Safety trainings. Several staff were not up to date on their first aid and CPR trainings and this is vital to all children’s safety that staff know how to react to a medical emergency. All purses should be 5 feet off the ground and in a locked cabinet away from children. There should be no chemicals around young children at any time. Today I found anti-freeze/coolant and windshield wiper fluid on the vans where children are present. This is a repeat violation from my last visit on 9/18/23. Today I monitored the playground area for the younger children and there is a sand box that has a wooden boarder around it that is broken and needs to be replaced. The director removed them while I was present. .0604(a) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. The last Fire Inspection was conducted on August 2, 2022, and the facility was approved for daytime care only. Please contact your local fire chief for a new inspection. The last Fire Drill was conducted on September 1, 2023. The last Lock Down/ Shelter-in-Place was on September 12, 2023. The last Playground Inspection was conducted on August 18, 2023. The facility’s owner is current/active. Today four children's files were monitored and found to be in compliance. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records, Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. Today three vehicles on site were monitored for general safety. 1. Dodge Ram 3500, tag number XZJ-5349, registration good through August/2024. This vehicles front tires had very little to no tread on them and need to be replaced. This vehicle had no mounted fire extinguisher on it. 2. Chevrolet 15 passenger bus, tag number JLJ-1980, registration good through January 2024. This vehicle had three containers of Prestone antifreeze/ Coolant behind the front drivers seat. 3. A 24 capacity bus, tag number PNL-3680, had several containers of Prestone antifreeze/coolant behind the drivers seat. The director removed all the Prestone containers of antifreeze off all vehicles during todays visit. The following violations were observed during today’s visit.
