The visit was conducted with Monica Moore, Operator, by Tammy Childress, Child Care Consultant. Currently this family child care home operates with a Temporary License, issued on April 23, 2024. The restrictions include Daytime care only, and Maximum of 5 Preschool Children at any time. Ms. Moore was initially licensed to provide care to children ages three to five-years-old but has requested age range changes throughout the temporary time period and as of October 1, 2024, is now licensed to provide care to children ages 0-12. The owner of the facility, Stephen’s Angels, LLC, is current and active with the NC Secretary of State. If any changes need to be made regarding ownership of the business or you decide to sale your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. During today’s visit, the following items were monitored: •Supervision •Capacity •Adequate Approved/Space •Storage of Hazardous Substances •Storage of Medication •General License Requirements •Staff Records (Not Applicable- No New Staff) •Program Records •License Posted •Applicable Rated License Requirements Children in care were engaged in rest time and an infant was being bottle fed, placed down for nap. Diaper changing and handwashing were also observed. The following violations were documented during today’s visit: Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. Two pets in the outdoor premises did not have up-to-date vaccinations on file. .1719(b)(1) 1741 Prior to enrollment of children in a Family Child Care Home, and before animals come into the Family Child Care Home, the operator did not obtain each parent's signature on a form acknowledging the type of animal located on the premises and where the animal will be kept during child care operating hours and /or the signed form was not maintained in the child's file. Two new pets on the outdoor premises were not added to a pet acknowledgement form and a new signature obtained by parents of all enrolled children. 10 NCAC 09 .1719(b)(3) All violations must be corrected immediately. You must submit documentation no later than October 16, 2024, to Ms. Childress at tammy.childress@dhhs.nc.gov. The documentation should contain the following information: -Date you are submitting the documentation -Name of the Facility and ID# -Date of Today’s Visit -Each Item# of the violation(s) Cited -Date when each Item# was Corrected -Description of how each Item# was corrected and a plan to prevent reoccurrence of the violation -Sign the documentation Rated License Assessment Review: We have discussed the two-component rated license process, subsidy requirements, the Environment Rating Scale (ERS) Assessment and Ms. Moore’s education evaluation as shown in her WORKS Account. Ms. Moore completed and provided Ms. Childress with a copy of an Application for Assessment for a Two Component Rated License. Ms. Moore has completed and provided a copy of the 3 month long self-study and self-assessment using the FCCERS-R. She will earn two (2) Points in the Program component. Ms. Moore’s WORKS account and status letter show that she has been evaluated as a FCCH Provider, is Lead Teacher Qualified, has earned the NCECC, has earned a AA in ECE, has earned a BA in B-Kindergarten, has earned Infant & Toddler Certificate, Education/Special Education Certificate, and the ECE Certificate. Ms. Moore exceeds 18 months of experience. With the education evaluation and experience Ms. Moore earns seven (7) points in education standards. Ms. Moore has obtained an Infant Toddler Certificate, giving her one (1) quality point. With a total of TEN points a FOUR Star License can be issued at the expiration of the Temporary License, without having the Family Child Care Environment Rating Scale Assessment completed. Ms. Moore did not request to have an environment rating scales assessment completed at this time. Consultation and Technical Assistance Provided During Today’s Visit: A violation was documented today regarding up-to-date vaccinations for two pets (dogs) (Child Care Rule 10A NCAC 09 .1719(b)(1)- Item #907) at the home. Prior to licensure the two dogs were staying with the operator’s daughter but have since had to return to the operator’s home and the vaccinations are not up to date. Ms. Moore’s son, who is responsible for and owns the dogs, has scheduled an appointment for the vaccinations. However, the appointment, is not until next week. For all pets in the household, you may consider documenting or setting reminders so that you can ensure vaccinations are always up to dat. For example, set technology reminders (phones, computers, tablets, calendars), schedule vaccination appointments ahead of time and request that your veterinarian mail card reminders or call you with an appointment reminder, and/or write the vaccination due date and reminder to schedule the appointment prior to expiration on your calendar each year during the applicable month or the month prior to. In addition, you also need to immediately update the forms in all children’s files with parent signatures acknowledging the two dogs are located on the premises and where they are located during operating hours, per Child Care Rule 10A NCAC .09 .1719(b)(3)- ( violation Item# 1741). As you receive the reminders to the pet update vaccinations for pet, you should also review your children’s files and ensure the acknowledgement forms are in place and current with signatures of parents. More lattice should be added to the back of the enclosure around the heat/air unit on the children's playground. We discussed how additional lattice would prevent children from accessing the back of the unit. Ms. Moore may have her daughter-in-law, Cassandra Clark, assist her in providing care to children. Ms. Clark has begun the process the CBC qualification check and we discussed the additional caregiver file checklist. Ms. Moore is interested in adding second shift to her final star rated license. We reviewed requirements for children's activity plans, menus, fire drills, and sleep requirements. 10A NCAC 09 .1728 OVERNIGHT CARE (a) A safe and comfortable bed, crib, or cot, equipped with a firm waterproof mattress at least four inches thick and a fitted sheet shall be provided for each child who remains in the home after midnight. The top of bunk beds shall be used by school-age children only. (b) A supply of clean linen must be on hand so that linens can be changed whenever they become soiled or wet. Linens shall be changed weekly or whenever they become soiled or wet. (c) An operator licensed to care for children overnight may sleep during the nighttime hours when all the children are asleep, provided: (1) the operator and the children in care, excluding the operator's own children, are on ground level; (2) the operator can hear and respond to the children; and (3) a battery operated smoke detector or an electrically operated (with a battery backup) smoke detector is located in each room where children are sleeping. Compliance History: During visits conducted at your facility, all or a portion, depending on the type of visit, of child care requirements will be monitored. Each child care requirement has a certain number of points attached to it. If you are in compliance with the requirement, you earn all the points associated with that requirement. For example, the supervision of children is 6 points. If you are supervising children, you earn 6 points, and if you are not in compliance, you earn 0 points. The total points for requirements for an annual compliance visit is 100. The compliance history is calculated using the percentage of points you earn over an 18-month time period. It is important, even while on your temporary license, that you maintain compliance history no lower than 75%. Failure to maintain the minimal 75% compliance history will affect your star rating and result in an Administrative Action. The following violations documented during today’s visit: Violation Number Comment Rule 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. One enrolled child did not have a signed discipline policy on file. G.S.110-91(10); .1727(a)&(b) 2041 Application did not include the names and the phone numbers of the child's physician and the persons to be contacted in an emergency situation.
