This visit was conducted with Ms. Cathy Shoffner, Director. This program currently operates with a GS 110-106 notice of Compliance. Restrictions include daytime care only, school age playground equipment not used by preschoolers. The Notice was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 11-04-2024. The program’s compliance history was 97% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing with toys, eating lunch, playing outdoors and interacting with the teachers. The last annual compliance visit was conducted on 01-08-2024. The last sanitation inspection was conducted on 01-30-2024. The last fire inspection was conducted on 01-19-2024. A fire drill was documented on 10-17-2024. A shelter in place drill was documented on 09-24-2024. An outdoor inspection was documented on 11-04-2024. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed emergency medicine and the paperwork that accompanies it. This program does not provide transportation. I documented the following violations during today’s visit: Violation Number Comment Rule 307 Only one caregiver was required to meet staff/child ratio, but the name, address and telephone number of agreed emergency relief was not posted or a second adult was not on the premises for emergency relief. The Emergency Plan was not posted. .0713(a)(9) 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. There was no First Aid Sheet posted. .0802(h) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Consultation: The mulch on the playground needs to be tilled. The cook stated that he gives a double portion of a fruit or vegetable on occasion to count as two servings and he intended to do that with oranges today. When I asked, he added pears to the lunch that was served. I have reviewed this visit summary with Ms. Shoffner at the conclusion of the visit and a copy was given to her. This visit was conducted with Ms. Cathy Shoffner, Director. This program currently operates with a Notice of Compliance. Restrictions include daytime care only, school age playground equipment not used by preschoolers. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing indoors and outdoors. The last annual compliance visit was conducted on 02-08-2023. The last sanitation inspection was conducted on 8-28-2023. The last fire inspection was conducted on 02-20-2023. A fire drill was documented on 08-22-2023. A shelter in place drill was documented on 04-20-2023. An outdoor inspection was documented on 08-04-2023. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. Ms. Shoffner stated that there is no medication on site today. I documented the following violations during today’s visit: Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. One staff person was present in the classroom with children and she did not have a Criminal Background Qualification letter on file. She left the premises while I was present. G.S. 110-90.2(b) This violation was corrected during the visit when Ms. Berman left the premises. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I provided the following Technical Assistance during today’s visit: Ms. Lisa Berman may return to work as soon as she receives the Criminal Background Qualification Letter. If Ms. Berman receives a Provisional Qualification letter, then she may return to work but can never be left alone with children, until the Qualification letter comes through. I have reviewed this visit summary with Ms. Shoffner at the conclusion of the visit and a copy was given to her.
