Latoya Francis, operator, assisted me with the visit. The facility was issued a temporary license on September 25, 2025. A second temporary license was issued on December 9, 2025, due to change in shifts of operation. The restrictions include 1st Shift, 2nd Shift, 3rd Shift; Maximum of 5 Preschool Children at any time; Children in Care on Ground Level Only. During today’s visit, the following items were monitored: •Supervision •Capacity •Adequate Approved/Space •Storage of Hazardous Substances •Storage of Medication •General License Requirements •Staff Records (Not Applicable- No New Staff) •Program Records •License Posted •Applicable Rated License Requirements One child, an infant, was in care and was observed to be sleeping and then engaged in interaction with Ms. Francis. You stated today there has been no changes to the information you provided on the Family Child Care Home Verification of Required Information for Operator and Additional Caregivers. There have been no changes to household members. We reviewed your training expiration dates, health questionnaire expiration date, and the number of required in-service training hours annually. You had a certificate for completion of Recognizing and Responding to Child Maltreatment dated September 20, 2025 on file. The following violations were documented during today’s visit: Violation Number Comment Rule 1887 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser were not in their original container or in another labeled container, used according to manufacturer's instructions. An aerosol bottle of Lysol Spray and a container of antibacterial wipes were on top of the refrigerator in the kitchen and an aerosol bottle of Febreeze spray was on a tall table in the entrance of the home. .1719(a)(7) Technical Assistance Provided During Today’s Visit: One bottle of Lysol aerosol spray and one container of antibacterial wipes were on top of the refrigerator in the kitchen and one aerosol bottle of Febreeze was on the tall sign in table at the entrance of the home. Ms. Francis, once brought to her attention, moved the items to the upstairs of the home in locked storage. Any items that include multiple warning, such as Keep Out of Reach of Children, Avoid Contact with Eyes/Skin, Do Not Ingest and that include first aid and handling guidance such as rinse eyes and skin water, contact poison control, store away from heat and flames must be kept in locked storage to avoid children from having access to the products and being injured. Remembering to keep hazardous products in locked storage can be difficult when operating a Family Child Care Home and when these products are used while you are not in operation. Make sure that you are visually scanning the child care areas each day to ensure these products are locked up or create a daily checklist for opening and closing tasks that includes monitoring storage of hazardous products. Rated License Assessment Review: We discussed the QRIS Modernization Rated License Pathway options, subsidy requirements, the Environment Rating Scale (ERS) Assessment and your experience and education during the last visit conducted on November 20, 2025. Also, during that visit, I provided you with a printed review of each pathway option from the DCDEE website. During a telephone call with you on February 4, 2026, we discussed your education star rating and experience. You stated during the call that at this time, you do not wish to apply for a two through five star rated license. We discussed options for increasing your star rating level in the future, including completing early childhood coursework and using the T.E.A.C.H. Early Childhood Project for scholarship assistance. To find out more about T.E.A.C.H. Early Childhood Project visit the website for Child Care Services Association at www.childcareservices.org or contact Child Care Resource Center at (336)345-4900 (childcareresourcecenter.org) for training. You stated again today that at this time you did not wish to apply for a 2-5 star rated license. Capacity Options: We reviewed North Carolina General Statute 110-1(7)(b) as follows: Family Child Care Home Capacity. – A family child care home is allowed to provide care for one of the following groups of children, including the operator's own preschool age children and excluding the operator's own school age children up to 13 years of age: 1. A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school age children. 2. A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school age children up to 13 years of age, for a total of nine children. 3. A maximum of 10 children if all children are older than 24 months of age. You understand that you can be licensed for 10 children per shift for children ages 0-12 years old, but you must be meeting one of the three options above at all times. You have the flexibility to switch between the options when needed, but you must maintain compliance at all times. The restriction for no more than 5 preschool children at any time will not be on your star rated license. Your zoning letter from Winston-Salem/ Forsyth County Planning and Development Services does not restrict you providing care to 10 children and your Fire Inspection approves daytime and overnight care. Consultation Provided During Today’s Visit: Criminal Background Check Provider Portal •As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. For Family Child Homes, this includes the hiring of yourself and also includes adding household members over the age of fifteen years old into your facility roster, which is discussed below. •The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Compliance History: During visits conducted at your facility, all or a portion, depending on the type of visit, of child care requirements will be monitored. The compliance history is calculated using the percentage of points you earn during visits conducted over an 18-month time period. It is important that you maintain compliance history no lower than 75%. Failure to maintain the minimal 75% compliance history will affect your star rating and result in an Administrative Action. Emergency Preparedness and Response Plan: You must complete the Emergency Preparedness and Response for Childcare training within one year of licensure (by September 25, 2026) and within four months of completing the training you must develop and Emergency Preparedness and Response Plan for your program. You must print the plan from the template provided to you at the training and review the plan annually. You will print off the first page of the plan each year noting the date you reviewed it. Required On-Going Training Hours: You are required to complete on-going training activities after one year of employment (licensure) as described in Child Care Rule 10A NCAC 09 .1703(d). Lead in Water, Lead Based Paint, Asbestos Testing You completed lead in water testing on December 6 and December 13, 2024. Lead in water testing must be completed every three years. When looking at the Clean Classrooms for Carolina Kids website, it shows that you have started enrollment for Lead Based Paint and Asbestos. You must immediately complete enrollment. If you have not already you will take the pre-enrollment webinar and then complete the enrollment questionnaire.
