Name of Operation: MUNCHKIN LAND Facility ID: 34000915 Consultant: JENNIFER DAVIS Operation Type: Family CC Home Case Number: Visit Date: 8/19/2025 Number Present: 5 Completed Date: 8/19/2025 Age: From 0 To 1 Total Minutes: 155 Time In: 09:40 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced An unannounced annual compliance visit was conducted at this family childcare home to monitor compliance with applicable childcare requirements. This visit was conducted with Andrea Midkiff, operator. Your program currently operates with a Three Star license. Restrictions include daytime care, maximum of five preschool children, fireplace not in use during child care hours. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 100% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 9/11/2024. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. Ms. Midkiff stated the only persons living in the home with her are her husband, daughter and mother. Be reminded that you and your family must have criminal record checks every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 8/1/2025. A lockdown drill was documented on 8/1/2025. An outdoor inspection was documented on 8/1/2025. There are four dogs inside the home and all vaccinations and parent acknowledgement forms were monitored. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Topical ointments had a permission form but did not have the name of the ointment and information required to administer it. The program does not provide transportation. The following violation was cited at today’s visit. Violation Number Comment Rule 1405 Authorization for up to 12 months to apply over-the-counter medications, topical ointment, topical teething ointment or gel, insect repellents, fluoridated toothpaste, lotions, creams, and powders; such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed, was not in writing and did not contain information required by rule .1720(b)(7). There was a signed permission form for ten(10) over-the counter and topical ointments but they did not contain the information required in rule. 10A NCAC 09 .1720(c)(4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before September 2, 2025. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. You may email the letter to jennifer.davis@dhhs.nc.gov Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Technical Assistance: - I emailed the topical ointment, over the counter permission form during the visit. Be sure all required information is included as specified on the form. Additional information: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • The DCDEE team looks forward to working with child care facilities across the state to transition to the new QRIS system, also known as Pathways to the Stars, located in Section .3200 of the Child Care Rules. Beginning In October, child care consultants will begin discussing the new rules in Section .3200; Standards for Two through Five Star Rated Licenses during licensing visits. Consultants will review the pathway options, identify facility needs, answer questions, and work with the facility operator to establish an individualized timeline for transition to a new rated license within 12 months based on the pathway chosen.
