This visit was conducted with Ms. Charlene O’Neal, Director. This program currently operates with a Four Star license. Restrictions include daytime care only, meets enhanced space and ratios. The license was posted and the restrictions were being followed. The program’s compliance history was 95% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit, the children were playing with toys and interacting with the teacher. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions Files for three new staff person(s) were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing CPR/First Aid, EPR, Playground Safety, SIDS, Health and Safety Training and Criminal Background Checks. The last fire drill was conducted on 01-02-2025. The last shelter in place drill was conducted on 12-10-2024. The last playground check was conducted on 01-08-2025. I observed the storage of hazardous items and cleaning supplies. I observed medicine and emergency medicine today, and the storage of hazardous cleaning supplies. This program does not provide transportation. The following violations were documented and corrected during today’s visit: Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. The lesson plan was not posted in space 2b. GS 110-91(12);.0508(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu posted for parents to see was dated for last week. 10A NCAC 09 .0901(b) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. The following Consultation was provided during today’s visit: I have suggested that Ms. O’Neal keep one menu in the hallway and not in each classroom, as this is more work for the cook to change the food listed. This visit was conducted with Ms. Delores Graham, Administrator. This program currently operates with a Four Star license. Restrictions include daytime care, meets enhanced space, meets enhanced ratios. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 08-20-2023. The program’s compliance history was 90% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing with toys and eating lunch. The last annual compliance visit was conducted on 12-02-2023. The last sanitation inspection was conducted on 06-29-2023. The last fire inspection was conducted on 12-03-2022. A fire drill was documented on 10-02-2023. A shelter in place drill was documented on 09-18-2023. An outdoor inspection was documented on 10-05-2023. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. This program does not provide transportation. I documented the following violations during today’s visit: Violation Number Comment Rule 432 The center did not have developmentally appropriate equipment and materials accessible daily. Chalk and chalk boards were added to the book center in space 1 because there were only two materials available instead of three. GS 110-91(12);10A NCAC 09 .0509(1) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff person did not complete Health and Safety Trainings by July 1, 2023. One staff person completed all the trainings except for the Emergency training. .1103(b) I have asked Ms. Graham to send me written documentation by October 23, 2023 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. I have reminded Ms. Graham that if the documentation indicates that corrections have been made when they have not, this may be considered falsification of information and an Administrative Action may be recommended. In addition, repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date indicated in this visit summary could lead to the recommendation of an Administrative Action. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I provided the following Technical Assistance during today’s visit: Ms. Graham and I have looked at one of two playgrounds today. The playground has new equipment that Ms. Graham stated was installed by Kaplans. There is a set of steps that are made to look like rock that ascend to a climbing structure that has two slides. The steps do not have a rail and the top step is approximately four feet from the ground. A rail will have to be installed for the children to use this. Ms. Graham stated that the playground is closed until this is completed and that children are using the other playground. I stated to Ms. Graham that the steps did not appear at first glance to match the structure, as one side of the steps are straight and one side is angled. I also pointed out to Ms. Graham that the base of the steps does not appear to be installed properly as the base at the front is completely touching the poured rubber surface, but the base at the sides is not touching the ground. There is at least a one inch gap on one side and a two or more inch gap on the other side, large enough for me to put my foot underneath it. I have called Kaplan’s today and Dawn at extension 6217 stated that Mt. Zion will need to send an email with photos to arickett@kaplanco.com. I have explained to Ms. Graham that NC Rules for child care state that Mt. Zion must have a playground available with a minimum four foot fence and that because the center has a playground available that meets the rules, restricting access to the other playground is not a violation. I have reviewed this visit summary with Ms. Graham at the conclusion of the visit and a copy was given to her.
