This visit was conducted by Jennifer Davis, Childcare Consultant, with Keith Dunlap, Operator. Your program currently operates with Four star license. Restrictions include first and second shift, maximum of five preschool children, upstairs off-limits and no more than two infants under one. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 100% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the downstairs child care area was completed today. Indoor and outdoor areas were observed. The smoke detector was tested during today’s visit, and it worked properly. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed the operator and child playing learning games and having free play. I observed materials and activities that were available to the children in the child care room. The last annual compliance visit was conducted on 12/4/2024. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. Mr. Dunlap stated the only persons living in the home with him is his wife. Be reminded that you and your wife must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at: https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 10/22/2025. A shelter-in-place drill was documented on 10/15/2025. An outdoor inspection was documented on 10/25/2025. There are no pets in the home. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does provide routine transportation. You stated that you do provide transportation in the afternoon for three school-age children. Inspections and insurance were current. Children's permission and emergency information was current. The following violations were cited during today’s visit. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. One can of aerosol and one bottle of cleaner was not in locked storage. .1719 (a)(7) 709 Equipment and toys were not in good repair and developmentally appropriate. The slide on the playground had a cracked hole on the connecting piece. 10 A NCAC 09.1720(a)(7) 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. It is necessary for all staff members and residents of the home who are 16 years of age or older to be linked to the FCCH’s facility profile in the ABCMS system, as the process of notifying the Division of any new child care providers working who were hired or moved into the child care facility within five business days has changed to include this process. I emailed a technical assistance reference for the ABCMS portal during the visit. • For children that are transported, be sure to have current photographs, emergency information and permission on file. • I emailed the hazardous item storage reference sheet during the visit today on what is required to be locked and what can be kept at least five(5) feet high. • Any broken equipment should be removed from child care areas and replaced or repaired. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • Begun in February 2023, the Child Care Commission’s work to modernize North Carolina’s QRIS is now complete. A lock down drill for the facility was recorded 3/27/2024. The program does provide routine transportation. I observed the 2002 Chevrolet Tahoe during the visit today. It was current on inspections/insurance and had five current car seats. One violation was cited at today’s visit. Restrictions include first and second shift, maximum of five preschool children, upstairs off-limits and no more than two infants under one. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. The smoke detector was tested during today’s visit, and it worked properly. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free play and in routine care. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on 2/1/2023. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. Mr. Dunlap stated the only persons living in the home with him is his wife. Be reminded that you and your wift must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 11/21/2023. A lockdown drill was documented on 11/6/2023. An outdoor inspection was documented on 11/5/2023. There is one pet dog inside the home and all vaccinations and parent acknowledgement forms were monitored. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does provide transportation. You stated that you do provide transportation in the afternoon for two school-age children. During today’s visit, I monitored emergency information, There was not permission to transport forms for all children. I observed a first aid kit and fire extinguishers on the vehicle. I monitored one vehicles today. I observed a 2002 Chevrolet Tahoe, TEC-1580. This vehicle was clean and in good repair, and had working seatbelts, adequate tire tread, and was current on inspection and insurance. The following violations were cited during today’s visit: Violation Number Comment Rule 1101 Operator or other transportation provider did not have written permission from each child's parents or guardian to transport the child. The operator did not have written permission to transport for seven enrolled children. .1723(5) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Foam edging on wooden shelves was accessible to children under three. .1719(a)(18) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before 12/29/2023. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to jennifer.davis@dhhs.nc.gov. If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance discussed during today’s visit: • I went over transportation requirements with Mr. Dunlap. He had a current written plan of care for all children but did not have permission to transport for enrolled children. This needs to be completed annually. He stated he will get it filled out today. • There was foam covering wood shelves for children. I provided information that any foam should not be accessible to children under three. Mr. Dunlap is going to cover it so it is not accessible or put a different protective edging the shelves. • • Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • You will need to access the www.ncrlap.org website to review and look at resources and training for the FCCERS-R. Mr. Dunlap will contact me when he is ready to request the ERS. He stated he is not sure at this time when he would like to request it. • Any outdoor swimming pool located on the family child care home premises shall be enclosed by a fence that is at least four feet high, separated from the remaining outdoor play area by that fence, and locked and inaccessible to children when not in use. There is an above ground pool in a different fenced area and is not currently filled with water. At the completion of the visit, this visit summary was reviewed with and provided to you.
