This visit was conducted by Jennifer Davis, childcare consultant, with Cierra Mitchell, lead teacher. Currently this center operates with a FIVE (5) star license issued 6/7/24 with the following restrictions: day time care only; enhanced ratios minus one; meets enhanced ratios; meets enhanced space. The license was observed, and the restrictions were found in compliance. This licensed program is operated by Winston Salem Forsyth County Schools and is designated NCPK. There are currently three licensed classrooms with enrollment. NCPK requirements were monitored. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children playing in centers, art activities and eating lunch I observed developmentally appropriate materials and activities were available to the children in the classrooms The last annual compliance visit was conducted on 5/6/2025. A sanitation inspection was completed 12/12/25 with a Superior classification. The last fire inspection was conducted 2/16/2026. Program records and required postings were monitored. A fire drill was conducted on 3/30/2026. A shelter-in-place drill was conducted 3/4/2026 . An outdoor inspection was documented on 3/30/2026. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Files were monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Teaching Strategies and Creative Curriculum. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted 3 times per year: at the beginning, middle and end of the school year. The first day of school was 8/13/2025 . The classroom operates from 7:50 a.m. to 2:50 p.m. Parent conferences are held 3 times per year. You stated that you use Dojo App and weekly messages to communicate with parents. A monthly newsletter is also provided to parents. The principal also communicates weekly with the school families. You verified that you incorporate family engagement by different family events including math night, reading curriculum night and Title One night. The following violations were cited during today’s visit: Violation Number Comment Rule 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. A shelter in place drill for the facility was recorded 10/16/2025. The most recent fire inspection was on 9/22/2025 and I received a copy of the inspection during the visit. I observed a sanitation inspection was last conducted on 4/24/2025 earning a Superior classification. The program does not provide transportation. The following violations were cited: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. I observed emergency medications and one topical ointment. The program does not provide transportation. The following items were reviewed from the ECERS-R that was conducted on 5/1/2024. - Observation Notes For Items Scoring 4 points or lower - Item 3 Indicator: 5.1 (see page: 14 in the ECERS-R) The book area had a soft couch and carpet. The quiet center had a bean bag over tile flooring; however, there was no area in the classroom that provided the level of softness required by this indicator. Indicator: 5.2 (see page: 14 in the ECERS-R) Since the classroom did not have an area that met the level of softness required in 5.1, credit could not be earned here. Response: An additional rug and soft cushions will be added to the area. The room will be rearranged so the area won’t be right beside the door and will be easier to supervise. Item 5 Indicator: 3.2 (see page: 16 in the ECERS-R) The teacher reported that the quiet center is limited to one child. However, due to the height of the cubbies directly beside this area, children sitting or lying on the bean bag in this center could not be seen by the teachers when standing in the block, dramatic play, and art areas. Response: This area will be moved to another area for better supervision. Item 6 Indicator: 5.1 (see page: 17 in the ECERS-R) The were pictures of the children on the class height chart, in the cubbies, and on the family photo bulletin board. Also, the children's names were on the tables, center tags, and birthday board, which meets part of the requirements for this indicator. The teacher reported that recent themes included water and planets. However, there were no items displayed that related to these themes. Indicator: 5.2 (see page: 17 in the ECERS-R) Though some of the display consisted of children's work such as paintings, drawings, coloring sheets, and 3-D art, most of the displayed materials were commercially produced and teacher made items such as educational posters, pictures of people, and a bulletin board with program policies and parent information. Response: Staff reported that the children do complete work related to the theme and that it is sent home. They will display additional work completed by children related to the theme. Item 7 Indicator: 3.2 (see pages: 18-19 in the ECERS-R) Outdoors, the mulch at the exit of the slide near the front of the climber measured 3 inches deep, rather than at least 6 inches. Refer to the document, NCRLAP's Requirements for Gross Motor Space and Equipment, for more information. Response: Maintenance fluffed mulch last week to meet requirement. Maintenance reported that the mulch around the stationary equipment will have to be dug out so additional mulch can be added this summer. Item 11 Indicator: 3.2 (see pages: 26-27 in the ECERS-R) When stored, the cots were stacked on top of one another. The blankets hung over the edges, or they were folded and placed in the middle of each cot and touched the bottom of the cot stacked above. The cots were made out of a permeable material, allowing for the potential transfer of germs between the children's bedding and the cots. Response: The blankets have been moved to the cubbies. Item 12 Indicator: 3.1 (see pages: 28-29 in the ECERS-R) The children and teachers used the sink in the classroom to wash their hands after toileting routines, arrival, returning from outdoors, before and after water play, sensory play, wiping noses, and before and after meals. The sink was not sprayed with a disinfectant after toileting routines before it was used for other purposes. Response: The teachers have been trained with the correct procedures. Examples of the times to complete tasks were given as well. Item 14 Indicator: 3.1 (see pages: 32-33 in the ECERS-R) The safety concern mentioned in item 7 applies to this indicator as well. Item 15 Indicator: 5.4 (see pages: 34-35 in the ECERS-R) The book "Little Red Riding Hood" showed an image of a wolf with exposed teeth and claws lunging toward a woman with a frightened expression on her face. Another image showed a wolf chasing a girl with a scared expression on her face. Please see the NC Additional Note for more information. Response: The book was removed from the classroom. Item 17 Indicator: 5.2 (see page: 37 in the ECERS-R) Examples of encouraging children to explain their thought processes related to problem solving were not observed. Response: Staff usually encourage problem-solving. They discussed some examples. Item 22 Indicator: 5.1 (see pages: 44-45 in the ECERS-R) There were wooden unit, hollow, cardboard, and foam brick blocks in addition to several types of accessories, which met a part of the indicator's requirements. However, the designated space for block play was not large enough to meet the requirements of this indicator. Please see the NC Additional Note for more information. Indicator: 5.3 (see pages: 44-45 in the ECERS-R) Some of the requirements for this indicator were met. However, wooden train tracks and duplos were stored in the block area. Also, during the observation, some children used alphabet connectors in the block area, which took up space for block play. This visit was conducted by Jennifer Davis, childcare consultant, with Cierra Mitchell, lead teacher. Currently this center operates with a FIVE (5) star license re-issued 6/6/18 with the following restrictions: day time care only; enhanced ratios minus one; meets enhanced ratios; meets enhanced space. The license was observed, and the restrictions were found in compliance. This licensed program is operated by Winston Salem Forsyth County Schools and is designated NCPK. There are currently three licensed classrooms with enrollment. NCPK requirements were monitored. The program’s compliance history was 92% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children playing in centers, having outside time, and eating lunch I observed developmentally appropriate materials and activities inspection were available to the children in the classrooms The last annual compliance visit was conducted on 11/2/2022. A sanitation inspection was completed 11/16/22 with a Superior classification. The last fire inspection was conducted 9/7/2023. Program records and required postings were monitored. A fire drill was conducted on 9/28/23. An emergency drill is scheduled for this month. An outdoor inspection was documented on 10/2/2023. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. They are waiting on one medication to be brought in by parent. The program does not provide transportation. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A selection of files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Teaching Strategies and Creative Curriculum. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted 3 times per year: at the beginning, middle and end of the school year. The first day of school was 8/28/2023 . The classroom operates from 7:50 a.m. to 2:50 p.m. Parent conferences are held 4 times per year. You stated that you use SeeSaw-App and weekly messages to communicate with parents. In addition, two reports are sent home to the parents which includes any information and any reminders.
