    Name of Operation: FRIEDBERG ELEMENTARY PRESCHOOL Facility ID: 29000736 Consultant: RENA MCCAFFREY Operation Type: Center Case Number: Visit Date: 4/11/2024 Number Present: 15 Completed Date: 4/11/2024 Age: From 4 To 5 Total Minutes: 190 Time In: 09:00 AM Time Out: 12:10 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s visit was to complete an annual monitoring of Applicable Child Care Requirements. The last Annual Compliance visit was conducted on May 11, 2023. The facility currently operates with a Five-Star Child Care License that was issued on May 8, 2018. The last Sanitation Inspection was completed on February 29, 2024, with a "Superior" classification and two (2) demerits. The last Fire Inspection was conducted on April 3, 2023, and the facility was approved for daytime care only. The last Fire Drill was conducted on March 26, 2024. The last Lock Down/ Shelter-in-Place was on March 6, 2024. The last Playground Inspection was conducted on March 10, 2024. Upon arrival, I was greeted by the lead teacher, Jennifer Bates. I stated the purpose of my visit and was assisted with any questions regarding information needed. I completed a walk-through of the classroom where attendance was documented, and observations made. The children were observed participating playing in centers and one on one instruction with teacher. Today the children were doing a gross motor activity inside. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. During today’s visit the following violations were cited. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was on April 3, 2023. 10A NCAC 09 .0304(a) 851 When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. One child has an Epi pen and the permission form was expired as of February 29, 2024. .0803(13)(a-e); .2318(3) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before April 22, 2024. The letter should include each item’s number of noncompliance cited, describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received by my office by April 22, 2024, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Technical Assistance: Parents must sign a permission to administer medications before it is dispensed. A medication log should be placed with this information to log in each time the medication is given. Fire inspections are due annually and one month before it expires I would reach out to the Lexington fire department. Rated License Information: -Current Rated Licenses are still in a ‘hold harmless’ state until June 2024. The Division has determined that your facility is in Cohort 1. This means that your preparation year is from July 1, 2023, to June 30th, 2024. Your reassessment year is from July 1, 2024, through June 30, 2025. Today we discussed the process of the Rated License Assessment. To prepare for the assessment I also recommend that you review the information on the North Carolina Rated License Assessment Project website at www.ncrlap.org. There you will find more checklists, forms, notes for clarification, and webinars. Also, reviewing the lower scoring items on your prior assessment can help in your preparation. Today we discussed the size of the block area, books and reading center area, lesson plans and showing examples of the children’s work. We discussed that loud centers and quiet centers should not be near each other. We spoke about placing children's art work low so that the children can see it. We discussed changing out animated pictures for real life pictures. We discussed bring the fish and butterfly exhibit. down for the children to interact. Today, I left the ERS assessment request with you. Once you are ready to have the ERS assessments completed, please complete the form, and return it to me by e-mail. Be mindful that NCRLAP has asked that any schools that are closed during the summer should send their request no later than March 29, 2024, to be evaluated before the close of summer. Today we discussed that everyone needs to upload their education into Works. We will complete an education evaluation once everyone is in. The Division of Childhood Development and Early Education will make the final determination of star rating. UPDATES: Center for Disease Control and Prevention new guidelines as of March 1, 2024. If you, your staff, or the children you care for should test positive for COVID-19, the CDC has given out new guidelines to follow. If you have a fever the CDC recommends self-isolation for 24 hours or longer, until you are fever free without any fever reducers. Please check our website for the latest Covid-19 information.
