This visit was conducted with Ms. Stephanie Brown, Director. This program currently operates with a Three Star license. Restrictions include daytime care only, must provide 35 square feet indoors per child, meeting enhanced space. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 11-18-2025. The program’s compliance history was 91% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing indoors and outdoors. The last sanitation inspection was conducted on 3-31-2025. The last fire inspection was conducted on 11-19-2024. A fire drill was documented on 11-17-2025. A shelter in place drill was documented on 10-28-2025. An outdoor inspection was documented on 11-17-2025. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. The following violations were documented today: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The lesson plan in space 2 was not current. GS 110-91(12); .0508(a) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. The teacher in the school age room has not completed BSAC training. Ms. Brown stated that either she or Ms. K Brown will care for school age children until the teacher has her BSAC training completed. They both have completed BSAC training. .2510(j) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I provided the following Technical Assistance during today’s visit: We discussed the new Rated License system today and the template was completed and is attached to this report. Today I discussed an overview of the new rules in section .3200 of the Child Care Rule Book with Ms. Brown. In addition, I discussed in detail the educations standards for the program to earn a 2-5 Star Rated License, continuous quality improvement requirements, family and community engagement requirements, accreditation, quality initiative recognition and choosing a pathway. A template was completed documenting the pathway(s) of interest, identified needs and resources. I answered Ms. Brown’s questions. I will work with her to establish a timeline for transition to a new rated license, if applicable. We discussed these resources today. Use these resources to help you in the future. • Section .3200 of the NC Rules for Child Care provides the standards for earning a two-though five-star-rated license. There are three pathways: Program Assessment, Classroom and Instructional Quality, and Accreditation and Head Start. This visit was conducted with Ms. Stephanie Brown, Director. This program currently operates with a Three Star license. The license was posted and the restrictions were being followed. The program’s compliance history was 92% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least 75%. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit, the children were playing indoors and outdoors. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • SIDS training • Criminal Background Checks • General health • Administration of medication • Storage of hazardous products • Storage of medication • General safety • General license requirements • Program records • License posted • Fire safety • Sanitation Limited monitoring of staff files was conducted in accordance with DCDEE procedures for the purpose of reviewing CPR and First Aid, EPR, SIDS training, Health and Safety trainings and Criminal Background Checks. The storage and administration of medications was monitored. Medication authorization was monitored. The storage of hazardous items was monitored. A fire drill was documented on 04-15-2025. A shelter in place drill was documented on 03-05-2025. An outdoor inspection was documented on 05-20-2025. The following violations were documented during this visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The lesson plan was out of date in spaces 7, 9 and 10. GS 110-91(12); .0508(a) 1301 Center did not maintain a record of daily attendance. The attendance had not been taken in space 7 and there was a child missing from the roll in space 12. GS 110-91(9) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Today I provided Technical Assistance in the following areas: Consultation: I have suggested the First Aid chart be posted in the hall as well. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS portal on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. I documented the following violations during today’s visit: Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child's file did not contain a form that stated the parent had received the copy of the Summary of the Law. GS 110-102 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu was not posted for parents to see. 10A NCAC 09 .0901(b) 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. The First Aid sheet was not posted for all to see. .0802(h) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff person is missing one Health and Safety Training class. One staff person is missing two Health and Safety classes. One staff person has not had renewed the Health and Safety Trainings since 2017. .1103(b) I have asked Ms. Lawson to send me written documentation by January 31, 2024 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. I have reminded Ms. Lawson that if the documentation indicates that corrections have been made when they have not, this may be considered falsification of information and an Administrative Action may be recommended. In addition, repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date indicated in this visit summary could lead to the recommendation of an Administrative Action. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I provided the following Technical Assistance during today’s visit: Children’s records may include a form that states what policies were given to parents when they enrolled their child as long as it includes the parent’s signature indicating that they received this. Staff will have two weeks to complete the Health and Safety Trainings that have not been completed. Consultation: Ms. Lawson is a new director. I have left the Legal Designee form with her today to be completed by the corporate office and I have left the Preservice for Administrators form with her today. I will pick these up on my next visit. I have not completed the visit today and will return very soon to finish the Annual Compliance visit. This visit was conducted with Ms. Ericka Lawson, Director. This program currently operates with a Three Star license. Restrictions include daytime care only, must provide 35 square feet indoors per child, meeting enhanced space. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 01-16-2024. The program’s compliance history was 82% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing with toys and interacting with the staff. The last annual compliance visit was conducted on 02-23-2023. The last sanitation inspection was conducted on 08-22-2023. The last fire inspection was conducted on 02-06-2023. A fire drill was documented on 12-29-2023. A shelter in place drill was documented on 12-20-2023. An outdoor inspection was documented on 01-04-2024. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I documented the following violations during today’s visit: Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child's file did not contain a form that stated the parent had received the copy of the Summary of the Law. GS 110-102 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu was not posted for parents to see. 10A NCAC 09 .0901(b) 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. The First Aid sheet was not posted for all to see. .0802(h) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff person is missing one Health and Safety Training class. One staff person is missing two Health and Safety classes. One staff person has not had renewed the Health and Safety Trainings since 2017. .1103(b) I have asked Ms. Lawson to send me written documentation by January 31, 2024 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. I have reminded Ms. Lawson that if the documentation indicates that corrections have been made when they have not, this may be considered falsification of information and an Administrative Action may be recommended. In addition, repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date indicated in this visit summary could lead to the recommendation of an Administrative Action. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I provided the following Technical Assistance during today’s visit: Children’s records may include a form that states what policies were given to parents when they enrolled their child as long as it includes the parent’s signature indicating that they received this. Staff will have two weeks to complete the Health and Safety Trainings that have not been completed. Consultation: Ms. Lawson is a new director. I have left the Legal Designee form with her today to be completed by the corporate office and I have left the Preservice for Administrators form with her today. I will pick these up on my next visit. I have not completed the visit today and will return very soon to finish the Annual Compliance visit. I have reviewed this visit summary with Ms. Lawson at the conclusion of the visit and a copy was given to her.
