This visit was conducted with Ms. Natasha Simmons, Director. This program currently operates with a Five Star license. Restrictions include daytime care only, no cooking in building, enhanced ratios minus one, meets enhanced ratios, meets enhanced space. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 04-08-2025 and is current-active. The program’s compliance history was 92% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing indoors and outdoors and interacting with the teachers and eating lunch. The last annual compliance visit was conducted on 05-07-2024. The last sanitation inspection was conducted on 10-22-2024. The last fire inspection was conducted on 09-12-2024. A fire drill was documented on 03-19-2025. A shelter in place drill was documented on 03-05-2025. An outdoor inspection was documented on 03-06-2025. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. I monitored NC Pre-K requirements today. This site completed the last Environment Rating Scale on 08-16-2022 and scored a 5.44. There are 60 children enrolled in the NC Pre-K Program at this site. Attendance was observed today. I have observed the NC Pre-K children’s files today and the following items were observed: Health Assessments Immunizations Vision Screenings Hearing Screenings Dental Screenings This site currently uses TSG for their Developmental Screening. I have observed a sampling of these today. This site currently uses Creative Curriculum for their Curriculum. I have observed the lesson plans today. This site currently uses Dial 4 for their Formative Assessment. I have observed a sampling of these today. Staff have stated today that family engagement activities include home visits, homework with parents, a newsletter, parents coming to the classroom to participate and celebrations (fashion show recently). The teachers stated that they work 40 hours per week. Lead teachers have BS in Sociology, BS in Child Development. All have Birth through Kindergarten Licensure. Assistant Teachers have AAS degrees in Early Childhood, and getting AAS degrees in Early Childhood. This program does not provide transportation. I documented the following violations during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The lesson plan posted in space 4 was dated for last week. GS 110-91(12); .0508(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet indoors and two outlets outdoors were within reach of children and were not covered. 10A NCAC 09 .0604(c) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. This visit was conducted with Ms. Natasha Simmons, Director. This program currently operates with a five star license. The license was posted and the restrictions were being followed. The program’s compliance history was 94% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least 75%. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit, the children were playing indoors and outdoors, having Spanish lessons and taking school photos. One teacher got out puppets for her classroom children that were put away in the cupboard during the visit. The last annual compliance visit was conducted on 10/12/2022. The last sanitation inspection was conducted on 05/12/2023. The last fire inspection was conducted on 08/31/2023. A fire drill was documented on 09/06/2023. A shelter in place drill was documented on 09/12/2023. An outdoor inspection was documented on 09/12/2023. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. This program does not provide transportation. All NC Pre-K requirements were monitored during this visit. Ms. Simmons reports that the dental screening is scheduled for November 17, 2023. I documented the following violations during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There were several items within reach of children on playground two, including a can of Monster drink, the gate portion of a fence, pieces of a broken flower pot and a broken plastic tube. These were removed during the visit. 10A NCAC 09 .0601(a) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I provided the following Technical Assistance during today’s visit: I have discussed the emergency medications and medications with Ms. Simmons and we have talked about the information that should be on the posted allergy list. All medicine and emergency medicine was properly stored today and all has the proper paperwork. Ms. Simmons and I have discussed the Staff and Training worksheet and she has filled it out according to the directions. Her staff and children’s files are in order. Ms. Simmons has completed new child care enrollment forms and stated that teachers will use them beginning tomorrow. One new staff person will continue to complete trainings in accordance with the rules. I have reviewed this visit summary with Ms. Simmons at the conclusion of the visit and a copy was given to her.
