    Name of Operation: Epiphany ELC Facility ID: 34001457 Consultant: TAMMY CHILDRESS Operation Type: Family CC Home Case Number: Visit Date: 6/3/2026 Number Present: 3 Completed Date: 6/3/2026 Age: From 1 To 4 Total Minutes: 130 Time In: 12:40 PM Time Out: 02:50 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor your program for compliance with all applicable child care requirements during the first temporary time period visit. You, Danielle Ingram, assisted me with the visit. This facility issued a temporary license on April 17, 2026, with restrictions of first and second shifts. During this visit, I observed and monitored the indoor and outdoor environment, and completed a full assessment of the licensed space. Children were observed engaged in free play activities, personal care routines/diapering/handwashing, lunch and rest time. Program records, including fire drills and monthly outdoor area inspections and enrolled children’s records were monitored. There were no changes to your staff file. Children’s attendance and arrival and departure times are documented in the Brightwheel app. The following violations were observed today and must be corrected immediately. Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. Neosporin packets and an enrolled child's topical (skin) medication were stored on a shelf that was five feet above the floor but not in locked storage. .1719(a)(5) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Aerosol bottles including Banana Boat Sunscreen, Trek Kids Insect Repellant, and Glade were not in locked storage. .1719 (a)(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 17, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware, any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Email the information to me at tammy.childress@dhhs.nc.gov. The compliance letter must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Technical Assistance Provided During Today’s Visit: Cleaning wipes, aerosol sunscreen spray and insect repellant spray, glade aerosol spray, Neosporin and an enrolled child’s over the counter cream was not in locked storage. When any item is in an aerosol, it must be in locked storage when not in use. Neosporin cream from the first aid kit cannot be administered to children as needed. All medication administered must be accompanied by permission by the parent with specific instructions for when the medication will be used, how much will be used, and where it will be used and the length of time the medication will be administered. Medications authorized by parents to be administered to children must be in locked storage. Locked storage prevents children from accessing the medication or hazardous item and inhaling or ingesting the contents which can cause serious health related issues. You will need to get a lock box or magnetic/key/electronic lock device to store the hazardous items and another device for medication. Consultation Provided During Today’s Visit: Rated License Discussion- During the pre-licensing and initial assessment visits and during today’s visit we reviewed QRIS Modernization rules and processes, “QRIS Pathway to the Stars, Where Quality Meets Choice” for Family Child Care Home operators during your pre-licensing visit and again today. Today, we reviewed each of the Pathway options and the requirements within each option for star ratings. You will need to take additional time to review this information, and if you feel that you would like to apply for a 2-5 Star Rated License prior to the expiration of your Temporary License, you will need to submit to me a Rated License Application no later than July 1, 2026, with the pathway option you have selected indicated. As a newly licensed operator, I encourage you to contact the Smart Start of Forsyth County at (336)725-6011 and/or Child Care Resource Center at (336)245-4900 for assistance in preparing for a star rated license. You are already working with Laura Brown, Early Childhood Coach, Child Care Resource Center, regarding the QRIS Pathways. You should visit the North Carolina Rated License Assessment Project (NCRLAP) website at ncrlap.org and review information for the Family Child Care Environment Rating Scale 3 (FCCERS-3) and the “FCCH Self-Study process.” The FCCERS-3 and the self-study are required for Pathway 1- Program Assessment for Family Child Care Homes (FCCH). Documents can be accessed there for you to use to complete the self-study (in the “FCCH Self-Study” blue box option) if you choose this pathway. In addition, you will need to obtain a copy of the spiral bound, green booklet entitled “Family Child Care Environment Rating Scale Third Edition.” You may order a copy online by google searching the name of the book or ask your local smart start office if there is a copy of the booklet that you can use for self-study. The NCRLAP website also has excellent resources and training that will be helpful for you. We reviewed education requirements for each star rating. You are currently meeting education requirements for the Five Star level. You are using the Creative Curriculum and Teaching Strategies Gold. You will visit the creative curriculum website for training on the use of this curriculum. If you experience any issues, please let me know. On-Going Professional Development In addition to the required Health and Safety Trainings, you can access professional development training on Moodle under Early Childhood Professional Development in the categories of Child Care and Development Fund (CCDF), Orientation and Regulatory. Specifically, under the Regulatory section, I would encourage you to complete the training for “Child Development Module.” This training module provides a basic overview about child development and is designed to help early educators support infants, toddlers, and preschool children to reach their next developmental milestone. In addition to gaining introductory knowledge about child development, early educators learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. Upon completion, you will receive one (1) contact hour credit (CHC). At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you.
