This visit was conducted by Jennifer Davis, childcare consultant with Zach Baker, administrator. Your program currently operates with a Five Star license issued 10/23/2024. Restrictions include first shift, meets enhanced space and enhanced ratios minus one. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 6/1/2026 and was listed Evangelical Lutheran Church of the Epiphany as current and active. The program’s compliance history was 89% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children outside playing, in routine care and free play. I observed that developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses the Creative Curriculum. The last annual compliance visit was conducted on 6/9/2025. A sanitation inspection was completed 4/10/2026 with a Superior classification. The last fire inspection was conducted on 5/12/2026 Program records and required postings were monitored. A fire drill was conducted on 5/19/2026. A lockdown drill was documented on 5/7/2026. An outdoor inspection was documented on 5/22/2026. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. Two new staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items was monitored today. A playground inspection was recorded 12/19/2025. A fire drill was recorded 12/17/2025. A lock down drill for the facility was recorded 12/16/2025. The most recent fire inspection was on 5/12/2025. I observed a sanitation inspection was last conducted on 9/16/2025 earning a Superior classification. The program does not provide transportation.     Name of Operation: EPIPHANY EARLY CHILDHOOD CENTER Facility ID: 34000157 Consultant: JENNIFER DAVIS Operation Type: Center Case Number: 0825-120L Visit Date: 8/15/2025 Number Present: 32 Completed Date: 8/15/2025 Age: From 1 To 5 Total Minutes: 175 Time In: 09:35 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Self Report Announced/Unannounced: Unannounced The purpose of today’s visit was to follow-up on the concerns regarding the self-reported information you shared with the Division of Child Development and Early Education. The visit today was conducted with Lisa Nichols, administrator. On 8/6/2025 an alleged incident was brought to administration. There is a concern regarding inapprpropriate discipline. Prior to the visit, I spoke to the administrator by phone on August 8, 2025, regarding the self-report. The administrator stated that the center does not have video cameras in the classroom spaces. I interviewed three staff members and observed paperwork regarding the incident. Staff stated that a child, age two, was spitting milk in the classroom for one and two year-old children. A staff member firmly held the face of a child , age two, looked at the child in a stern manner and in a harsh tone stated “No Spitting. the child by the face and used a harsh tone, telling the child to stop spitting. The staff member put the child in timeout for a short time. Two staff members observed red lines on the cheeks of the child after the incident. Staff stated that there were no previous concerns related to rough handling with this staff member. The administrator stated that the staff member was terminated. The administrator stated that she met with the parent. She also stated that a staff meeting was being held today and on the 19th to review policies and procedures. Staff reported that there was offered outside support assistance if any staff member needed it. The administrator has requested technical assistance from Smart Start. I observed the discipline policy and the center’s operational policies during the visit today. The following violations were cited during the visit today: Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident report was not completed for a child that had red marks after a staff member grabbed the child's face. .0802 (e) 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. Two staff members observed red lines on the cheeks of a child, age two, after a staff member firmly held a child's face. .1803(a)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Administrative staff should continue conducting routine walk-through to be proactive in identifying staff burnout and offer staff members time to step out of the classroom. The following violations were cited during today’s visit: Violation Number Comment Rule 707 Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. In Space 3, two battery-operated toys that were not working were not removed or made inaccessible. .0601(d) 807 A safe indoor and outdoor environment was not provided for the children. A bush that had been cut back was sticking up causing a tripping hazard. 10A NCAC 09 .0601(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Restrictions include first shift, meets enhanced space and enhanced ratios minus one. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 7/17/2023 and was listed Evangelical Lutheran Church of the Epiphany as current and active. The program’s compliance history was 85% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in center free play and routine care. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses the Creative Curriculum. The last annual compliance visit was conducted on 7/27/2022. A sanitation inspection was completed 1/18/2023 with a Superior classification.
