Name of Operation: EPHESUS LEARNING CENTER Facility ID: 34000469 Consultant: KELLY HEALY Operation Type: Center Case Number: Visit Date: 5/15/2026 Number Present: 12 Completed Date: 5/15/2026 Age: From 0 To 4 Total Minutes: 180 Time In: 10:00 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced An unannounced visit was conducted at this child care today to monitor compliance with all applicable childcare requirements and to discuss the Rated License. This visit was conducted with Ms. Nicole Chiles, Administrator and Ms. Regin Marshall, Lead Teacher. This program currently operates with a Four Star license. Restrictions include first and second shift, children in rooms 5, 6, 7 and 8 age 2nd grade or above, meets enhanced space, meets enhanced ratios. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 05-15-2026. The program’s compliance history was 99% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing, eating and completing routines. The last sanitation inspection was conducted on 04-30-2026. The last fire inspection was conducted on 06-23-2025. A fire drill was documented on 04-23-2026. A shelter in place drill was documented on 04-23-2026. An outdoor inspection was documented on 04-23-2026. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. This program does not provide transportation. I documented the following violations during today’s visit: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. There are several forms missing from the staff records, as listed below. G.S. 110-91( 9) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Two children do not have the signed Shaken Baby form in their files. .0608(b)(1-6) I have asked Ms. Chiles to send me written documentation by May 29, 2026 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. I have reminded Ms. Chiles that if the documentation indicates that corrections have been made when they have not, this may be considered falsification of information and an Administrative Action may be recommended. In addition, repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date indicated in this visit summary could lead to the recommendation of an Administrative Action. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical Assistance: Ms. Marshall has made a list of each employee and number for the violation above. The specifications are as follows: Employees 2, 3, and 4 do not have an updated HQ on file. Employees 1, 2, and 4 do not have an updated Emergency Information form on file. Employees 1, 2 and 4 do not have proof that the Emergency Plan was reviewed with them in the last year. Employee 1 does not have proof of inservice training in the file, including Child Maltreatment and Health and Safety Trainings. These trainings are good for five years. Employees 1 and 3 do not have proper documentation of Orientation in the file. Consultation: I have observed the ABCMS information in Ms. Baker’s file, however, she will need to update the information. Ms. Chiles reports that the center is actively looking for a full-time teacher for the preschool room and has been conducting interviews. In the meantime, a substitute teacher is working and I have observed her file today. Rated License: Ms. Chiles and Ms. Marshall reported today that Ms. Baker has chosen a Pathway for the star system for the center to pursue, however they could not remember if it was Pathway 1 or 2. Ms. Marshall reported that she did not know Ms. Baker’s plan for a timeline to apply for a Rated License. I have observed all staff files today and the education listed in the Regulatory system. All of the education for each employee has been updated and sent to the Raleigh office. The information for the education in the Raleigh system is reflected in the staff files and no further action needs to be taken unless more education or degrees are achieved. If the center chooses Pathway 1 and wishes to have the ERS completed, they will need to call me and I will request it on their behalf. I have printed and reviewed this visit summary with Ms. Chiles at the conclusion of the visit and a copy was given to her. The Summary of the Law was not posted. G.S. 110-102 307 Only one caregiver was required to meet staff/child ratio, but the name, address and telephone number of agreed emergency relief was not posted or a second adult was not on the premises for emergency relief. The emergency information was not posted. .0713(a)(9) 802 Safe pick-up and delivery procedures were not communicated to parents and/or were not posted where they can be seen by the parents. The Safe Pick Up and Delivery was not posted. .1003(b) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Consultation: North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS portal on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Comments – Ms. Sharon Roey has contacted me and told me that she serves on the church board and she will be helping the center as they search for a new director. Ms. Roey will need to call the fire marshal and have him complete a visit as soon as possible. Ms. Roey has received the Staff worksheet and stated she will fill it out.
