This visit was conducted by Jennifer Davis, childcare consultant, with Linda Jarrett, Operator. Your program currently operates with a Four-star license. Restrictions include daytime and overnight care, maximum of five preschool children, and no more than two infants under age one. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 96% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed the enrolled child in an art activity and fine motor play. I observed developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 11/9/2022. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. Ms. Jarrett stated she is the only person living in the home. Be reminded that you must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 9/8/2023. A shelter in place drill was documented on 9/8/2023. An outdoor inspection was documented on 9/13/203. There are no pets reported in the home. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. .1714(e ) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. One enrolled child did not have on file that the policy was received. .1726(b)&(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before 10/27/2023. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to: jennifer.davis@dhhs.nc.gov If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance provided during today’s visit: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • You are in Cohort 1 for the rated license reasssment. • We discussed Ms. Jarrett’s restriction on the license to serve no more than two infants under age one. A copy of the shaken baby syndrome policy shall be given and explained to the parents of children up to five years of age on or before the first day the child receives care at the home.
