The visit was conducted with Lawanda Brown, operator, by Jennifer Davis, Child Care Consultant. Currently your home operates with a Four-Star License, issued May 24, 2024. Restrictions include daytime and overnight care, maximum of five preschool children at any time, and sunroom is not approved child care space. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. The following items were monitored during today’s visit: Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health • Administering of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. I observed positive interactions today. I observed children in free play during the visit. Limited monitoring of operator files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Special Training including Health and Safety Training and Criminal Background Checks. Household members were discussed during the visit today. Qualification letters were monitored and were current for the operator, her husband and three children. Three grandchildren also live in the home but are under age sixteen. Storage and administration of medication were monitored. There are no children enrolled currently taking medication during operating hours. Household medicine is kept in a locked box. Storage of hazardous items was monitored and found to be in compliance. A playground inspection was recorded on 9/18/2025 A fire drill was conducted on 9/23/2025 . A shelter-in-place drill for the facility was recorded on 7/7/2025. Ms. Brown stated she is not providing transporation. No violations were cited during today’s visit: Violation Number Comment Rule 1831 A valid qualification letter was not on file and available for review at the facility. Qualification letters for five(5) household members need to be printed and on file. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before October 22, 2025. I sent the QRIS application link during the visit today as well. - During the visit, we discussed planting on the playground in beds or containers so any grass or vegetation can be easily cleaned up. This facility was issued a temporary license on 3/11/2024 with restrictions of daytime and overnight care and a maximum of five preschool children. During this visit, I observed the indoor and outdoor child care areas. Children were observed engaged in free play activities, and personal care routines. There were four children present today, ages 1 to 5 years. Ms. Brown is currently operating first and second shifts. Ms. Brown completed a rated license application. Ms. Brown stated that she does not currently have any children on any medication. She keeps household medicine locked in the kitchen. I monitored program files, staff, and children’s files. Ms. Brown is not providing transportation. Your compliance history was 92%. All facilities must maintain at least 75% or above. The following violations were cited today: Violation Number Comment Rule 711 Outdoor stationary play equipment was not anchored and/or equipment was installed over concrete or asphalt. The outdoor swing set was not anchored. 10 A NCAC .1719(a)(22) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. Poison ivy was located on the playground and needs to be removed. .1719(a)(6) 2004 For off-premise activities occurring on regular basis, the standing permission for 12 months had expired. Four(4) erolled children had an expired off-premise permission. .1723(15)(b) 2030 Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. One enrolled child did not have a signed policy on file. .1719(a)(11) 2041 Application did not include the names and the phone numbers of the child's physician and the persons to be contacted in an emergency situation.
