This visit was conducted by Jennifer Davis, childcare consultant, with Brian Parrish, center director and Judy Parrish, teacher. Your program currently operates with a notice of compliance. The NC Secretary of State website was reviewed on 5/26/2025 and Cedar Forest Baptist Church, Inc. was listed as current and active. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in center play, art activities, and routine care. Staff were engaged and supervising adequately during the visit today. The last annual compliance visit was conducted on 5/27/2025. A sanitation inspection was completed 4/8/2025 with a Superior classification. The last fire inspection was conducted 6/6/2025. Program records and required postings were monitored. A fire drill was conducted on 5/8/2025. A lockdown drill was documented on 3/14/2025. An outdoor inspection was documented on 5/17/2025. I observed the playground today. There was six inches of mulch under swings and slide. I observed a large pile next to the playground that can be added as needed. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. There are currently no children on medications. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 603 All floors and floor coverings were not constructed of nonabsorbent material and/or were not kept clean and in good repair. The carpet in the main child care room had two large seams in the middle and near the dollhouse and slide that were coming apart and fraying and need repair. The boy's bathroom had a hole in the vinyl flooring and there was a large gap on the edge of the wall that was a large gap where the quarter round was removed and needs repair. 15A NCAC 18A .2824(a)&(b) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The staff was not connected to the facility roster in the ABCMS Portal. I observed a sanitation inspection was conducted on 10/16/2025 earning an Approved classification. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. I observed children eating a morning snack and group time. No new staff were reported at this program. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 11/15/2024. A fire drill was recorded 12/18/2024. A lock down drill for the facility was recorded 12/18/2024. The most recent fire inspection was on 6/5/2024. I observed a sanitation inspection was last conducted on 12/6/2024 earning an Approved classification. The program does not provide transportation. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in center play, group time, and routine care. Staff were engaged and supervising adequately during the visit today. The last annual compliance visit was conducted on 6/13/2024. A sanitation inspection was completed 1/ 19/2024 with a Superior classification. The last fire inspection was conducted 6/2/2023. An inspection had been scheduled for today. The pastor contacted the fireman today by phone during the visit to verify. He will send me the inspection today. Program records and required postings were monitored. A fire drill was conducted on 6/4/2024. A lockdown drill was documented on 5/1/2024. An outdoor inspection was documented on 5/14/2024. I observed the playground today. There was six inches of mulch under swings and slide. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The following violation was cited during today’s visit: Violation Number Comment Rule 1322 A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available.
