    Name of Operation: BAILEY PRESCHOOL Facility ID: 34001394 Consultant: TAMMY CHILDRESS Operation Type: Center Case Number: Visit Date: 9/18/2023 Number Present: 3 Completed Date: 9/18/2023 Age: From 4 To 4 Total Minutes: 150 Time In: 11:30 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance with all applicable child care requirements during the second temporary time period visit and to discuss preparation for a rated license assessment. The visit was conducted with Dennette Bailey, Owner/Operator, by Tammy Childress, Lead Child Care Consultant. Currently this facility operates with a Temporary License, issued on June 1, 2023, on first and second shifts for children ages 2 to 8 years old. Other restrictions include no more than 4 children in Space 1, 5 children in Space 2, 12 children in Space 3, and lunches must be brought from the upstairs kitchen by James Bailey. Ms. Bailey operates as the sole proprietor of the business. If any changes need to be made or you decide to sale your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. All designated indoor and outdoor spaces for child care were monitored, as well as all children’s files and Ms. Bailey’s file. Ms. Bailey is the sole caregiver for the children. All program records and documents required to be posted were monitored. The facility does not provide transportation. Currently, parents provide all meals for children and Ms. Bailey provides pre-packaged individual snacks and beverages. All enrolled children have a signed nutritional opt out form on file. There is a refrigerator in Space 1, approved by environmental health, where children’s meals are stored. A thermometer read 40 degrees Fahrenheit. Ms. Bailey may prepare meals ahead of opening, which can be brought downstairs, as stated above. There are currently three children, all ages four years old, and there are no children enrolled on second shift. If children are enrolled for second shift care, a daily schedule and activity plan must be posted, fire drills and shelter-in-place or lock down drills must be conducted, children must have cots/mats available or resting or sleep. All requirements applicable on first shift remain the same for second shift care. If school age children are enrolled, create an activity plan and daily schedule applicable for this age group. Ms. Bailey uses the creative curriculum. During today’s visit children present were observed to be resting/ napping and having snacks. Ms. Bailey currently follows staff/child ratios and group sizes as required by Child Care Rule 10A NCAC 09 .0713(b): Age of Children Ratio Staff/Children 0 to 12 Months 1/5 preschool children plus three additional school-age children 12 to 24 Months 1/6 preschool children plus two additional school-age children 2 to 13 Years 1/10 3 to 13 Years 1/12 All school-age 1/15 The following violations were cited today: Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. The health questionnaire on file for the operator was dated August, 2022. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Emergency Information on file for the operator was dated August, 2022. .0701(a) All violations were corrected during the visit today. During today’s visit we again discussed the Rated License Assessment Process and the Environment Rating Scale Assessment. We reviewed the program and education components for a star rated license, how points are earned, and the Environment Rating Scale Assessment. The application for a Star Rated License Assessment was completed during the visit. Ms. Bailey stated that she did not wish to request the Environment Rating Scale Assessment at this time. If you decide to request the ERS Assessment, you must notify Ms. Childress by September 29, 2023, and you should prepare for the ERS Assessment by reviewing the Early Childhood Environment Rating Scale (revised edition) booklets, visit the North Carolina Rated License Assessment Project website at www.ncrlap.org and review resource guides/additional notes/ informational videos and participate in the free online training. In order to continue to participate in the NC Subsidized Child Care Program, you must be issued a license with Three Stars or higher. The Rated License Assessment will include a review of the following to determine points in each component: Program Standards- 2 Points Ms. Bailey wishes to meet enhanced space requirements, allowing for 30 square feet per child. We reviewed the numbers of children that can be in each space at 30 square feet per child ( Space 1=4, Space 2= 5, Space 3=12). Education- 7 Ms. Bailey’s WORKS Qualification Letter a BA/BS or higher in Early Childhood/Child Development, is a Level III Administrator, has the Family Childcare Credential, is Lead Teacher Qualified, and is an ECE Sale Level 12, and SA Level 12. Ms. Bailey earns 7 points in the Education component. Quality Point- 1 Point The Quality Point option(s) were reviewed with Ms. Bailey today and she selected the options below: • 75% of Lead Teachers have a BA/BS or higher in EC/CD • 75% of the Lead Teachers and Teachers have at least 10 years of documented early childhood work experience that can be verified by the Division. Ms. Bailey has been a licensed FCCH provider since 1998 and has worked with Winston-Salem Forsyth County School system as a teacher. • Staff Benefits Package (per Ms. Bailey’s approved Personnel Handbook, she provides all benefit options) and Infrastructure of Parent Involvement (Per Ms. Bailey’s approved Operational Policies she provides a Quarterly Newsletter, has periodic conferences for all children, and has quarterly parent meetings). Technical Assistance: Playground Safety Training Ms. Bailey must complete Playground Safety training within 6 months of licensure. She stated that she completed the final training session on September 14, 2023, but has not yet received her certificate. She will place the certificate on file once received. Health and Safety Training Ms. Bailey, as a previously licensed Family Child Care Home provider, has completed all Health and Safety. The trainings must be repeated every five years with training certificates on file for review. If new staff are hired, they must complete the trainings within one year of hire. These trainings can be accessed on the DCDEE website at https://ncchildcare.ncdhhs.gov under the Provider tab from the home screen, select Training and Professional Development, and finally DCDEE Moodle. It is important to remember that the child care administrator and all staff members must complete the specific Health and Safety Training called “Recognizing and Responding to Suspicions of Child Maltreatment”, within 90 days of employment. This training can be accessed through Moodle or directly at https://www.preventchildabusenc.org/services/trainings-and-professional-development/rrcourse. Emergency Preparedness and Response Plan Emergency Preparedness and Response Training has been completed. Ms. Bailey has updated successfully her EPR Plan but had not printed out the updated cover page. She entered the plan template and showed Ms. Childress the date the plan was updated, July 12, 2023, and the new license number was attached. Ms. Bailey will print the cover page and page 7, which shows the new license number. All other pages of the plan remains the same information that was already entered and have not been edited. A summary of today’s visit was reviewed with and provided to Ms. Bailey. Ms. Bailey was given the opportunity to ask any questions.
