This visit was conducted with Dwanna Agnew, operator. Your program currently operates with a Three Star license. Restrictions include daytime and overnight care, maximum of five preschool children at any time, children on ground level only, and no more than two infants under age one. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 90% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it was working properly. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed the child in free play and routine care. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on 7/17/2025. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. I have asked Ms. Agnew who lives in the home and have recorded his/her answer on the Verification of Required Information for Operator and Additional Caregiver Form with his/her signature. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 6/11/2026. A shelter in place drill was documented on 4/10/2026. An outdoor inspection was documented on 6/11/2026. There are no pets reported in the home. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Ms. Agnew is currently on the food program. Storage of hazardous items was monitored today. No medications were observed nor reported. Household medication/hazardous products are locked in the garage. The program does provide transportation. You stated that you do routine transportation in the mornings and afternoon for one child. During today’s visit, I monitored one vehicle for transportation. The vehicle was not current on inspection but insurance was current as documented on the FCCH verification form. Ms. Agnew stated that she had some repair work done on the vehicle and a light was on and needed to be checked by the repair shop again. I stated to Ms. Agnew that she is not approved for transporation until the vehicle has an approved inspection. Restrictions include daytime and overnight care, children in care on ground level only, maximum of five preschool children at any time and no more than two infants under age one. The license was observed and the restrictions were found in compliance. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. One child, age two, was present during the visit. Ms. Agnew is currently operating on first shift. I observed positive interactions today. I observed children in free play, routine care, and teacher directed play. I observed developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 8/7/2024. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. Ms. Agnew stated that she was not currently transporting but would be transporting when school starts back in August. The following violations were cited during today’s visit: Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. One propane tank was accessible in the playground area. .1719 (a) (1) 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. Household medication was in a cabinet not in locked storage. .1719(a)(5) 709 Equipment and toys were not in good repair and developmentally appropriate. This visit was conducted by Jennifer Davis, childcare consultant with Dwanna Agnew, operator. Your program currently operates with a Three Star license. Restrictions include daytime and overnight care, children in care on ground level only, maximum of five preschool children at any time and no more than two infants under age one. The license was observed and the restrictions were found in compliance. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. One child, age two, was present during the visit. Ms. Agnew is currently operating on first shift. I observed positive interactions today. I observed children in free play, routine care, and teacher directed play. I observed developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 8/7/2024. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. Ms. Agnew stated that she is the only person living in the home.Be reminded that you must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 6/30/2025. A shelter-in-place/lockdown drill was documented on 4/10/2025. An outdoor inspection was documented on 6/30/2025. Ms. Agnew has one pet that regularly visits her home. She has current paperwork and vaccintations on file. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does provide routine transportation during the school year. During today’s visit, I observed the vehcile used for transportation had an expired inspection tag. Ms. Agnew stated that she was not currently transporting but would be transporting when school starts back in August. The following violations were cited during today’s visit: Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. One propane tank was accessible in the playground area. .1719 (a) (1) 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. Household medication was in a cabinet not in locked storage. .1719(a)(5) 709 Equipment and toys were not in good repair and developmentally appropriate. This visit was conducted by Jennifer Davis, childcare consultant with Dwanna Agnew, operator. Your program currently operates with a Three Star license. Restrictions include daytime and overnight care, children in care on ground level only, maximum of five preschool children at any time and no more than two infants under age one. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 93% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. One child, age two, was present during the visit. Ms. Agnew is currently operating on first shift. I observed positive interactions today. I observed children in routine care and teacher directed play. I observed developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 9/14/2022. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. Ms. Agnew stated that she is the only person living in the home.Be reminded that you must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 8/11/2023. A shelter in place drill was documented on 8/11/2023. An outdoor inspection was documented on 8/11/2023. There are no pets reported in the home. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does provide transportation. You stated that you do provide routine transportation to and from school for two children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported. I observed a first aid kit and fire extinguisher on the vehicle. I monitored one vehicle today. I observed a 2016 Ford Expedition (RDM-2049). This vehicle was clean and in good repair, had working seatbelts, had adequate tire tread, and was current on inspection and insurance. Your next inspection date on this vehicle is due and the insurance is valid through 11/16/2023. The following violations were cited during today’s visit: Violation Number Comment Rule 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. Two children did not have a photograph on file and emergency information on file. .1723(13) 1713 The written plan of care did not specify times for completing routine tasks and did not include those times on the written schedule. Four enrolled children need updated times on the written plan of care. .1712(e)(1) 1715 The written plan of care did not specify how the operator will maintain compliance with transportation requirements specified in Rule .1723 when children are transported. Four enrolled children need updated operator transportation requirements. .1712(e )(3) 1717 The written plan of care did not specify any other steps the operator will take to ensure routine tasks will not interfere with the care of children. Four enrolled children need specified informaiton on written plan of care. .1712(e )(5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before September 13, 2023.
