This visit was conducted with Johane Jacaman, Director. This program currently operates with a four-star license. Restrictions include daytime care, enhanced space and enhanced ratios, and children under age 2 ½ in rooms with direct exits. The NC Secretary of State website was reviewed on 6/4/25 and Hy-Tec of Winston-Salem, Inc. was listed as current and active. The program’s compliance history was 99% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children engaging in free play, routine activities, and group time. One new staff was reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items was monitored today. A playground inspection was recorded 06/10/2025. A fire drill was recorded 06/19/2025. A lock down drill for the facility was recorded 04/17/2025. The most recent fire inspection was on 12/16/24. I observed a sanitation inspection was last conducted on 1/28/25 earning a superior classification. You stated that you do routine transportation in the morning and afternoon and summer field trips for school-age children. During today’s visit, I observed a first aid kit and fire extinguisher on both vehicles. I monitored two vehicles today. I observed a 2003 Ford Econoline, HMH-3800. This vehicle was current on inspection and insurance. Your next inspection date on this vehicle is due 01/2026 and the insurance is valid through 03/01/2026. I also observed a 2007 Thomas Build Minibus, TLZ-3088. This vehicle was current on inspection and insurance. Your next inspection date on this vehicle is due 06/2026 and the insurance is valid through 03/01/2026. The following violation was cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. During the visit, I observed exposed stakes on the balance beam on the large playground. 10A NCAC 09 .0601(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before 7/30/25. Technical assistance discussed during today’s visit related to violations: • We discussed that all employees must complete CPR and Fist Aid training within the first ninety (90) days of employee and that the training must be completed in person. Online CPR and First Aid cannot be accepted. There is a list of approved CPR and First Aid trainers on the DCDEE website. • We discussed that stakes must be flush with the balance beam at all times to ensure children are not exposed to a tripping or falling hazard. You made the stakes flush during the visit. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • For the latest updates on the QRIS Modernization and House Bill 412, please keep a check on the Raise NC Newsletters where updates will be provided as information is put together. • On June 26th, the North Carolina Rules Review Commission (RRC) approved the QRIS Modernization rules adopted by the NC Child Care Commission during their April meeting. The Division of Child Development and Early Education is preparing a summary of the rules, an online training module for Moodle, and an implementation plan. A webinar will be scheduled later to share the implementation plan and will also provide resources that can assist you with understanding the new rules. The implementation plan will allow time for administrators/operators to learn and understand the different pathways and the changes within our system. We encourage you to visit the QRIS Modernization page on the DCDEE website to learn more now. • Last week Governor Stein signed HB 412 into law, which became Session Law 2025-36. The law includes revisions to several sections within Chapter 110 of the NC General Statutes regarding child care and early childhood education. It requires DCDEE to develop and propose a possible plan to separate the quality rating improvement system (QRIS) from the requirements and payments for participation in the Subsidized Child Care Assistance (SCCA) Program. Some of the wide-ranging regulatory options and changes include: lead teacher requirements and responsibilities, new education equivalencies, space and equipment requirements in a public school serving school age children, group sizes for centers, and exemptions for Department of Defense certified family child care homes. In addition, the law addresses child care business-related issues such as residential building code and liability insurance. DCDEE is working to bring our staff and partners up-to-date with the new law and will update our materials and website as soon as possible. You stated that you do routine transportation in the morning and afternoon for school-age children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed a first aid kit and a fire extinguisher on the vehicle. I monitored one vehicle today. I observed a 2003 Ford Econoline, HMH-3800. This vehicle was clean, had working seatbelts, had adequate tire tread, and was current on inspection and insurance. Your next inspection date on this vehicle is due 01/2025 and the insurance is valid through 03/01/2024. The following violation was cited during today’s visit: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. This visit was conducted by Whitney Parker, Childcare Consultant, with Johane Jacaman, Director. This program currently operates with a four-star license. Restrictions include daytime care, enhanced space and enhanced ratios, and children under age 2 ½ in rooms with direct exits. The NC Secretary of State website was reviewed on 09/21/2023 and Hy-Tec of Winston-Salem, Inc. was listed as current and active. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. I observed an outlet that did not have a safety plug in space two. This was corrected during the visit. I observed a lattice tunnel on the small playground that is showing signs of wear. You stated this piece was being removed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children engaging in free play, routine activities, and group time. Two new staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items was monitored today. A playground inspection was recorded 09/08/2023. A fire drill was recorded 09/15/2023. A lock down drill for the facility was recorded 07/12/2023. The most recent fire inspection was on 01/03/2023. I observed a sanitation inspection was last conducted on 01/26/2023 earning a superior classification. You stated that you do routine transportation in the morning and afternoon for school-age children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed first aid kits and fire extinguishers on the vehicle. I monitored two vehicles today. I observed a 2003 Ford Econoline, HMH-3800. This vehicle was clean, had working seatbelts, had adequate tire tread, and was current on inspection and insurance. I observed a broken arm rest with torn fabric. Your next inspection date on this vehicle is due 01/2024 and the insurance is valid through 03/01/2024. I observed a 2007 Chevrolet Express, TLZ-3088. This vehicle was clean, in good repair, had working seatbelts, and was current on inspection and insurance. I observed the back tires had some cracking and the tire tread was getting low. Your next inspection date on this vehicle is due 01/2024 and the insurance is valid through 03/01/2024. The following violation was cited during today’s visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. I observed one outlet that was not covered by a safety plug in space two. 10A NCAC 09 .0604(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation cited during today’s visit was corrected during the visit. No letter is due. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance provided during today’s visit: •You must notify your consultant 30 days prior to changing the ownership status of your facility. •Repair the broken arm rest on the Ford Van. •Keep a check on the fire extinguisher in the minibus to ensure it is fully charged at all times. •Keep a check on the back tires of the minibus. They are beginning to show signs of cracking and the tread is getting low. •Make sure all outlets stay covered at all times. •Make sure that all medication forms document the expiration date of the medications. •Make sure to check medication permissions regularly.
