E. Pagara, Lead Teacher, and T. Sutton, Assistant Teacher, assisted me with today’s visit. Your program currently operates with a five-star license, issued December 13, 2023, earning 7 points in the education component, 7 points in the program standards component (meeting enhanced space and reduced ratios) and 1 quality point for 75% for offering a staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum (Creative Curriculum with Teaching Strategies Gold) as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on May 1, 2025. The sanitation inspection was completed October 23, 2025, with a “Superior” classification. The last fire inspection was conducted on December 16, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety percent as of April 14, 2026. Bertie County School is listed as the current owner of this program. Prior to making any changes in the ownership status, contact me to discuss changes in ownership procedures. I visited the indoor and outdoor space used by children. Children were engaged in choice play activities at the table which also included a writing activity, completing morning routine activities, completing toileting and hand-washing routines, circle time, interacting with teachers, and eating breakfast. Proper hand-washing procedures were observed. Breakfast included Chex cereal, dried cranberries, grape juice, graham crackers, and milk. Children also participated in fire drill during today’s visit. Children also participated in a food tasting activity which included bananas, lemons, tomatoes, lime, cheese ham, kiwi, pickles, strawberries, honey, and crackers. Children were asked to determine how each item tasted. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was not reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance The current NC Pre-K staff are lead teacher, Emilda Pagara, and teacher, Thelma Sutton. A selection of files was monitored for complete health assessments and developmental screenings. Completed Brigance screenings were observed in children files reviewed. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was 8-25-25. The classroom operates from 8:00am. to 3:00 p. The environment rating scale score received was 5.3. The following violations were documented today. Hand-washing did not occur prior to eating breakfast and before children completing a tasting activity with food items. 15A NCAC 18A .2803(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Lysol and cleaning spray in aerosol dispensed containers were in an unlocked room connected to the pre-K classroom. The door to the room was left open and accessible to children for five minutes during observation. .2820(b) 843 A drug or medicine was administered after its expiration date. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. On or before April 28, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Children hands were not washed prior to being served/eating breakfast and engaging in a food experiment conducted today. Staff should ensure hand-washing occurs when required according to the child care requirements which include prior to meals. Ongoing practice of hand-washing routines should be implemented to ensure compliance is always maintained. MEDICATION REQUIREMENTS: Two medications kept onsite for children had expired. The school nurse indicated the medication would be kept until a replacement is provided. Additionally, you and the school nurse stated the parents have also been notified about the medication and that it needed to be replaced. You stated parents would be contacted again today to request replacement medication. It was suggested having staff periodically check the medication to monitor expiration dates so parents can be notified about the replacements in a timely manner. HEARING SCREENING: A hearing screening was not on file for one preschool child. The health assessment indicated the hearing screening was attempted; however, no follow-up was observed. You stated you would contact the necessary individual regarding completion of the hearing screening. I recommend having staff check health assessments upon receipt for completion of screening and creating a plan to work with families when they have not been done. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. S. Watford and E. Pagara, lead teachers assisted me with the visit. B. Holley, Director of Early Learning and NC Pre-K arrived during the visit. Your program currently operates with a five star license, issued 12/13/23, earning 7 points in the education component, 7 points in the program standards component(meeting enhanced ratios plus reduced ratios and space) and 1 quality point for having staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 5/23/24. The sanitation inspection was completed 12/3/24 with a “Superior” classification. The last fire inspection was conducted 3/31/25 and your facility was approved for day time care only. The center's compliance history was reviewed with the operator. The program’s compliance history was percent as ninety-two of 5/1/25. The facility is owned by Bertie County Schools. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. I visited all indoor and outdoor spaces. In classroom space #14 children were observed transitioning to the outdoor space. Children rode tricycles, ran, and played basketball. Later children were observed during free play in designated learning areas. Transition to lunch was observed, lunch included cheeseburger, baked potato, string beans, tropical fruit and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The center uses the Teaching Strategies Gold instrument/Smart Teach to document evidence of children's ongoing progress. Communication through chat messages with families were observed, staff had parent contact logs available. The following violations were documented. Later when I enter classroom space #13 one staff member was caring for 18 children four to five years of age. Later one staff member left classroom space #13 to go get a copy of the menu, leaving one staff member to care for 18 children four to five years of age. .3009 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. S. Watford and E. Pagara, lead teachers assisted me with the visit. B. Holley, Director of Early Learning and NC Pre-K arrived during the visit. Your program currently operates with a five star license, issued 12/13/23, earning 7 points in the education component, 7 points in the program standards component(meeting enhanced ratios plus reduced ratios and space) and 1 quality point for having staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 5/23/24. The sanitation inspection was completed 12/3/24 with a “Superior” classification. The last fire inspection was conducted 3/31/25 and your facility was approved for day time care only. The center's compliance history was reviewed with the operator. The program’s compliance history was percent as ninety-two of 5/1/25. The facility is owned by Bertie County Schools. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. I visited all indoor and outdoor spaces. In classroom space #14 children were observed transitioning to the outdoor space. Children rode tricycles, ran, and played basketball. Later children were observed during free play in designated learning areas. Transition to lunch was observed, lunch included cheeseburger, baked potato, string beans, tropical fruit and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The center uses the Teaching Strategies Gold instrument/Smart Teach to document evidence of children's ongoing progress. Communication through chat messages with families were observed, staff had parent contact logs available. The following violations were documented. In classroom space #14 one staff member was caring for 17 children four to five years of age. Later when I enter classroom space #13 one staff member was caring for 18 children four to five years of age. Later one staff member left classroom space #13 to go get a copy of the menu, leaving one staff member to care for 18 children four to five years of age. .3009 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The facility uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was 8/26/24. The classroom operates from 7:45am to 3:00pm. Parent conferences are held three times per year. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. Upon arrival children were transitioning from breakfast to personal care routines and indoor free play. Breakfast consisted of cheese toast, apple juice, and milk. Developmentally appropriate toys were available on low-lying open shelves. After free play children transitioned to personal care routines and outdoor freeplay. Proper hand washing procedures were observed. The following violation was documented. Technical Assistance: Storage of hazardous products - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. Any product under pressure and any product that includes more than one warning must be in locked storage. In space 2 an aerosol can of Lysol spray was on the shelf beside the teacher’s desk. These items are required to be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. This was corrected during the visit. RESOURCES LEAD IN WATER/PAINT AND ASBESTOS The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation.
