Upon arrival, M. Roulhac, Family Service Worker, assisted me with the visit. You (L. Riddick) Administrator, arrived shortly after and assisted. Your program currently operates with a five-star license, issued April 8, 2026as part of the QRIS Pathway to the Stars – Pathway #3 (Accreditation & Head Start). The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent as of May 26, 2026. The NC Secretary of State website was reviewed on May 26, 2026, and Inc Choanoke Development Association of NC, Inc was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas used by children were monitored. Children were adequately supervised and capacity was met. Children were completing toileting and hand-washing routines interacting with teachers with teacher-directed activities and morning review. Breakfast was also observed. Breakfast consisted of sausage biscuits, 100% orange juice and milk. The posted activity plans included developmentally age-appropriate activities and were current. Artwork was observed posted in the classrooms that corresponded with the activities listed on the posted plans. The following violations were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A small bottle of hand sanitizer with the labeled” keep out of reach of children” on the teacher's desk was accessible to children. 10A NCAC 09 .0601(a) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. The authorization to administer medication form competed by the parent for an EpiPen kept on site for one child enrolled expired on April 6, 2026. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. On or before June 9, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. The family service worker reached out to the child’s primary care physician and parent to attempt to retrieve an updated form. It was suggested that documentation provided for medication be reviewed regularly to ensure all dates are current and accurate. Tracking expiration dates will help ensure parents can be notified when updates are needed to prevent medication documentation from expiring. Maintaining current documentation is important to ensure children can receive required medication while in care. SAFE ENVIRONMENT: A small container of hand sanitizer was on the teacher’s desk in space accessible to children. When notified the teacher removed the sanitizer and placed it in a space out of reach of children. Be reminded potentially hazardous items must be stored appropriately. Periodic checks of the environment prior to children arriving could assist with identifying and removing hazards and avoid safety concerns. ACTIVE ENGAGEMENT WITH CHILDREN: It was recommended that preschool children be actively engaged in meaningful activities while waiting to transition from one activity to another rather than remaining seated with no engagement. Children were observed sitting at tables for seven minutes while waiting for breakfast to arrive. Providing songs, movement activities, finger plays, storytelling, simple games, classroom jobs, etc. can help keep children and reduce potential challenging behaviors during transition times. Engaging children during transitions supports learning, promotes. Positive behavior and helps maintain a structured and nurturing environment. WAGE$ & STEPS: I provided you with information regarding two education-based salary supplement programs to help support the early care and education workforce. We talked about the options avaiable, and you were encouraged to share it with staff who would benefit from it. Instructions regarding applying were also included in the flyer. SEX OFFENDER REGISTRY: All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. (§14-208.19) To register for the e-mail notification, go to http://sexoffender.ncsbi.gov. If you have any questions, please contact your local sheriff's department. Upon arrival, M. Ambrose, Teacher Assistant assisted me with the visit. You arrive shortly after the visit started and assisted me. Your program currently operates with a five-star license, issued April 21, 2020, earning 7 points in the education component, 37 points in the program standards component (meeting enhanced space and reduced ratios and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. You stated the program continues to use the Creative Curriculum. I observed curriculum booklets, activity plans and other materials. The last annual compliance visit was conducted on February 11, 2025. The sanitation inspection was completed November 5, 2025, with a “Superior” classification. The last fire inspection was conducted on February 25, 2025, and your facility was approved for daytime care only. You provided me with a copy of the sanitation inspection report during the visit. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of January 14, 2026. The NC Secretary of State website was reviewed on January 14, 2026, and Choanoke Area Development Association of North Carolina, Incorporated was listed as current- active. I visited each indoor and outdoor space. Children were observed engaged in free choice play in both the indoor and outdoor environments. Children completed morning routines which included hand washing and toileting. Proper hand-washing techniques were observed. Breakfast was observed and consisted of mini bagels with strawberry cream, grape juice, and milk. The following violations were documented: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection report dated February 3, 2025, was not submitted to the Division within one week from completion. 10A NCAC 09 .0304(a) 807 A safe indoor and outdoor environment was not provided for the children. Boards were protruding from the decks on the front and back walkways posing a tripping hazard. Paint is peeling from the window seals on the four back entrance doors and exposing rust. 10A NCAC 09 .0601(a) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Written authorization from a child’s parent was not on file for a EpiPen kept on site. 10A NCAC 09 .0803(1)(a & b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. An EpiPen kept onsite for one preschool child was not returned to the parent within 72 hours after the expiration date of December 31, 2025. .0803(12) 1756 Enhanced staff/child ratios and group sizes were not met. Enhanced staff child ratio requirements were not met when one staff was observed providing care to eleven children ranging from ages three to five years old. 10A NCAC 09 .2818 9995 A violation was found for which there is no item number. The gates to the fence enclosing the playground were secured with a lock requiring a key that could not be accessed during the visit. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. On or before January 28, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. CORPORATE CONTACT/ADMINSTRATOR CHANGE: It was reported that the corporate contact for the agency and administrator at this facility has changed since the last visit completed. During our conversation, we discussed and reviewed the written documentation that must be submitted for these changes to be updated in our system. I left copies of the Preservice Administrators in Child Care Center and Legal Designee forms with you today. I shared the written documentation requesting the change must be completed on company letterhead and submitted either from the email address currently on file for the facility or via postal mail. You stated that you would contact corporate office to request that information immediately so the changes can be processed. Also, if the email address needs updating, it should be included in the written documentation. STAFF/CHILD RATIO REQUIREMENTS: While completing an observation in the classroom, one assisting staff member exited the classroom and went down the hallway. During this time, one teacher was left alone providing care for eleven children ranging from ages three to five years old. Your program is currently meeting reduced staff/child ratio requirements; therefore, may only care for no more than nine children at any given time. AS the youngest child enrolled is three years old, I recommended you talk with staff to review and reinforce the staff to child ratio requirements and develop strategies to implement to ensure compliance is always maintained. You said you would have a discussion with staff regarding staff/child ratios. Maintaining appropriate staff to child ratios is critical to ensuring the health, safety and supervision of children in care. Required ratios are established to ensure children are adequately always supervised, including transitions, arrival, and departure and when administrative staff are not on the premises. When ratios are not maintained, children may not receive appropriate supervision, which increases the risk of injury, behavioral incident, and unmet individual needs. Maintaining compliance with staff-child ratio requirements supports child safety, promotes positive interactions, and ensures the program remains in compliance with child care licensing regulations. A follow-up visit will be conducted in the future to monitor staff/child ratio and applicable child care requirements. MEDICATION REQUIREMENTS: Medication kept on site for one child did not have the required parental written authorization documentation on file. Also, the medication had an expiration date of December 31, 2025. We discussed reviewing all documentation for medication upon receipt to ensure required information and signatures are retrieved prior to having to administer it to children. I recommended contacting the parent to request medication with a current date and have the authorization paperwork updated. It was suggested you develop a system to track renewal dates for medication and conduct monthly medication checks to ensure none had expired. Be reminded if the medication does expire, it must be returned to the parents within seventy-two hours. SAFE ENVIRONMENT: During the visit, both gates on the fence enclosing the outdoor playground used by children were locked with devices requiring a key. One staff member reported that one of the locks had been tampered with and could not be opened with the key, resulting in no access to the lock that gate. You stated that the key to the lock on the second gate was not available or accessible. We discussed the safety concerns related to not having immediate access to unlock the gates in the event of an emergency. I explained that the back entrance and secondary exit doors lead directly to the playground; therefore, if children and staff were required to exit the building through the rear and transition to the front of the building, they would be unable to do so due to the locked gates. I further explained that this is a fire code regulation relating to means of egress on the fire inspection report (item # 27). You stated you would contact the main office to report on the issue and request the locks be changes/removed immediately. QRIS CONVERSATION: A meeting was conducted with all Head Start administrative staff to review and discuss the QRIS transition plan and pathways. During the meeting information was shared about the three pathways and requirements. Applications requesting completion for the QRIS Pathway to the Stars have been emailed to Head Start directors. The Directors were encouraged to complete the application and submit it when ready to begin the transition process. If additional information, guidance, or assistance is needed, please contact me to discuss. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. Your program currently operates with a five-star license, issued April 21, 2020, earning 7 points in the education component, 37 points in the program standards component (meeting enhanced space and reduced ratios and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. You stated the program continues to use the Creative Curriculum. I observed curriculum booklets, activity plans and other materials. The last annual compliance visit was conducted on February 11, 2025. The sanitation inspection was completed November 5, 2025, with a “Superior” classification. The last fire inspection was conducted on February 25, 2025, and your facility was approved for daytime care only. You provided me with a copy of the sanitation inspection report during the visit. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of January 14, 2026. The NC Secretary of State website was reviewed on January 14, 2026, and Choanoke Area Development Association of North Carolina, Incorporated was listed as current- active. I visited each indoor and outdoor space. Children were observed engaged in free choice play in both the indoor and outdoor environments. Children completed morning routines which included hand washing and toileting. Proper hand-washing techniques were observed. Breakfast was observed and consisted of mini bagels with strawberry cream, grape juice, and milk. The following violations were documented: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Maintaining appropriate staff to child ratios is critical to ensuring the health, safety and supervision of children in care. Required ratios are established to ensure children are adequately always supervised, including transitions, arrival, and departure and when administrative staff are not on the premises. When ratios are not maintained, children may not receive appropriate supervision, which increases the risk of injury, behavioral incident, and unmet individual needs. Maintaining compliance with staff-child ratio requirements supports child safety, promotes positive interactions, and ensures the program remains in compliance with child care licensing regulations. A follow-up visit will be conducted in the future to monitor staff/child ratio and applicable child care requirements. MEDICATION REQUIREMENTS: Medication kept on site for one child did not have the required parental written authorization documentation on file. Also, the medication had an expiration date of December 31, 2025. We discussed reviewing all documentation for medication upon receipt to ensure required information and signatures are retrieved prior to having to administer it to children. I recommended contacting the parent to request medication with a current date and have the authorization paperwork updated. It was suggested you develop a system to track renewal dates for medication and conduct monthly medication checks to ensure none had expired. Be reminded if the medication does expire, it must be returned to the parents within seventy-two hours. SAFE ENVIRONMENT: During the visit, both gates on the fence enclosing the outdoor playground used by children were locked with devices requiring a key. One staff member reported that one of the locks had been tampered with and could not be opened with the key, resulting in no access to the lock that gate. You stated that the key to the lock on the second gate was not available or accessible. We discussed the safety concerns related to not having immediate access to unlock the gates in the event of an emergency. I explained that the back entrance and secondary exit doors lead directly to the playground; therefore, if children and staff were required to exit the building through the rear and transition to the front of the building, they would be unable to do so due to the locked gates. I further explained that this is a fire code regulation relating to means of egress on the fire inspection report (item # 27). You stated you would contact the main office to report on the issue and request the locks be changes/removed immediately. QRIS CONVERSATION: A meeting was conducted with all Head Start administrative staff to review and discuss the QRIS transition plan and pathways. During the meeting information was shared about the three pathways and requirements. Applications requesting completion for the QRIS Pathway to the Stars have been emailed to Head Start directors. The Directors were encouraged to complete the application and submit it when ready to begin the transition process. You said you would have a discussion with staff regarding staff/child ratios. Maintaining appropriate staff to child ratios is critical to ensuring the health, safety and supervision of children in care. Required ratios are established to ensure children are adequately always supervised, including transitions, arrival, and departure and when administrative staff are not on the premises. When ratios are not maintained, children may not receive appropriate supervision, which increases the risk of injury, behavioral incident, and unmet individual needs. Maintaining compliance with staff-child ratio requirements supports child safety, promotes positive interactions, and ensures the program remains in compliance with child care licensing regulations. A follow-up visit will be conducted in the future to monitor staff/child ratio and applicable child care requirements. MEDICATION REQUIREMENTS: Medication kept on site for one child did not have the required parental written authorization documentation on file. Also, the medication had an expiration date of December 31, 2025. We discussed reviewing all documentation for medication upon receipt to ensure required information and signatures are retrieved prior to having to administer it to children. I recommended contacting the parent to request medication with a current date and have the authorization paperwork updated. It was suggested you develop a system to track renewal dates for medication and conduct monthly medication checks to ensure none had expired. Be reminded if the medication does expire, it must be returned to the parents within seventy-two hours. SAFE ENVIRONMENT: During the visit, both gates on the fence enclosing the outdoor playground used by children were locked with devices requiring a key. One staff member reported that one of the locks had been tampered with and could not be opened with the key, resulting in no access to the lock that gate. You stated that the key to the lock on the second gate was not available or accessible. We discussed the safety concerns related to not having immediate access to unlock the gates in the event of an emergency. I explained that the back entrance and secondary exit doors lead directly to the playground; therefore, if children and staff were required to exit the building through the rear and transition to the front of the building, they would be unable to do so due to the locked gates. I further explained that this is a fire code regulation relating to means of egress on the fire inspection report (item # 27). You stated you would contact the main office to report on the issue and request the locks be changes/removed immediately. QRIS CONVERSATION: A meeting was conducted with all Head Start administrative staff to review and discuss the QRIS transition plan and pathways. Your program currently operates with a five-star license, issued April 21, 2020, earning 7 points in the education component, 37 points in the program standards component (meeting enhanced space and reduced ratios and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. You stated the program continues to use the Creative Curriculum. I observed curriculum booklets, activity plans and other materials. The last annual compliance visit was conducted on February 11, 2025. The sanitation inspection was completed November 5, 2025, with a “Superior” classification. The last fire inspection was conducted on February 25, 2025, and your facility was approved for daytime care only. You provided me with a copy of the sanitation inspection report during the visit. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of January 14, 2026. The NC Secretary of State website was reviewed on January 14, 2026, and Choanoke Area Development Association of North Carolina, Incorporated was listed as current- active. I visited each indoor and outdoor space. Children were observed engaged in free choice play in both the indoor and outdoor environments. Children completed morning routines which included hand washing and toileting. Proper hand-washing techniques were observed. Breakfast was observed and consisted of mini bagels with strawberry cream, grape juice, and milk. Upon arrival twelve(12) children were present and finishing breakfast. Breakfast consisted of pancake, applesauce, milk. Children were observed participating in teacher directed activities, free play, and completing handwashing and personal care routines. A variety of developmentally appropriate toys were available and accessible on low-lying open shelves. The fenced outdoor play area consisted of a bike trail, large stationary equipment, area for shade, trikes, and a variety of balls. Lunch consisted of pepperoni pizza, corn, blueberry cup, and milk. The following violations were documented. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. This violation was corrected during the visit. Storage of hazardous products - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. Any product under pressure and any product that includes more than one warning must be in locked storage. An aerosol can of Hospital Disinfectant deodorant was stored in an unlocked cabinet in the hallway bathroom. These items are required to be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. This was corrected during the visit. Your program currently operates with a five(5)-star license, issued April 21, 2020. The last Annual Compliance visit was conducted 2/29/24. The sanitation inspection was completed 2/5/2025 with a “Superior” classification. The last fire inspection was conducted on 3/6/2024 and your facility was approved for daytime care. The program’s compliance history was ninety-eight percent as of 2/11/2025. The facility is currently owned by Choanoke Area Development Assoc. and is current-active. Please contact me prior to any information change regarding the owner of this facility. The contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed. It is important to frequently check your email to remain informed about any new changes that have or may occur. J. Bryant, Lead teacher, was available and able to assist me with the visit. Upon arrival twelve(12) children were present and finishing breakfast. Breakfast consisted of pancake, applesauce, milk. Children were observed participating in teacher directed activities, free play, and completing handwashing and personal care routines. A variety of developmentally appropriate toys were available and accessible on low-lying open shelves. The fenced outdoor play area consisted of a bike trail, large stationary equipment, area for shade, trikes, and a variety of balls. Lunch consisted of pepperoni pizza, corn, blueberry cup, and milk. The following violations were documented. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One electrical outlet located on the wall near the window was not protected by a safety cover. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. An aerosol can of Hospital Disinfectant deodorant was stored in an unlocked cabinet in the hallway bathroom. .2820(b) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance: Electrical Outlets - Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. One electrical outlet located on the wall near the window, was not protected by a safety cover. All electrical outlets not in use that are in areas accessible to children must be protected with an outlet cover. This includes unused outlets on power strips. This violation was corrected during the visit. Storage of hazardous products - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. Any product under pressure and any product that includes more than one warning must be in locked storage. An aerosol can of Hospital Disinfectant deodorant was stored in an unlocked cabinet in the hallway bathroom. These items are required to be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. This was corrected during the visit. Roulhac, Family Service Specialist was present and available to assist. Eleven (11) children age ranging from three (3) to four (4) years old were present. Children were observed in handwashing and toileting, outside play, teacher directed activities, and free choice activities. All staff interacted with children in a positive and nurturing manner. The center is owned by Choanoke Area Development Association of North Carolina, Incorporated. This corporation was found to have a Current/Active Status under the NC Secretary of State website. I used the Annual Compliance Monitoring Checklist and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility.
