    Name of Operation: TINABUG CHILDCARE LLC Facility ID: 98000220 Consultant: KESHIA HAYWARD Operation Type: Family CC Home Case Number: Visit Date: 6/25/2025 Number Present: 3 Completed Date: 6/25/2025 Age: From 2 To 4 Total Minutes: 115 Time In: 08:35 AM Time Out: 10:30 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor your program for compliance with applicable child care requirements during the routine unannounced visit. Vontina Fields, Owner/Operator, assisted with the visit. Three (3) children were observed in care today ranging from ages two to three years old. Children were engaged in free choice play with developmentally appropriate toys/materials, participating in outdoor play, completing toileting and hand washing routines and eating breakfast. Breakfast consisted of cereal, apple sauce, graham crackers and milk. You interacted with children during free play and outside time. You encouraged children to play together and share toys/materials. You demonstrated ways of playing in the sand. Behaviors were redirected in a positive and nurturing manner. Currently this program operates with a three-star License earning three points in education, three points in the program standards and one quality point for caring for no more than two infants under age one. The annual compliance visit was completed on January 23, 2025. The following violations were documented today. Violation Number Comment Rule 1409 Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. Equate antibacterial wipes, Clorox wipes, the operator’s purse, and white out were stored 5ft. below the finished floor in spaces accessible to children. 10A NCAC 09 .1719 (a) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator did not complete the ABCMS training as required. G.S. 110-90.2 & .2703(r) The violations documented must be corrected immediately. Please send a compliance verification letter to me describing how the violations were corrected. You can submit the compliance verification letter by postal mail to Keshia Hayward, PO Box 1504 Greenville NC 27834, or email keshia.hayward@dhhs.nc.gov. The compliance verification letter must be received on or before July 9, 2025. The two-week time frame is established to allow you time for submitting your compliance verification letter. This timeframe is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Please be aware that any written information submitted by you regarding correction of violations documented during the visit is legal documentation. Therefore, it is important that all information submitted be accurate and truthful. Please be aware that if it is determined the information submitted was inaccurate or willfully falsified, Administrative Action, including Revocation of the facility's Star Rated License, could be issued. If sufficient information is not received by the due date, a follow-up visit will be conducted. COMPLIANCE HISTORY: The program's compliance history was reviewed with the operator. The program’s compliance history was 100% as of June 24, 2025. OWNERSHIP STATUS: Tinabug Childcare, L.L.C. is listed as the current owner of this family child care home. Prior to making any changes to the ownership status, contact me to discuss the changes in ownership procedures such as forming a corporation, etc. FACILITY PROFILE INFORMATION: You stated the mailing address, phone number and email address listed for your program are correct. If changes in your facility information occurs, please contact me at 252-214-2709 or email keshia.hayward@dhhs.nc.gov to discuss the changes and ensure the information is updated in the system. SAFE ENVIRONMENT: During the visit, Equate Antibacterial wipes and Clorox wipes were observed in areas accessible to children. As a reminder, any potentially hazardous products labeled with “keep out of reach of children” must be stored at least 5ft. above the finished floor to ensure child safety. You stated the items were brought in by parents and you were not aware they had been placed in children’s cubbies. I advised you to remove these products and store them in an appropriate location that meets safety guidelines. Additionally, your purse and white out were also observed in a space accessible to children. Personal items must also be stored properly to prevent potential safety risks. We discussed the importance of reviewing labels, particularly on items brought into the child care facility to determine if they contain any hazard or safety warnings. This practice helps ensure that all items are stored in compliance with child care standards. To assist I provided the “Storage of Hazardous Product in Child Care” flyer which outlines best practices and guidelines for identifying and storing hazardous products appropriately. You removed the potentially hazardous products and placed them in spaces inaccessible to children during the visit. ABCMS TRAINING/REQUIRMENTS: You stated you did not complete the ABCMS training yet. You asked about the timeframe for completing the training. I informed you the training should have already been taken to meet the child care requirements for criminal background checks. I reminded you of the email that was sent out to providers on April 16, 2025, which included the link to access the training. You can also access the training on the Division’s website in the Moodle platform. Should you need assistance please contact the Criminal Background Check Unit at 919.814.6401 and someone will assist you. TRANSPORTATION REQUIREMENTS: You stated a vehicle was purchased to transport children. During today’s visit, I monitored a 2016 Honda Pilot. The first aid kit and fire extinguisher were in the back storage compartment in the vehicle. You provided proof of liability insurance, a copy of your valid driver’s license and inspection report. Based on my observation and information provided, all transportation requirements were met except for compliance with 2/32-inch tire tread. You stated you would purchase tires prior to starting transportation. I asked that you send verification of replacing tires on the vehicle. Once the tires are replaced, you can begin transporting children. HEAT SAFETY: To ensure heat safety in a child care setting, prioritize staying cool, hydrated, and informed, and be aware of the signs of heat-related illnesses. Move outdoor activities to cooler times, dress children in appropriate clothing, and provide plenty of fluids. Regularly check for signs of heat exhaustion or heatstroke and have a plan in place for extreme heat situations. Additionally, always check your vehicle when transporting children. Never leave children unattended in parked cars, even for a short time, as temperatures can rise rapidly. At the conclusion of today’s visit, the visit summary report was completed, reviewed, and left with you.
