Currently, this facility operates with a Five-Star license issued on September 22, 2017. The center's last annual compliance visit was completed on February 24, 2025. A checklist was used to note the requirements monitored during the visit. The last sanitation inspection was completed on September 9, 2025, earning a “Superior” classification and “5” demerits. The last satisfactory fire inspection was completed on October 8, 2025, and the facility was approved for day care. A.Riggins, facility Administrator, was present and available to assist during the visit. Fifty children were present during the visit. School aged children were present today since school was out due to inclement weather. Children in various groups were observed sleeping, resting, interacting with staff, and being fed. The school aged children were interacting with one another and their assigned staff member. The following violations of child care requirements were documented during the visit. Two (2) of the violations documented today were verified as corrected during the visit. When addressing the correction of the other two (2) violations, ensure the compliance letter states specifically how each violation has been corrected. You can mail or email me with verification of compliance regarding the violations that were documented. Submit the compliance letter on or before February 19, 2026. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The facility's EMC plan was not reviewed annually with all staff. 10A NCAC 09 .0802(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One (1) staff member did not renew First Aid training prior to the expiration date, 1/18/26. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One (1) staff member did not renew CPR training prior to the expiration date, 1/18/26. .1102(d) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. The facility's EPR Plan was not reviewed annually with all staff. .0607(f) During the visit, you emailed me a request for an extension for correction of the two (2) pending violations pertaining to CPR and First Aid training. You stated you are registered to complete CPR and First Aid training on February 23, 2026. All other staff have current CPR and First Aid training. Once you complete the training, obtain some type of documentation to verify the completion of the training. You stated you will be emailed the CPR and First Aid card, but you can obtain documentation of completion from the instructor after the training. You stated you were scheduled to complete both CPR and First Aid training on January 18, 2026, but the training was canceled due to inclement weather. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules. We reviewed saving the most current NC Child Care rules to your computer so you can easily access the rules in the future. You are aware of how to access the NC Child Care rules from DCDEE’s website. COMPLIANC HISTORY: Prior to today's visit, this facility maintained an 18-month compliance score of 96%. The violations documented today may affect the facility’s compliance history score. CONSULTATION/TECHNICAL ASSISTANCE EMERGENCY MEDICAL CARE PLAN: When asked, the facility’s Emergency Medical Care Plan has not been reviewed with staff since 2024. Staff have changed since the last Plan was completed; therefore, revisions were required and completed during the visit today. You reviewed the Plan with all staff during the visit, and they signed a document reflecting the Plan was reviewed today. A violation was documented today since the Plan was not reviewed annually with staff but was also verified as corrected during the visit. We discussed that in the future, to update/revise the facility’s Emergency Medical Care Plan when applicable based on staff and their roles at the facility in the event of an emergency. EMERGENCY PREPAREDNESSS PLAN: When asked, the facility’s EPR plan has not been reviewed with staff for 2025. The facility’s EPR plan was revised to make staffing changes, etc. but the revised plan was not reviewed with the staff. You also shared the revisions with the staff during the visit today and they signed a document reflecting the review of the Plan. This was documented as a violation today but was also verified as corrected during the visit. In the future, when revisions/changes to the EPR plan are made or if not, ensure the plan is reviewed with all staff annually and documentation of the review is on file. CHILDREN AND STAFF RECORDS: -Ten (10) percent of children’s records and ten (10) percent of existing staff and all new staff hired since last year’s annual compliance visit will be monitored next week when I return to conclude today’s visit. -Staff records were not monitored during today’s visit due to change of administration and not fully completed staff and training worksheets. We reviewed how to complete the staff and training worksheet using previous worksheets. We discussed emailing the completed worksheets to me prior to next week’s visit. You will use last year’s worksheets to make changes and add new staff to the worksheets. CRIMINAL BACKGROUND APPLICATIONS: A new feature in the ABCMA portal now links criminal background applications to facilities. The training via Moodle reviewing the feature has been completed. A roster for this facility has been created in ABCMS. We tried to access the roster during today’s visit but had difficulty logging into the ABCMS portal. We discussed contacting the Help Desk for further assistance. We referenced a job aid today regarding how to create and maintain a facility roster in ABCMS. You printed the job aid for reference during the visit. Remember to use the facility’s business NCID and password to access the facility CBC roster, but anyone trying to access their account must log in using their individual NCID and password. Information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five (5) business days. Please note, the hard copy of the Change of Information form is no longer needed or accepted. Should you need assistance please contact the Criminal Background Check Unit at 1-800-859-0829 and someone will assist you. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids status for lead in water is “analysis complete”. The last lead water test was analyzed May 22, 2025. Lead water testing is required every three (3) years. The status for lead based paint and asbestos is “Survey Review by RTI”. This information was verified in the clean water for US kids facility data. We discussed checking the Clean Water for Us Kids website regarding any further information for both lead based paint and asbestos. PROVIDER DOCUMENTS/FORMS: Continue using the most current forms from the Provider Documents and Forms tab from DCDEE’s website. We spent some time today discussing various checklists such as the Program Records Checklist, Staff Records Checklist, and the Children’s Records Checklist. You began serving as this facility’s administrator a few months ago and are continuing to become familiar with applicable NC Child Care rules pertaining to program records. You served in a different role at this facility prior to being hired as the facility’s administrator. I am available anytime to provide any needed technical assistance regarding NC Child Care rules. We discussed the staff and training worksheet and how to enter information on the worksheet and save the document to your computer. You developed an “Annual Compliance 2026” folder during the visit today and saved last year’s staff and training worksheets in the folder so you can go back and revise the worksheets for 2026. Work on revising/updating the worksheets prior to my return visit next Thursday. We also referenced how to find the application for a Rated License and all required forms are hyperlinked related to QRIS. We will further review QRIS information next week. You stated the facility will probably be assessed using Pathway Two. S. Howard, administrator, assisted with the visit. This center currently operates with a five-star license, earning seven points in the education component, six points in the standard component program and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. This program continues to meet enhanced ratio and enhanced space requirements. The last annual compliance visit was completed on March 5, 2024. The last sanitation inspection was completed on December 30, 2024, earning a “Superior” classification. The last fire inspection was completed on October 4, 2024, and the center was approved for daytime care only. Thirty-nine (39) children ranging from ages zero (0) to five (5) years old were observed in care today. Children were engaged in morning routine/activities, free-choice play, eating breakfast/lunch, completing hand washing and toileting routines, and interacting with caregivers. Breakfast consisted of egg omelets, peaches, strawberries, and milk. Lunch consisted of Chicken with rice, mixed vegetables, mixed fruit and milk. The following violations were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A rug, water hose and tee pee stick was laying on the ground posing a tripping hazard. SAFE INDOOR/OUTDOOR ENVIRONMENT: We discussed the potential hazards on the playground used by children. You submitted a work order to have maintenance complete the tasks to remove potential hazards identified. According to you, the first playground is only used by infants in the mat area. I explained the playground must always be in compliance if it is being utilized by children. Maintaining a safe environment for children ensures they can explore freely play without concerns. Staff can ensure the environment used by children remains safe by conducting daily checks. Also be reminded, plastic bags should be stored in spaces inaccessible to children under the age of three. The caregiver removed the plastic bags from the cabinet and shelf and discarded them so they could be inaccessible to children. TRANSPORTATION REQUIREMENTS: Children transported by the center must have emergency information on file that includes photographs. Two children did not have photographs with their emergency information. Photographs should be retrieved and attached to the emergency information. You can retrieve photographs by requesting from parents or taking the pictures at the center. I informed you the tires were deflated on bus #18 used to transport children. During the visit, you contacted maintenance to request air be added to the tires. I recommend you and staff check your passwords to ensure they meet the new requirements. If needed the password should be changed to align with the new password requirements. S. Howard, Administrator assisted with the visit. Thirty-two (32) children were observed in care today ranging from zero to five years old. Children were engaged morning activities, free choice play with developmentally age-appropriate materials, interacting with the caregivers, eating breakfast, and completing toileting, diapering and hand washing routines. Breakfast consisted of French toast sticks with syrup, mandarin oranges, and milk/water. Currently this program operates with a five-star license. The last sanitation inspection was completed on June 28, 2024, earning a superior classification. The last fire inspection was completed on October 30, 2023, and the center was approved for daytime care. The last annual compliance visit was completed on March 5, 2024. The following violations were documented today. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. STAFF/CHILD INTERACTIONS: During my observation, staff were observed interacting with children about activities they were engaged in while playing. Staff engaged children in conversations about stories read, encouraged them to share materials, play with each other and talked about food served for breakfast. Children were reminded and encouraged to use soap and count when washing hands. One diaper changing was observed and steps were followed according to the diapering chart. Children talked and played with their peers in a positive manner. The caregiver held one inf ant while bottle feeding and talked to the others engaged in floor play. Breakfast consisted of French toast sticks with syrup, mandarin oranges, and milk/water. Currently this program operates with a five-star license. The last sanitation inspection was completed on June 28, 2024, earning a superior classification. The last fire inspection was completed on October 30, 2023, and the center was approved for daytime care. The last annual compliance visit was completed on March 5, 2024. PROVIDER ACCESS to the BACKGROUND CHECK MANAGEMENT SYSTEM DCDEE has launched the Provider Portal for ABCMS effective February 1, 2024. This access will allow providers to: •See applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster •See the real-time background check status of staff members •Run a printable report of the staff roster to assist with compliance visits To gain access to the Provider Portal complete the ABCMS Child Care Provider Portal Training on DCDEE's Moodle. The training consists of watching a short video then taking a test about the video. A certificate will be issued upon successful completion of the test. Email the certificate along with your full name, position and Facility ID to DCDEE_ABCMS_Provider@dhhs.nc.gov to get access to the portal. We will grant your access within 2 business days. Please do not use this email account to ask background check questions. If you have general questions, please continue to use DHHS.CBC.Unit@dhhs.nc.gov. EQUIVALENCY EXAM OFFLINE for MAINTENANCE: The NC Early Childhood Equivalency exam located in Moodle will be unavailable until further notice for updates. In the meantime, earn the Early Childhood Education Certificate by enrolling in EDU 119 at a local community college. Better yet, take advantage of DCDEE’s offer of a No-Cost Professional Development Teacher Membership and earn your CDA! The NC Early Childhood Equivalency exam located in Moodle will be unavailable until further notice for updates. In the meantime, earn the Early Childhood Education Certificate by enrolling in EDU 119 at a local community college.
