B. Batts, Early Childhood Coordinator, arrived shortly after the visit began and assisted. Currently, this center operates with a Five-Star license issued on December 2, 2021, earning seven (7) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on January 7, 2025. The last sanitation inspection was completed December 3, 2025 earning a Superior rating. The last fire inspection was completed April 3, 2025 and the facility was approved for care on first shift only. Children were observed participating in free choice activity center play, and engaging in teacher directed art making reindeer from paper plates. Observed interactions were positive and nurturing in nature. Both classrooms have NC Pre-K children enrolled. Therefore, the NC Pre-K program requirements were monitored today and found to be in compliance. However, transportation is not provided. You confirmed that no children receive medication while in care. FACILITY PROFILE This facility is owned by Wilson County Public Schools. I reviewed contact information for the owner and the facility with you, and you verified the current information listed with DCDEE remains correct. The following violations of child care requirements were documented. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The posted activity plan in space #211 and #212 were not current. GS 110-91(12); .0508(a) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by December 24, 2025. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how the violations were corrected. Please be aware any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. COMPLIANCE HISTORY Prior to today’s visit your compliance history was 99% over an eighteen (18) month period. CHILDREN RECORDS I reviewed ten (10) percent of children files, and no violations were observed. STAFF RECORDS We scheduled a visit for January 5, 2026 to complete the annual compliance visit by reviewing staff records. The staff and training worksheet was not completed; however, a violation was not warranted at this time. DAILY SCHEDULE & ACTIVITY PLANS While monitoring spaces #211 and #212 I observed the posted lesson plans were dated for the first week of December and the last week of November. Space #211 does have a copy of an emergency activity plan and this posted during the visit. Space #212 used the activities from last week as they were out several days last week. Ensure that staff have access to all current activity plans and are able to post them on Fridays or Mondays for the current week prior to children arriving. Activity plans must be posted for teachers, substitutes, parents and other visitors to reverence when in the classrooms. TECHNICAL ASSISTANCE & REMINDERS: ABCMS PROVIDER PORTAL TRAINING Upon checking the ABCMS website there is a roster available for your program. This confirms that you have completed the required ABCMS provider portal training. This roster must be maintained so be sure to add and remove caregivers as applicable. QRIS RATED LICENSE TRANSITION You attended the QRIS: Pathway to the Stars meeting that was conducted by Child Care Consultants in Wilson County in the month of September. We will complete the QRIS TA review for the program during the January 5th staff records review. The Clean Water for Carolina Kids program expanded to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. After reviewing the website at I observed that as of April 1, 2025 no lead paint or asbestos hazards were identified. The programs lead water testing was completed in 2024. Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules. The best way to do so is to periodically review the child care laws and rules. The current rule book dated July 1, 2025, is available at the DCDEE website https://ncchildcare.ncdhhs.gov/. There are also modules reviewing the new Child Care Rules available on Moodle, which can be found on the Division website. B. Batts, Education Coordinator and C. Savage, Lead teacher assisted with the visit. This center currently operates with a five-star license, earning seven points in staff education, seven points in program standards and one quality point for offering a staff benefits package and infrastructure of parent involvement. The center’s last annual compliance visit was completed on January 26, 2024. The sanitation inspection was completed on November 21, 2024, earning a “Superior” classification. The fire inspection was completed on September 11, 2024, and the center was approved for daytime care only. The center continues to meet enhanced space and reduced ratios. Twenty-four (24) children were observed in care today. Children were engaged in morning activities, free choice play in various activity areas, completing toileting and hand washing routines, eating breakfast, participating in a science activity making snow, and interacting with teachers. Breakfast consisted of pancake/sausage on a stick, cranberries and milk. The center’s compliance history score was reviewed with the operator. The program’s compliance history was 95% as of January 7, 2025. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. The health assessment dated October 4, 2024 was on file for a child enrolled August 26, 2024. GS110-91(1) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. A health action plan for a child diagnosed with asthma was not on file and available for review. .0801(b) The violations documented must be corrected immediately. Lesson plans for November 2024 through January 2025 were reviewed today. Children were engaged in activities documented on the lesson plan reviewed. Staff observed working in the classrooms were the same listed in the NC Pre-K plan. One substitute was observed assisting in classroom 212. Staff interacted with children in a positive and nurturing manner. You reported the program continues to use the Creative Curriculum and Teaching Strategies Gold. You stated the program continues to u se the Foundations for Early Learning. Developmental screenings using the Dial 4 were completed and on file in children records reviewed today. The program’s monitoring tool was not available for review. Vision, hearing and dental screenings were on children files in file reviewed. HEALTH ACTION PLAN: A health action plan was not on file for one child identified with a chronic illness. You stated medication and information was recently provided for the child; however, the health action plan was not included. I printed you a copy of the health action plan to give the parent for completion. You stated the document would be completed during departure and placed in the child’s file. I recommended reviewing application upon receipt to identify if it a chronic illness was listed and request the proper documentation. Once receive the health action plan should be attached to the child’s application. HEALTH ASSESSMENTS: The health assessment for one child was dated after the thirty-day timeframe. You stated the child was not allowed to return to school until the health assessment was submitted. I explained although the health assessment was received and on file a violation was warranted because it was completed after the thirty-day requirements. It was suggested working with parents at the start of enrollment to ensure the health assessment is received within the recommended timeframe. At the end of today’s visit, the visit summary report was completed, reviewed and left with you. Children were engaged in morning activities, free choice play with developmentally age-appropriate materials, interacting with the teachers, eating breakfast, and completing toileting, diapering and hand washing routines. Breakfast consisted of grits, toast, bananas, and strawberry milk. Currently this program operates with a Five-Star License earning seven points in education, seven points in program standards and one quality point for offering a staff benefits package and infrastructure of parent involvement. Your program continues to meet enhanced space and reduced staff/child ratio. The last sanitation inspection was completed on November 9, 2023, earning a superior classification. I recommended hanging the bag on a higher hook that is five feet above the floor or placing it on one of the higher shelves in the classroom. NUTRITION REQUIREMENTS: Children were served strawberry milk with breakfast. The teacher stated flavored milk has been served twice to children this year; however, it is when there is no unflavored milk available. I informed the staff programs shall serve only unflavored skim or low fat milk for children 24 months through five years. The nutritionist serving the food stated she would notify the cafeteria. LEAD WATER/PAINT AND ASBESTOS: Ensure you review the information provided concerning Lead in Water/Paint and Asbestos. Menus for the month of January 2024 were posted and reviewed. SAFE ENVIRONMENT We discussed covering the cracks on the two black barriers enclosing the surfacing around the play structure until they can be replaced. It was suggested checking the black barriers closely when completing the outdoor play inspection to ensure they remain in good repair. Be reminded you are responsible for always maintaining a safe environment for children in care. HEARING SCREENING: The health assessment for one child enrolled did not include a hearing screening. You stated the teacher has spoken with the parents about attempting to get the screening completed. We discussed obtaining updated documentation verifying the screening was completed. A copy of the documentation should be placed in the staff file. It was suggested having staff check children’s health assessment when received for completion of required screening. If the screening has not been completed, staff should notify the appropriate individual to ensure the necessary steps are followed to obtain one. COMING SOON! PROVIDER ACCESS to the BACKGROUND CHECK MANAGEMENT SYSTEM: Beginning February 1, child care administrators can access the Automated Background Check Management System (ABCMS) This access will allow providers to: •See the real-time background check status of staff members. •Run a printable report of the staff roster to assist with compliance visits. •See new background check applicants and add them to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. We will include additional details and links in the next issue of the newsletter. CHILD CARE HEALTH CONSULTANTS ARE HERE TO HELP! CCHCs are trained to focus on health and safety in the child care environment. This includes illness and infectious disease. As part of their role, CCHCs use the North Carolina Health and Safety Assessment and Encounter Tool (NC HSAET) to guide them while completing classroom assessments. The assessment results can be used to help child care programs plan and take steps to reduce the spread of illnesses. The CCHC can use the NC HSAET to observe: •Diapering •Handwashing •Toileting •Sanitation practices •Exclusion You can your local health department to find out what child care health consultant is assigned to your program.
