    Name of Operation: Little Hands of Loving Care Facility ID: 98000242 Consultant: RAMIE DOLL Operation Type: Center Case Number: Visit Date: 6/23/2026 Number Present: 29 Completed Date: 6/23/2026 Age: From 0 To 10 Total Minutes: 340 Time In: 10:30 AM Time Out: 04:10 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to a third and final temporary time period visit. This program currently operates with a Temporary License issued February 2, 2026. The temporary license is valid through August 2, 2026. This facility is owned and operated by K & J Collectives, Inc. According to the NC Secretary of State, this corporation is current and active. Both administrators, K.Farmer, and J.Farmer, were present today. I met with both of you today to further discuss the Rated License for this facility and other applicable NC Child Care requirements. A total of twenty-nine children, ranging in ages from infancy to ten (10) years of age were present. Three separate groups were being cared for in the main building, building 1 and one (1) group of children was being cared for in building 2. The children in various groups were observed interacting with assigned staff members, completing bathroom and handwashing routines, playing outside, and resting on their assigned covered mats. The school aged children were present today since they are out of school for the summer. All required posted items were available for review and were current. Continue conducting monthly fire drills and outdoor playground inspections at different times each month. When asked, you have not completed the staff and training worksheet, but you worked on the worksheet during the visit today. I showed you how to access the most current staff and training worksheets from DCDEE’s website. You worked on completing the worksheet for my review today. Staff records were monitored today. The staff that are working at this facility worked and were employed with the facility that operated at this location prior to you opening this facility. The following violations were documented today: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A medical exam was not on file for two (2) staff members whose hire date was 2/2/26. Medical exams were completed for three (3) staff members after their hire date of 2/2/26. A medical exam was completed August 28, 2017 for a staff member who was hired 2/2/26. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. TB skin tests for three (3) staff members were read negative more than 12 months before their hire date of 2/2/26. The read negative dates were: 3/25/24, 1/6/17, and 4/3/24. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Documentation was not on file verifying orientation training was not completed for six (6) staff who were all hired 2/2/26. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First Aid training expired April 20, 2026 for two (2) staff members. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. CPR training expired April 20, 2026 for two (2) staff members. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) The above violations must be corrected immediately. Email or mail me verification of correction of the violations within the next two (2) weeks, on or before July 7, 2026. Email me as soon as possible if an extension is needed to verify correction of any of the violations. As of today, this facility has earned a compliance history score of 95%. The violations documented today may affect this facility’s current compliance history score. We reviewed that all facilities must maintain at least a 75% compliance history score over an eighteen-month time frame to be in good standing with DCDEE. If a facility’s compliance history falls below 75% over an eighteen-month time frame, some type of administrative action may be issued. Technical Assistance/Consultation/Reminders Specific to Today’s Visit: STAFF RECORDS: This facility began operating February 2, 2026. Prior to this facility operating, another facility was operating at this location and the same staff that worked for the previous facility began working for you as of February 2, 2026. New information such as TB skin test or screening forms, medical exams, and orientation was needed prior to their new hire date of February 2, 2026. You stated you have completed some orientation training with staff but did not complete the information of the training completed on the orientation form. We discussed having a few staff meetings to review the topics/rules/etc. for orientation. Complete the orientation forms for all applicable staff and keep the information on file. Two (2) staff will need to obtain a new medical exam. Three (3) staff need to obtain either a TB screening form or proof of a negative TB skin test. RECOGNIZING AND RESPONDING TO SUSPICIONS OF CHILD MALTREATMENT: One (1) staff member needs to complete the Recognizing and Responding to Suspicions of Child Maltreatment training via the Prevent Abuse Child Abuse NC website: www.preventchildabusenc.org. CPR AND FIRST AID TRAINING: CPR and First Aid training for two (2) staff expired April 20, 2026. When asked, neither staff member renewed CPR and First Aid training. These staff must register and complete CPR and First Aid training as soon as possible, on or before July 3, 2026. Ensure the agency where they completes the training is approved by DCDEE. If you have questions about the approved agencies, please contact me. STAFF TRAINING: We discussed when staff need to complete their annual on-going training hours. Their hours are based on when the facility was issued the temporary license, which was February. Training hours for staff should be completed in January of each year. When staff complete training, ensure they provide you with a certificate of completion so you can keep the certificate on file. Ensure staff complete the on-going training log and attach their certificates to the log for verification. CURRICULUM: During the last temporary time period visit, you stated you were using Funshine Express. You were using the curriculum on a trial basis to get your staff used to using something different; however, you have decided to choose another curriculum. You stated you may choose to use Learn Everyday, an approved curriculum. It is developed via Kaplan. Learn Everyday has a curriculum for Infants, Toddlers, Twos, and Preschoolers. If you choose to purchase this curriculum, please let me know. We discussed completing training related to the curriculums as well and maintaining documentation of the training on file for review. We discussed using Experience Early Learning Authentic Assessment as an approved formative assessment. I will need to know what curriculum and formative assessment you choose in the next couple of weeks and see verification of the training you completed for the curriculum. RATED LICENSE ASSESSMENT: We discussed and referenced the various Pathways related to the QRIS, but specifically Pathway 2. We discussed formative assessments and curriculums today. We discussed continuous quality improvement plans, family and community engagement options, and other information related to QRIS for Pathway 2. I showed you how to reference specific forms pertaining to Pathway 2 from DCDEE’s website during the last temporary time period visit. You plan to transition from a temporary license to Pathway 2. You signed the Application for the Rated License today and the Star Level Assessment for Child Care Centers/Centers Located in a Residence. We did not fully complete the forms today as you both are still working on the CQI plans and other documents and information pertaining to Pathway 2. I will be back in contact with you regarding completing/finalizing the rated license for this facility. We have to also complete the education information. We discussed finalizing and submitting all required information to transition this program to a Rated License from a temporary license on or before July 20, 2026. Education Standards: I will be reviewing staff education via the WORKS portal. We reviewed any staff that are not listed nor are evaluated via WORKS to create a WORKS account and apply for the appropriate position before submitting an official transcript so that the Workforce Education Unit is aware of what needs to be evaluated for a person. We reviewed how to access DCEEE’s WORKS logon link today via DCDEE’s website. I will be reviewing staff education and will be in contact with you. To continue being eligible to receive subsidy via DSS, a facility must earn at least a Three-star license.
