J. Sabella, facility administrator, was present and assisted with the visit. The visit summary was reviewed with Ms. Sabella today. This center currently operates with a Notice of Compliance issued June 14, 2019. The last annual compliance visit was completed on March 12, 2025. The facility’s last sanitation inspection was completed on December 30, 2025, earning a “Superior” classification and no demerits were issued. The last fire inspection was completed November 18, 2025, and the facility was approved for day and nighttime care only. Seventy-seven (77) children ranging from ages birth (0) to five (5) years old were observed in care today. Children were engaged in morning routine/activities, coloring, being fed, and interacting with assigned staff members. The following violations were documented today: Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. One (1) bottle of Children's Motrin, one (1) bottle of Mylicon Gas Drops, and one (1) bottle of Infant Tylenol were stored in an unlocked cabinet under the sink in Room D105. One (1) bottle of Infant Tylenol and one (1) bottle of Children's Acetaminophen were stored in an unlocked cabinet over the sink in Room D109. 15A NCAC 18A .2820(d) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff member who was hired on 10/21/25 had a negative TB skin test completed on 6/7/21. .0701(a) The violations documented must be corrected immediately. Prior to making any changes in the ownership status contact me to discuss the change of ownership procedures. Failure to comply could impact the program license status. SAFE INDOOR/OUTDOOR ENVIRONMENT: We discussed maintaining any items with the label “keep out of reach of children” stored at least five (5) feet above the finished floor and inaccessible to children. I added items with “keep out of reach of children” and additional warning labels must be kept in locked areas. Acceptable types of locks are a combination, electronic, or magnetic device, and lock and key. These unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. We also reviewed staff personal belongings, including their purses, and bags should be kept in spaces inaccessible to children. This includes any type of over-the-counter medication and prescription medications. Over-the-counter medications were not stored in locked storage today in two (2) rooms. You removed the medications and brought them to your office until they are returned to the parents/guardians. You stated you plan on updating your medication policy to include no longer bringing over-the-counter medications to the facility. We referenced Section .0803 in the NC Child Care rules today pertaining to medication. STAFF RECORDS: Ten percent of existing staff records and all new staff records were monitored today. We reviewed that if a person is hired and their proof of a negative TB skin test or TB screening form was completed more than 12 months before their hire day, a new TB skin test or screening form are required prior to hire. One staff member was hired October 21, 2025, and the TB skin test was read negative on June 7, 2021. CHILDREN’S RECORDS: Ten percent of children’s records were monitored today, (nine) based on current enrollment. The records were well organized and all applicable information was on file for review. OUTDOOR TIME: We discussed ensuring all groups of children have an opportunity to play outdoors every day, weather permitting. Continue using the weather watch chart as a guide to determine when it is safe for children to go outdoors. This is applicable to the infants and toddlers as well as preschool aged children. This facility has two (2) outdoor play areas for the children to use. RECORD RETENTION: We discussed the NC Child Care rule in section .2318(8) regarding record retention. The information pertaining to how long to keep records on file is now listed in a chart form in the NC Child Care rules. CRIBS: We discussed cribs being placed at least 18 inches apart or separated by a solid barrier when children are napping. The cribs were correctly spaced today. CRIMINAL BACKGROUND CHECK REQUIREMENTS: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. You completed the ABCMS training and have developed a roster for the facility. Continue to update the roster in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. It was suggested having staff conduct safety checks of their classroom to look for any items that may be potentially hazardous to children and store them appropriately or remove them. Maintaining a safe environment for children creates opportunities for them to explore their spaces freely. HAND WASHING: Washing hands as required help cut down on the spread of germs and ensure a healthy and safe environment is available for children and staff. Be reminded that children and staff should wash their hands upon arrival as they enter the classroom. As soon as they enter the classroom. CRIBS: Although children were not observed in the cribs, we talked about the way they were arranged in classrooms D105 and D107. The cribs were placed next to each other and not 18 inches apart to separate the children. One caregiver stated the mobile children push the cribs out of place when up and moving around; therefore, the have placed them so they cant be moved. I reminded you cribs must be placed 18 inches apart when children are napping. You and staff stated the cribs are moved when children are placed down for naps. You added you are considering making some changes in the classroom with infants. MODELING APPROPRIATE EATING BEHAVIORS: Staff shall role model appropriate eating behaviors by consuming only food or beverages that meet the nutritional requirements specified according to the USDA guidelines in the presence of children in care. One caregiver had an unopened Mountain Dew soda on the table while working with children during my walk-through in classroom D106. The same caregiver had the soda when the class was transitioning outside; however, a small amount of the soda was gone from the bottle. As I observed the classroom from the window during outside time the soda bottle half empty was sitting on the table accessible to children. You shared staff typically have food delivered to the center and eat while in the classrooms. You added you were not aware of this child care rule. J. Sabella, the administrator, assisted with the visit. This center currently operates with a Notice of Compliance issued June 14, 2019. The last annual compliance visit was completed on March 20, 2024. The last sanitation inspection was completed on December 5, 2024, earning a “Superior” classification. The last fire inspection was completed November 2024, and the center was approved for day and nighttime care only. Eighty-two (82) children ranging from ages zero (0) to five (5) years old were observed in care today. Children were engaged in morning routine/activities, free-choice play, eating breakfast and lunch, completing hand washing and toileting routines, and interacting with caregivers. Breakfast consisted of biscuits with gravy, bananas and milk. Lunch consisted of ravioli, mixed fruits, green beans and milk. The following violations were documented. Violation Number Comment Rule 609 Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. One employee did not wash hands upon entering the classroom and interacting with children in Space D104. 15A NCAC 18A .2803(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. An employee’s purse was on a counter less than 5 ft. from the finished floor in Space D104. Two small bottles of hand sanitizer labeled “keep out of reach of children was on a counter less than 5ft. from the finished floor in Space D108. Swifter cleaning solution labeled “keep out of reach of children with additional warning labels was in an unlocked drawer in the bathroom used by children in Space D106. .2820(b) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags were observed in an unlocked drawer in the bathroom in Space D106 and in an unlocked cabinet in the bathroom in Space D109. .0604(q) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The center’s EPR plan was not reviewed annually as required. The last dated review documented was December 2023. .0607(e) The violations documented must be corrected immediately. The program’s compliance history was 82% as of January 16, 2025. CORPORATION STATUS: Peace Free Will Baptist Church Inc. is listed as the current owner of this center. According to information verified on the North Carolina Secretary of State’s website, Peace Free Will Baptist Church Inc. remains current and active. Prior to making any changes in the ownership status contact me to discuss the change of ownership procedures. Failure to comply could impact the program license status. SAFE INDOOR/OUTDOOR ENVIRONMENT: We talked about the potentially hazardous items observed throughout the facility in each classroom. You stated you have spoken with staff about some of the items such as locking their purses and storage of the cleaning solutions. I explained any items with the label “keep out of reach of children only can be stored 5ft. above the finished floor and inaccessible to children. I added items with “keep out of reach of children” and additional warning labels must be kept in locked areas. Locked areas shall include those that are unlocked with a combination, electronic, or magnetic device, key, or equivalent locking device. These unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. Please reference child care rule 10A NCAC 09 .1719 (7) for the full rule reference regarding approved locking devices. I also explained staff personal belongings including their purses and bags should be kept in spaces inaccessible to children. PLASTIC BAGS: Plastic garbage and grocery bags were observed in classrooms where care is provided to children under age three. According to the child care rule, plastic bags, toys, and toy parts small enough to be swallowed shall not be accessible to children under three years of age. We discussed ensuring plastic bags are always stored out of reach of children. It was suggested having staff conduct safety checks of their classroom to look for any items that may be potentially hazardous to children and store them appropriately or remove them. Maintaining a safe environment for children creates opportunities for them to explore their spaces freely. HAND WASHING: Washing hands as required help cut down on the spread of germs and ensure a healthy and safe environment is available for children and staff. Be reminded that children and staff should wash their hands upon arrival as they enter the classroom. As soon as they enter the classroom. CRIBS: Although children were not observed in the cribs, we talked about the way they were arranged in classrooms D105 and D107. The cribs were placed next to each other and not 18 inches apart to separate the children. One caregiver stated the mobile children push the cribs out of place when up and moving around; therefore, the have placed them so they cant be moved. I reminded you cribs must be placed 18 inches apart when children are napping. You and staff stated the cribs are moved when children are placed down for naps. You added you are considering making some changes in the classroom with infants. MODELING APPROPRIATE EATING BEHAVIORS: Staff shall role model appropriate eating behaviors by consuming only food or beverages that meet the nutritional requirements specified according to the USDA guidelines in the presence of children in care. One caregiver had an unopened Mountain Dew soda on the table while working with children during my walk-through in classroom D106. The same caregiver had the soda when the class was transitioning outside; however, a small amount of the soda was gone from the bottle. As I observed the classroom from the window during outside time the soda bottle half empty was sitting on the table accessible to children. You shared staff typically have food delivered to the center and eat while in the classrooms. You added you were not aware of this child care rule. I reviewed child care rule 10A NCAC 09 .0901 (j) with you during the visit. I provided the rule reference for you to access later. I also shared this rule applies during nap time as well when children are still awake. CRIMINAL BACKGROUND CHECK REQUIREMENTS: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Upon arrival, C. Bradshaw, Assistant Director assisted with the visit. You arrived after the visit started and assisted. Seventy-four (74) children were observed in care today ranging from zero to four years old. Children were engaged in free choice play with developmentally age-appropriate materials, outdoor play, interacting with the caregivers, eating breakfast, and completing toileting, diapering and hand washing routines. Breakfast consisted of sausage/pancakes on a stick with syrup, mixed fruits, and milk/water. Currently this program operates with a Notice of Compliance. The last sanitation inspection was completed on June 21, 2024, earning a superior classification. The last fire inspection was completed on November 29, 2023, and the center was approved for daytime care. The last annual compliance visit was completed on March 20, 2024. The following violations were documented today. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. STAFF/CHILD INTERACTIONS: Upon arrival, preschool children were observed playing with various developmentally age-appropriate materials. I observed staff talking to children as they completed the diaper changing and toileting routines. Children were assisted with cleaning after finishing breakfast. Younger toddlers were placed in bye bye buggies after finishing breakfast to head outside. Staff engaged children in singing songs while they waited for others to finish hand washing. Older children were allowed to get books and sit in the carpet while waiting for their peers to finish breakfast and complete toileting routines. Children were cared for in a safe and nurturing environment. MEDICATION REQUIREMENTS: We discussed ensuring accurate medication requirements are met. I reminded you all prescription medications except for rescue medicines should be stored locked spaces. I further shared authorization forms should be completed with all required information such as valid start and end dates. For medications that have expired, they should be returned to the parents. It was recommended notifying parents prior to the expiration dates to ensure the medication or over the counter creams can be replaced as needed. One EpiPen onsite was not in the original container with the pharmacy label. The label was inside the bag with the EpiPen; however, there was no container. I recommended asking parent to provide the containers for the medicine. If interested staff could complete the Administering Medication training in Moodle of the Division's website as a refresher. HAND WASHING: A three-month-old infant hands were not washed after diaper changing. The caregiver stated she was not aware infants hand needed to be washed after diaper changing if a wipe was used. I explained with the guidance of the caregiver, infants with neck control should be taken to the sink and have their hand washed with soap and water. This facility is owned by Peace Free Will Baptist Church Inc. The corporation is listed as current/active in the North Carolina Secretary of State corporation’s database. The corporation that owns this facility is required to always maintain a current/active status. Failure to do so could result in an Administrative Action. I reviewed contact information for the owner and the facility with you and you verified the current information listed with DCDEE remains correct. The following violations of child care requirements were documented. Documentation of review of the program’s Shaken Baby Syndrome and Abusive Head Trauma policy for a child enrolled 5/13/19 was completed 6/12/19. .0608(b) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by April 3, 2024. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter must detail how the violation was corrected. The two (2) week timeframe is not intended to be used as a guide for correcting violations as they should be corrected immediately. Please be aware any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all included information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. COMPLIANCE HISTORY Prior to today’s visit your compliance history was 90% over an eighteen (18) month period. STAFF RECORDS I monitored all new and ten (10) % or more of staff records and they were found to be in compliance with Child Care Rules. CHILDREN RECORDS I monitored ten (10) % of children’s records and two (2) violations were documented. While a child enrolled 5/13/19 had documentation that parents reviewed the center’s Shaken Baby Syndrome and Abusive Head Trauma (SBS/AHT) policy it was not completed until 6/12/19. It is required that all parents of enrolled children review and document receipt of the program’s SBS/AHT policy prior to attendance. Two (2) children enrolled 5/13/19 and 7/26/19 did not have their immunization records on file within the required thirty (30) day timeframe. Children are required to have immunization records on file within the first thirty (30) days of attendance to ensure they have received the appropriate shots for their age and are healthy enough for licensed child care. The children were indicated on the child records form left with the provider. MEDICATION While monitoring medications in each classroom I noted that medication authorization forms for chronic illness and over the counter (otc) topical medication were missing information such as the list of symptoms that required the medication be administered, where and when to apply topical medication, or documenting “as prescribed” for amount of otc topical medication. While verbal instructions may have been given, this information must be written so staff providing care to that group of children know when, where and how much of a medication to administer when required. You and/or appropriate staff were notified of the forms that were missing this information in spaces #104, 105, 107, 108, and 111. TECHNICAL ASSISTANCE & REMINDERS The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs must complete the application summary for their program regarding lead paint and asbestos by May 1, 2024. The application is on the following website: https://www.cleanwaterforuskids.org/en/carolina/. The rule requires all licensed child care centers to test all drinking water faucets and food preparation sinks for lead contamination once every three years. The following technical assistance was discussed: All appropriate forms must include a child’s enrollment date in addition to the parent’s signature and date of signature. Applications should be reviewed before filing to ensure that parents have included not on the name of their hospital preference but the phone number as well. This saves time during emergency where the information may be needed. Continue to ensure that staff place all purses or personal bags in a cabinet that has a 2-step lock, such as your magnetic locks. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all applicable laws and rules. The best way for you to ensure you are meeting all requirements is to periodically review the child care laws and rules.
