C. Radford, lead teacher, was present and assisted with the visit. B. Batts, Early Childhood Coordinator, arrived shortly after the visit began and assisted. Currently, this center operates with a Five-Star License issued October 25, 2018, earning seven (7) points in the Education Component, six (6) points in the Program Component and one (1) Quality point. The last Annual Compliance visit at this center was completed March 26, 2026. The posted sanitation inspection was completed February 17, 2026 earning a “Superior” rating. The last fire inspection was completed August 25, 2025 and the facility was approved for daytime care only. Children were observed engaging in teacher directed art (making tire tracks with black paint and toy trucks), outdoor play, free choice center play, handwashing and lunch routines. Observed interactions between the teachers, interns and the children were positive and nurturing in nature. FACILITY PROFILE This facility is owned by Wilson County Schools. I reviewed contact information for the owner and the facility with you and you verified the current information listed with DCDEE remains correct. A violation of child care requirements was documented. Violation Number Comment Rule 1053 On-going training received was not within the 9 topic areas listed in GS 110-91(11) and/or the health and safety training topic areas in rule .1102. The staff member hired has not completed the “Administration of Medication” or the “Emergency Preparedness and Response Planning” trainings which are part of the health and safety training. GS 110-91(11); 10A NCAC 09.1102(b)(1-11) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by March 25, 2026. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how the violation was corrected. Please be aware that any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. COMPLIANCE HISTORY Prior to today’s visit your compliance history was 99% over an eighteen (18) month period. CHILDREN’S RECORDS During the visit 10%, or more, of children’s records were monitored and found to be in compliance with Child Care Rules. STAFF RECORDS I monitored 10% of staff records and a violation was documented. The staff member hired 10/16/24 completed the health and safety trainings except for the “Administration of Medication” and “Emergency Preparedness and Response” training. Both are offered for free through Moodle. I showed you where to find the medication training on Moodle as it is a separate link from the other health and safety trainings. TECHNICAL ASSISTANCE & REMINDERS Continue to monitor the large playground equipment for peeling paint as it appears it may be starting to crack and eventually chip and rust. All equipment and furniture are required to always be in good repair. Technical assistance discussed regarding keeping a copy of the “Requirements for Preschool Children on Public School Buses” forms that include pages with bus driver, administrator and parent responsibilities that each must sign and date on site. That is the only way it can be verified as being completed. The Clean Water for Carolina Kids program has expanded to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. The application is on the following website: https://www.cleanwaterforuskids.org/en/carolina/. I reviewed the website and observed that this program was exempt form the lead paint testing, and the asbestos testing is listed as “waiting for on-site visit results.” The lead water portion is noted as “mitigation pending.” I recommend ensuring that the program’s RTI account is updated with the current mitigation which you stated was no longer using the identified water fountain. Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules. The best way to do so is to periodically review the child care laws and rules. The current rule book dated July 1, 2025, is available at the DCDEE website https://ncchildcare.ncdhhs.gov/. There are also modules reviewing the new Child Care Rules available on Moodle, which can be found on the Division website. Upon arrival M. Galvan, teacher, assisted with the visit. You arrived shortly after and assisted. This center currently operates with a five-star license, earning seven points in education standards, six points in program standards and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. The last annual compliance visit was completed on April 11, 2024. The last sanitation inspection was completed on March 6, 2025, earning a “Superior” classification. The last fire inspection was completed on July 22, 2024, and the center was approved for daytime care only. Ten (10) children ranging from ages three (3) to four (4) years old were observed in care today. Children were engaged in morning routine/activities, free-choice play, outdoor play, eating breakfast, completing hand washing, and toileting routines, and interacting with caregivers. Breakfast consisted of Cherry strudels, mixed berry fruit cups, and milk. Interns were observed conducting circle time, preparing and serving breakfast and interacting with children. The following violation were documented. Violation Number Comment Rule 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. Cherry strudels served to children today were not recorded on the menu as substitution for eggs. 10A NCAC 09 .0901(b) The violation documented was corrected during the visit; therefore, a compliance letter is not required. The teacher added the food substitute to the menu when notified of the requirement. Staff stated information is provided during daily interaction with parents. I reminded staff the substitutions must also be recorded on the menu. The center’s compliance history score was reviewed with the operator. The program’s compliance history was 99% as of March 25, 2025. OWNERSHIP STATUS: Wilson County Public School is listed as the current owner of this center. Prior to making any changes in the ownership status contact me to discuss the change of ownership procedures. Failure to comply could impact the program license status. QRIS PUBLIC COMMENT PERIOD: On March 26 at 1:00 p.m., the N.C. Child Care Commission will hold a special meeting including a Public Hearing at 1:15 p.m. to hear from the public about its proposed rules to modernize the state Quality Rating and Improvement System (QRIS). Members of the public wishing to comment on any of the proposed rules may sign up to speak upon arrival. Instructions for webinar attendees to sign up to speak will be shared once the webinar begins. On March 27 at 10 a.m., the Commission will meet to continue considering public comments.
