Upon arrival J. Smith Administrative assistant assisted with the visit. You and J. Gray, Education Coordinator arrived shortly after and assisted. This center currently operates with a five-star license, earning seven points in the education component, six points in the program standard component and 1 quality point for 75% of teachers have an AAS or higher in ECE/CD. This program continues to meet enhanced ratios. The last annual compliance visit was completed on January 10, 2024. The last sanitation inspection was completed on June 18, 2024, earning a “Superior” classification. The last fire inspection was completed on March 20, 2024, and the center was approved for daytime care only. One hundred and fifty (150) children ranging from ages zero to five years old were observed in care today. Children were engaged in morning routine/activities, free-choice play, eating breakfast, completing hand washing, diapering and toileting routines, and interacting with caregivers. Breakfast consisted of toast, bananas, turkey sausage, and milk. The following violations were documented. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. W. Mitchell, Administrator assisted with the visit. One hundred thirty-one (131) children were observed in care today ranging from zero to twelve years old. Children were observed napping, interacting with the caregivers, eating lunch, completing toileting, and hand washing routines. Lunch consisted of BBQ sandwiches with slaw on bun, peas, boiled potatoes, and milk. Currently this program operates with a Five-Star License earning six points in education, seven points in program standards and one quality point for 75% of teachers have an AAS or higher in ECE/CD. The last sanitation inspection was completed on December 21, 2023, earning a superior classification. The last fire inspection was completed on March 20, 2024, and the center was approved for daytime care. The last annual compliance visit was completed on January 10, 2024. Children washed hands and completed toileting routines. Staff talked to children about their meals during lunchtime and encouraged them to try the foods. Children were cared for in a positive ad nurturing environment. FIRE INSPECTION: I reminded you fire inspections must be completed on or before the date on the current inspection report. You stated there was a delay in getting the fire extinguishers serviced; therefore, the fire inspection could not be completed. We discussed scheduling for services and the inspection early to avoid going past the due date. We talked about designating an individual to be responsible for tracking the dates of annual inspections and scheduling. ARRIVAL/DEPARTURE: Be reminded arrival and departure times must be documented for all children each day. It was recommended you or staff check the arrival and departure logs periodically throughout the day to ensure the arrival and departure times have been documented. This will ensure accurate times recorded and this requirement is met. I explained this violation was also documented during the last visit conducted on January 10, 2024. Children were engaged in free choice play with developmentally age-appropriate materials, outdoor play, completing toileting, diapering and hand washing routines, interacting with caregivers, and breakfast and lunch. Breakfast consisted of Cereal, applesauce, and milk. Lunch consisted of spaghetti, toss salad, apricots, garlic bread and milk. The center’s compliance history score was reviewed with the operator. The program’s compliance history was 83% as of January 9, 2023. The following violations were documented today. Departure times were not documented for six children between January5, 2024 and January 9, 2024. 10A NCAC 09 .0302(d)(4) 807 A safe indoor and outdoor environment was not provided for the children. Cracked containers exposing sharp edges in spaces accessible to children were observed in classrooms #6, 8, and 14. Five bolts were protruding from the black barrier enclosing the surfacing on the school-age playground. NC Pre-K children arrived for school at 10:aam until 2:30pm. A different set of staff provided care for each group of children. You stated the routine was off today due to school delays. SECOND VISIT: Another visit will be conducted to monitor children's records and to finalize the annual compliance visit and NC Pre-K monitoring. SAFE ENVIRONMENT We discussed the hazards observed during the walk-through. You removed and replaced the cracked containers during the visit. You stated maintenance would be notified to reinsert the bolts in the black border, and the was screw was removed from the cabinet. You stated staff typically inform you when hazards are identified; however, it was overlooked. It was suggested you conduct periodic monitoring and remind staff to report hazards when they are identified. Be reminded you are responsible for always maintaining a safe environment for children in care. ARRIVAL/DEPARTURE: Be reminded arrival and departure times must be documented for all children each day. It was recommended you or staff check the arrival and departure logs periodically throughout the day to ensure the arrival and departure times have been documented. This will ensure accurate times recorded and this requirement is met. When children are transferred to other classrooms, they should be added to the log for that space. Staff could also transfer the logs for their classroom when the transition children to ensure departure times are recorded as required. SAFE SLEEP PRACTICES: I informed you and staff; children should be placed down in their cribs for napping when they fall asleep. The time and position should be documented on the safe sleep check forms when children are napping. Children should also be visually checked every fifteen minutes while napping. It was suggested you review the center’s safe sleep policy with staff as a refresher. The caregiver stated she became nervous because I was in the classroom. I explained the routine should not change when visitors are observing unless instructed to do so. Maintaining consistency in routine keeps the children on track and avoids forgetting required tasks. STAFFING CHANGES: Three new staff members have been hired since the last visit. The staff files were reviewed, and the staff/training worksheet was completed. Based on my review of staff files, requirements were met. I encouraged you to have new staff create a DCDEE WORKS account and upload current education to be evaluated. This could be included in the on-boarding process to ensure its completed. UPDATED CHILD CARE RULEBOOK: I informed you of the updated child care rulebook dated January 1, 2024 is located on the Division of Child Development and Early Education website. W. Mitchell, Administrator assisted with the visit. This center currently operates with a five-star license, earning seven points in the education component, six points in the program standard component and 1 quality point for 75% of teachers having an AAS degree or higher in ECE/CD. Your program currently meets enhanced space and ratios. The last annual compliance visit was completed on February 2, 2023. The last sanitation inspection was completed on December 21, 2023, with a “Superior” classification. The last fire inspection was completed on March 13, 2023, and the center was approved for day and nighttime time care only. One hundred forty-six children were in care today. Children were engaged in free choice play with developmentally age-appropriate materials, outdoor play, completing toileting, diapering and hand washing routines, interacting with caregivers, and breakfast and lunch. Breakfast consisted of Cereal, applesauce, and milk. Lunch consisted of spaghetti, toss salad, apricots, garlic bread and milk. The center’s compliance history score was reviewed with the operator. The program’s compliance history was 83% as of January 9, 2023. The following violations were documented today. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Departure times were not documented for six children between January5, 2024 and January 9, 2024. 10A NCAC 09 .0302(d)(4) 807 A safe indoor and outdoor environment was not provided for the children. Cracked containers exposing sharp edges in spaces accessible to children were observed in classrooms #6, 8, and 14. Five bolts were protruding from the black barrier enclosing the surfacing on the school-age playground. A screw was protruding from a cabinet accessible to children in classroom #14. 10A NCAC 09 .0601(a) 871 Center staff did not comply with the safe sleep policy. The safe sleep policy was not followed when a infant was sleeping in a rocker from 9:33am-9:37am in classroom #13. 10A NCAC 09 .0606(a) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Sleep checks were not completed for one infant when placed down for nap and every fifteen minutes during the visit. .0606(g) The violations documented must be corrected immediately. NC Pre-K children arrived for school at 10:aam until 2:30pm. A different set of staff provided care for each group of children. You stated the routine was off today due to school delays. SECOND VISIT: Another visit will be conducted to monitor children's records and to finalize the annual compliance visit and NC Pre-K monitoring. SAFE ENVIRONMENT We discussed the hazards observed during the walk-through. You removed and replaced the cracked containers during the visit. You stated maintenance would be notified to reinsert the bolts in the black border, and the was screw was removed from the cabinet. You stated staff typically inform you when hazards are identified; however, it was overlooked. It was suggested you conduct periodic monitoring and remind staff to report hazards when they are identified. Be reminded you are responsible for always maintaining a safe environment for children in care. ARRIVAL/DEPARTURE: Be reminded arrival and departure times must be documented for all children each day. It was recommended you or staff check the arrival and departure logs periodically throughout the day to ensure the arrival and departure times have been documented. This will ensure accurate times recorded and this requirement is met. When children are transferred to other classrooms, they should be added to the log for that space. Staff could also transfer the logs for their classroom when the transition children to ensure departure times are recorded as required. SAFE SLEEP PRACTICES: I informed you and staff; children should be placed down in their cribs for napping when they fall asleep. The time and position should be documented on the safe sleep check forms when children are napping. Children should also be visually checked every fifteen minutes while napping. It was suggested you review the center’s safe sleep policy with staff as a refresher. The caregiver stated she became nervous because I was in the classroom. I explained the routine should not change when visitors are observing unless instructed to do so. Maintaining consistency in routine keeps the children on track and avoids forgetting required tasks. STAFFING CHANGES: Three new staff members have been hired since the last visit. The staff files were reviewed, and the staff/training worksheet was completed. Based on my review of staff files, requirements were met. I encouraged you to have new staff create a DCDEE WORKS account and upload current education to be evaluated. This could be included in the on-boarding process to ensure its completed. UPDATED CHILD CARE RULEBOOK: I informed you of the updated child care rulebook dated January 1, 2024 is located on the Division of Child Development and Early Education website.
