Your program currently operates with a Three-star licensed issued December 12, 2023, earning 4 points in the education component, 2 points in the program standards component and 1 quality point. The last annual compliance visit was completed on July 10, 2024. Prior to the visit the compliance history score was 97%. The NC Secretary of State website was reviewed today, and Young Memories Center LLC was listed as current- active. Natalie Pond, Child Care Consultant was present during today’s visit. Sanitation – 5/23/2025 – Superior Classification Fire – 6/25/2025 – Approved for daytime care K. Anderson – Young, Administrator, was present and available for consultation. Six indoor and two outdoor spaces are approved for use with children. Infants in space 3 received care based on individual need, including diapering, feeding and napping routines. Safe sleep checks and feeding schedules were monitored and found to be in compliance. Children in spaces 1 and 5 were seated down and coloring ditto sheets. Children in spaces 2 were enjoying music/movement activities. Spaces 4 and 6 are not currently in use. Breakfast consisted of pancakes, mandarin oranges, and milk. Lunch planned for the day consisted of hot dogs, buns, carrots, strawberries, and milk. The following violations were observed and must be corrected. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. In space 1, a diaper cream used for a child expired in April 2025. 10A NCAC 09 .0803(1)(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member, hired on March 5, 2025, did not complete the maltreatment within the 90 days as required. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 07, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The facility’s Temporary License was issued July 17, 2023, and is valid through January 17, 2023. Restrictions on the permit include a first and second shift capacity of 95 children ages 0 to 12 years old; meets enhanced ratios, and children under 2 and half years old in rooms with direct exits only. According to the North Carolina Secretary of State website, your corporation Young Memories Learning Center LLC, is current and active. Ms. Anderson was present and available for consultation today. Lynette Robbins, Licensing Supervisor, assisted with today’s visit. Sanitation – 07/24/2023 – Superior Fire - 06/07/2023 - Approved for daytime care only There are six approved indoor spaces and two approved outdoor playgrounds. Infants were observed seated in a circle while a staff member read a book. Following, infants were removed from the varying devices and allowed to move freely, accessing a variety of age-appropriate materials. Feeding schedules and safe sleep checks were monitored and found to be in compliance. Three-year-old children were seated at a child size table, engaged in morning group time. Following, table toys were provided for the group of children to explore. Two-year-old children were engaged in free play, accessing a variety of age-appropriate materials. Toddlers were observed during outdoor time. School-age children were not present due to the timing of the visit. Breakfast today consisted of blueberry muffin, mandarin oranges and milk. The center had the following items posted/located in a prominent place in the center for parents and available for me to review: the Temporary License, current menu, Safe Arrival and Departure Procedure, fire drill log, emergency phone numbers, and the Summary of the NC Child Care Law. A selection of new children’s files were reviewed previously and found to be in compliance. -Two new staff member were hired since the last visit. New staff records were monitored today and found to be in compliance. Program records, including fire drills and monthly outdoor inspections were completed and current. The following violation was observed and must be corrected immediately: Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. One staff member exited the classroom door leading to the outside to take out the trash, leaving her co-teacher out of ratio. There were ten children present. Technical Assistance Related to Violation Cited: Maintaining staff/child ratios and maximum group sizes is also essential to the health and safety of children. If children are combined with similar age groups, such as in the early morning or later evening hours, the staff/child ratio and maximum group size for the youngest child in the group must be maintained. Staff must be aware of children’s ages and how many children are in each group at all times. Today, one staff member exited the classroom door leading to the outside to take out the trash, leaving her co-teacher out of ratio. There were ten children present. The enhanced ratios for two-year-old children is 1 to 9. As discussed, please ensure you discuss the staff to child ratios with all staff members so they are aware and following the correct ratios at all times. Rated License: During the visit dated August 11, 2023, you stated you would like to request the Environmental Rating Scales (ERS) for your program. The application was emailed to you for completion. During a follow-up phone call, you stated you changed your mind and did not wish to have the ERS conducted at this time. You may choose to request an assessment at any time. Your 2-5 star rated license will be processed without the results of an ERS assessment. Reminders: As discussed today, young children should not be expected to sit down in large group gatherings for a long time. For most of their day, they should be allowed to move around the classroom freely, accessing a variety of materials while staff members interact with them in a positive manner. Today, eight infants were observed restrained in a variety of devices while staff members read a book to them. Although this was observed going on for less than a minute, it is developmentally inappropriate and unacceptable. As discussed at length, infants should not be placed in devices and restrained from being free to move around at any time during the day. I encourage you either provide training on developmentally appropriate activities or discuss this topic during your staff meeting. During the visit, I discussed this with the classroom teacher and informed her that this should not be reoccurring. T. Conyers, hired on September 11, 2023, is missing the following: A professional development plan within the first year of employment and annually thereafter. CPR/FA certification before or by December 11, 2023 to maintain compliance. Documentation of operational and personnel policy receipt. Remember to wash each child’s hands upon arrival. In the case the classroom is outdoors, I would encourage you to train all parents to wash their child’s hands upon entry to the classroom and prior to taking them outside. According to Center for Disease Control and Prevention, Clean hands save lives. Handwashing is like a "do-it-yourself" vaccine—it involves five simple and effective steps (Wet, Lather, Scrub, Rinse, Dry) you can take to reduce the spread of diarrheal and respiratory illness so you can stay healthy. Regular handwashing, particularly before and after certain activities, is one of the best ways to remove germs, avoid getting sick, and prevent the spread of germs to others. It's quick, it's simple, and it can keep us all from getting sick. Handwashing is a win for everyone, except the germs. The facility’s Temporary License was issued July 17, 2023, and is valid through January 17, 2023. Restrictions on the permit include a first and second shift capacity of 95 children ages 0 to 12 years old; meets enhanced ratios, and children under 2 and half years old in rooms with direct exits only. According to the North Carolina Secretary of State website, your corporation Young Memories Learning Center LLC, is current and active. Ms. Anderson was present and available for consultation today. Lynette Robbins, Licensing Supervisor, assisted with today’s visit. Sanitation – 07/24/2023 – Superior Fire - 06/07/2023 - Approved for daytime care only There are six approved indoor spaces and two approved outdoor playgrounds. Infants were observed seated in a circle while a staff member read a book. Following, infants were removed from the varying devices and allowed to move freely, accessing a variety of age-appropriate materials. Feeding schedules and safe sleep checks were monitored and found to be in compliance. Three-year-old children were seated at a child size table, engaged in morning group time. Following, table toys were provided for the group of children to explore. Two-year-old children were engaged in free play, accessing a variety of age-appropriate materials. Toddlers were observed during outdoor time. School-age children were not present due to the timing of the visit. Breakfast today consisted of blueberry muffin, mandarin oranges and milk. The center had the following items posted/located in a prominent place in the center for parents and available for me to review: the Temporary License, current menu, Safe Arrival and Departure Procedure, fire drill log, emergency phone numbers, and the Summary of the NC Child Care Law. A selection of new children’s files were reviewed previously and found to be in compliance. -Two new staff member were hired since the last visit. New staff records were monitored today and found to be in compliance. Program records, including fire drills and monthly outdoor inspections were completed and current. The following violation was observed and must be corrected immediately: Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. One staff member exited the classroom door leading to the outside to take out the trash, leaving her co-teacher out of ratio. There were ten children present. The enhanced ratios for two-year-old children is 1 to 9. 10A NCAC 09 .2818 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before October 20, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. Due to the severity of the violation cited, a follow-up visit will be conducted. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Technical Assistance Related to Violation Cited: Maintaining staff/child ratios and maximum group sizes is also essential to the health and safety of children. If children are combined with similar age groups, such as in the early morning or later evening hours, the staff/child ratio and maximum group size for the youngest child in the group must be maintained. Staff must be aware of children’s ages and how many children are in each group at all times. During a follow-up phone call, you stated you changed your mind and did not wish to have the ERS conducted at this time. You may choose to request an assessment at any time. Your 2-5 star rated license will be processed without the results of an ERS assessment. Reminders: As discussed today, young children should not be expected to sit down in large group gatherings for a long time. For most of their day, they should be allowed to move around the classroom freely, accessing a variety of materials while staff members interact with them in a positive manner. Today, eight infants were observed restrained in a variety of devices while staff members read a book to them. Although this was observed going on for less than a minute, it is developmentally inappropriate and unacceptable. As discussed at length, infants should not be placed in devices and restrained from being free to move around at any time during the day. I encourage you either provide training on developmentally appropriate activities or discuss this topic during your staff meeting. During the visit, I discussed this with the classroom teacher and informed her that this should not be reoccurring. T. Conyers, hired on September 11, 2023, is missing the following: A professional development plan within the first year of employment and annually thereafter. CPR/FA certification before or by December 11, 2023 to maintain compliance. Documentation of operational and personnel policy receipt. Remember to wash each child’s hands upon arrival. In the case the classroom is outdoors, I would encourage you to train all parents to wash their child’s hands upon entry to the classroom and prior to taking them outside. According to Center for Disease Control and Prevention, Clean hands save lives. Handwashing is like a "do-it-yourself" vaccine—it involves five simple and effective steps (Wet, Lather, Scrub, Rinse, Dry) you can take to reduce the spread of diarrheal and respiratory illness so you can stay healthy. Regular handwashing, particularly before and after certain activities, is one of the best ways to remove germs, avoid getting sick, and prevent the spread of germs to others. Ms. Anderson attested that all children’s files, staff files, and facility/program records are current and contain all the required information. The indoor and outdoor learning environments were monitored for all pertinent safety and equipment related requirements. There are five approved indoor spaces and two approved outdoor playgrounds. There are thirty-seven children enrolled and twenty-nine children, ranging in age from infants to ten years old, were present today. Infants received care based on individual need. Mobile infants not involved in routine care were observed moving about the classroom, playing freely. Feeding schedules and safe sleep checks were monitored and found to be in compliance. Toddlers, preschool and school age children were observed completing breakfast and transitioning to hand washing/free play. A variety of age-appropriate materials were accessible. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. A variety of age-appropriate learning materials were observed in each classroom. Adequate supervision and positive interactions were observed throughout. Breakfast today consisted of cereal, pineapples and milk. The following violations were observed today: Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Nystatin diaper cream was observed in space 4 that was missing the original box with the written instructions from the prescribing physician. .0803(2)(a) 851 When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. Technical Assistance for violations cited: Today, a prescribed Nystatin diaper cream was observed being used with a child in Space 4 without proper permission form. The original box, containing the instructions provided by the child’s pediatrician was also missing. We discussed at length the importance of ensuring the correct permission form is completed and checked prior to accepting medications. I also reminded you that all prescription medications must have the original container/box and instructions. Moving forward, I encourage you to set a system where one staff member is responsible for receiving the medication from the parent and checking for proper documentation prior to accepting or using the medicine on the child. RATED LICENSE: Today you stated you would like to request the environment rating scales. I emailed you the application during my visit. Please submit your application to me no later than August 25, 2023. Failure to receive your application by August 25, 2023, may result in your application being processed without the results from your ERS assessments. Education Standards: WORKS is a self-service applicant portal that will be used by an individual applicant to register and maintain their workforce information to support education requirements. The WORKS portal is available online and requires that the user maintain an individual North Carolina Identity Management (NCID) username and password. Only official unopened transcripts will be submitted to the Workforce Education Unit, all other documents will be uploaded by the applicant. All staff need to ensure current education information has been submitted to WORKS and they have requested the appropriate evaluation for their position.
