Limited child care requirements pertinent to this visit type were monitored. Your program currently operates with a 4-star license issued December 7, 2022, earning 4 points in the education component, 5 points in the program standards component (meeting enhanced space) and 1 quality point for having a staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. As of 04/12/26, this facility has an 18-month compliance history of 94%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The NC Secretary of State website was reviewed today, and Young Memories Learning Center, LLC was listed as current-active. The last annual compliance visit was conducted on 10/28/25. Two new staff have been hired since the last monitoring visit and their files were monitored today. Sanitation Inspection – 02/25/26 – Superior Classification Fire Inspection – 04/01/26 – Approved for day time care only There are six (6) indoor spaces, and two (2) outdoor spaces approved for use. There are thirty-nine (39) children enrolled and twenty-one (21) children, ages 0-5, were present today. Accurate attendance records are kept and monitored today. Children 2-3 years of age participated in free play with age appropriate materials activities. Preschool children were observed transitioning to the school age classroom to use the bathroom. Infants were observed in free play on the floor with their teachers. Afternoon snack consisted of cheese cubes and juice. Each classroom had spaces arranged into activity areas that include art, blocks, science, library, dramatic play, manipulative, music and music. Age-appropriate learning materials were accessible and observed in each classroom. Activity plans and daily schedules were available for each group of children, observed posted in each classroom and aligned with NC Early Childhood Foundations. During today's visit child care requirements monitored for compliance included supervision, staff/child ratio, CPR and First Aid, Criminal Record Checks, storage of hazardous substances and medications, approved space, posting of the license, and other applicable requirements. Fire inspections must be obtained annually. Other Information: Criminal Background Check Information – the required ABCMS training has been completed and the program is currently using the ABCMS Provider Portal to update information at facilities by verifying the roster to ensure current staff are noted on the roster. Children two years of age in space 5 had group circle time where they sang songs. Toddlers in space 6 were observed in outdoor free play. The teacher pushed the children around the merry go round. The NC Secretary of State website was reviewed today, and Young Memories Learning Center LLC was listed as current- active. You visited each indoor and outdoor space with us. School aged children enrolled in space 1 were not present because they were in school. Infants enrolled in space 2 participated in free play with age-appropriate materials and activities. Infant feeding schedules were observed, and safe sleep charts were being maintained. Caregiver was on the floor with one infant as infant explored their environment, played with toys, and had tummy time. Children three, four, and five years of age in space 3 also engaged in free play; this was a combined classroom with children enrolled in space 4. Children two years of age in space 5 had group circle time where they sang songs. Toddlers in space 6 were observed in outdoor free play. The teacher pushed the children around the merry go round. Activity plans were observed posted in each classroom. A variety of age-appropriate learning materials were observed in each classroom. Routine toileting and handwashing procedures were monitored, and requirements were met. Staff/child ratios were in compliance in all classrooms. All children were adequately supervised. Caregivers and children interacted in a positive and appropriate manner. Lunch was observed and consisted of chicken hot dogs, whole grain buns, peas, bananas, and milk. Prior to the visit the compliance history score was 95%. The NC Secretary of State website was reviewed today and Young Memories Center LLC was listed as current- active. The license was posted, and the restrictions were in compliance. Six classrooms are operating with thirty-four children present. All children were appropriately supervised. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. During today’s visit, infants in Space 2 were observed transitioning to outdoor play time. Children 4-5 years of age in Space 3 participated in free play with age appropriate materials and activities. Children in space 4 and 5 participated in group circle time in their designated classrooms. Staff in all classrooms were observed interacting with children in a positive and nurturing manner. Lunch consisted of ground beef, noodles, baked beans, strawberries, whole grain rolls, and milk. Assistant Director, Kynesha Anderson, was present and available for consultation. The last sanitation inspection was completed August 22, 2024, and the facility receive 4 demerits and a superior rating. The last fire inspection was completed on February 28, 2024, and the facility was approved for daytime care only. Children were observed in spaces 2, 4, and 5. School aged children were at school. Toddlers enrolled in space 6 were combined with infants in space 2. Older preschoolers enrolled in space 3 were with younger preschoolers in space 4. Children observed participated in free play activities with age-appropriate materials both indoors and outdoors. Breakfast was being prepared that consisted of french toast sticks, pineapples, and milk. All activity plan aligned with NC Early Childhood Foundations. Staff and children’s interactions were positive. Lunch for today was mini corndogs, carrots, applesauce, and milk. Administration reported they are currently not operating second shift at this time. The following violations were observed today. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A staff member who was rehired on November 4, 2024 was working with an expired criminal record background check qualification letter. G.S. 110-90.2(b) & .2703(n)&(o) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Photographs were not included with all emergency medical care information in the vehicles used for transportation. 10A NCAC 09 .1003(d) 1757 A valid qualification letter was not on file and available to review at the facility. McIntyre has a qualification letter that expired on September 25, 2024, and information for requalification was not submitted prior to the expiration. This violation must be corrected within fifteen (15) days, by November 21, 2024, and a copy of the qualification letter must be sent with the required compliance letter. If the qualification letter is not on file within the 15 days, the employee cannot be at the facility any longer until they receive their qualification letter. When transporting children, there must be a picture of the child attached to the emergency information for that child. Today technical assistance was provided on how ensuring that each child that is transported has identifying information in the vehicle about each child being transported, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a)(3). OR .1003(d). Rated License: The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you will not be required to have a reassessment according to the timelines discussed previously; however, you may request one voluntarily. Today I encouraged you to review information about the new standards as it becomes available and to plan on training your staff on the new assessment tools if you are interested in Environment Rating Scales assessments. I will share more information as I am able. North Carolina Rated License Assessment Project (NCRLAP): Starting February 1, 2025, the new and improved Environment Rating Scales – Third Additions will be implemented. You, Keresa Anderson, Director, assisted with the visit. Your program currently operates with a 4 star licensed issued December 7, 2022, earning 4 points in the education component, 5 points in the program standards component (meeting enhanced space) and 1 quality point for having a staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. Child Care Consultant, April Bosse, assisted with the visit today. Prior to the visit the compliance history score was 97%. The NC Secretary of State website was reviewed today and Young Memories Center LLC was listed as current- active. The license was posted, and the restrictions were in compliance. Six classrooms are operating with thirty-four children present. All children were appropriately supervised. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. During today’s visit, infants in Space 2 were observed being held or in the bouncing chairs with a caregiver sitting next to them. One-year olds in Space 2 were sitting at the table eating lunch while a caregiver sat next to them and talked with them. Three-year-olds in Space 4 were combined with four-year-olds in Space 5. Children in Space 5 were gluing cut out bugs to poster board and engaged in activity areas with fake phones and magnet blocks. One year old children in Space 6 were sitting at tables as lunch was being served. Caregivers sat close by and talked with children. Two-year-old children in Space 7 were engaged in group time talking about bugs. Staff in all classrooms were observed interacting with children in a positive and nurturing manner. Lunch consisted of chicken nuggets, peas, oranges, and milk. Violations were not observed today. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space 5, leftover ointment was not returned to the parent or discarded after the course of the treatment. .0803(12) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance: Today there was one expired diaper cream form in Space 5. We discussed ensuring that caregivers are checking forms and creams to ensure they are not expired each month. The form today expired on May 5, 2024. The medication was discarded today, and this violation was corrected during the visit. Today there were allergy lists post in each room and in the kitchen. We discussed that if you have times when there are no allergies you can keep the lists in the same place just write that there are no allergies at that time on sheets. Keeping the allergy list visible will tell staff where they can find it and look for it in the event there is any allergy.
