Jessica Mullins, Pre K Coordinator, assisted with the visit today. The program currently operates with a five-star license, issued July 1, 2024, earning 7 points in the education component, 7 points in the program standards component meeting enhanced space and enhanced ratios per .2818(c) and 1 quality point for having a staff benefits package and an infrastructure of parent involvement. The last annual compliance visit was conducted on April 29, 2025. The last sanitation inspection was conducted on January 8, 2026, and the program received a supervisor classification. The last fire inspection was conducted on August 5, 2025, and the program was approved for day time care only. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-three (83) percent as of March 29, 2026. All licensed spaces were monitored today. Children in spaces 301, 305, and 302 were observed in free play either indoors or outdoors. All activity plans were current and aligned with NC Early Childhood Foundations. Staff had informal conversations with children and engaged in play as well. Staff were all engaged in play with children in all spaces. Lunch for today consisted of sweet potato fries, waffles, sausage links, pears, and milk. Staff and Training Worksheets- There were eight staff members employed at this facility. There was no new permanent staff; however, a substitute was present. The substitute file and a percentage of veteran staff’s files were reviewed. Health & Safety Training – All staff members who have been employed for at least one year, have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. New staff are in the process of completing these training courses. Health and safety trainings are required to be completed during the first year of employment and renewed every 5 years. Remember, the Recognizing and Responding to Suspicions of Child Maltreatment training must be taken at www.preventchildabusenc.org, within the first 90 days of employment. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. Ensure this information in completed, recorded on the required form, and on file for review. NC Pre K Monitoring: The NC Pre-Kindergarten Program Site Monitoring Tool was reviewed today. The tool has been completed by the contract administrator. The program follows the New Hanover County school calendar, the length of the day meets requirements, and all staff meet education requirements. The Creative Curriculum for Preschool, 6th Edition, has been implemented and the assessment tool, GOLD, Teaching Strategies are all in place. Three formal assessments with parent conferences are scheduled through the school year. Portfolios are maintained on Teacher Strategies, GOLD by taking pictures of the children’s work and uploading them to the program. The facility uses the developmental screening tool, Brigance Early Childhood Screen III (3-5 Years). NC Pre-K requirements were met today. The following violation was observed and corrected during the visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the breezeway to the licensed spaces there was a bottle of cleaner accessible to children and a bucket of mop water. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. Technical Assistance: Criminal Background Check Information – This is a requirement and if it is not completed, it will be documented as a violation. DCDEE launched the Provider Portal for ABCMS on February 1, 2024. This access will allow providers to see applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster, see the real-time background check status of staff/household members, and run a printable report of the staff roster to assist with compliance visits. The facility has completed the training and has entered staff. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Name of Operation: WRIGHTSBORO ELEMENTARY Facility ID: 65000914 Consultant: APRIL LESTER Operation Type: Center Case Number: Visit Date: 9/22/2025 Number Present: 48 Completed Date: 9/22/2025 Age: From 4 To 5 Total Minutes: 110 Time In: 08:40 AM Time Out: 10:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable childcare requirements for a routine unannounced visit and to follow-up on the approved space violation cited September 10, 2025. Administration was not available at the start of the visit. The program currently operates with a five-star license, issued July 1, 2024, earning 7 points in the education component, 7 points in the program standards component meeting enhanced space and enhanced ratios per .2818(c) and 1 quality point for having a staff benefits package and an infrastructure of parent involvement. Prior to the visit the compliance history score was 87%. There are fifty-four (54) children ages 4-5 years old enrolled and forty-eight (48) were present during the visit. The license was posted, and the restrictions were in compliance. A walk-through of the licensed spaces was completed, and all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children were participating in free play in the indoor and outdoor learning environments. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Fire and sanitation inspections remain current. Lunch for today was popcorn macaroni, fruit, steamed broccoli, and milk. All children were cared for in approved licensed spaces during the visit. The following violations were observed. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Hanging bulb lights in space 305 did not have safety plugs and an outlet in space 301 was not plugged. 10A NCAC 09 .0604(c) 1757 A valid qualification letter was not on file and available to review at the facility. A new staff member who is qualified to work in child care did not have a copy of her qualification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. A new hire did not have a signed statement of the prevention of shaken baby syndrome and abusive head trauma policy on file. .0608(d)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before October 6, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. I suggest you use this checklist to review the file for new staff member and substitute staff to ensure all requirements are met by these deadlines. Please also update your Staff and Training Worksheet based on revisions you make and ensure you keep it updated as needed. This document should serve as a tracking tool for required paperwork and deadlines. You may wish to delegate some recordkeeping responsibilities to another staff member who has good organizational skills and is able to assist. This will allow you to trust that accurate records are being kept while you focus on other aspects of managing your facility. Reminders: Furnishings must be kept in good repair. There are two couches in space 301 and a vinyl chair in space 302 that are starting to tear. These furnishings must be replaced or covered to avoid a future violation. Today we discussed ways to repair the furnishings to reduce cost of purchasing new. Child Care Rule 10A NCAC 09 .0901(j) GENERAL NUTRITION REQUIREMENTS states that staff shall role model appropriate eating behaviors by consuming only food or beverages that meet the nutritional requirements specified in this Rule in the presence of children in care. Fast Food or coffee drinks that staff might consume do not follow these requirements and should not be consumed in the classroom or in the presence of children. Prior to the visit the compliance history score was 87%. There are fifty-four (54) children ages 4-5 years old enrolled and forty-eight (48) were present during the visit. The license was posted, and the restrictions were in compliance. A walk-through of the licensed spaces was completed, and all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children were participating in free play in the indoor and outdoor learning environments. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Fire and sanitation inspections remain current. Lunch for today was popcorn macaroni, fruit, steamed broccoli, and milk. All children were cared for in approved licensed spaces during the visit. This facility currently operates with a Five Star license issued January 14, 2019, earning 7 points in Education, 7 points in Program, and 1 quality point for choosing a Programmatic Option. Prior to this visit, the facility’s eighteen-month compliance history was 92%. Restrictions on the permit include: daytime care only; meets enhanced space; and reduced ratios as outlined in 10A NCAC 09 .2818(c). The facility is operated by New Hanover County Public Schools. Child Care Consultant, Heather Elliott, was present and assisted with today’s visit. Jessica Mullins, Pre K Coordinator for New Hanover County Schools reached out to me over the summer requesting the need to approve room 302 and increase the capacity. However, in August she stated that they were not moving a forth classroom to this location, therefore, we could wait to approve the space at the next monitoring visit. On Monday, September 8, 2025, I spoke to Ms. Mullins and she stated that the program was under the impression that room 302 was approved for use and they placed children 4-5 years of age in this classroom. I informed Ms. Mullins that a violation would need to be cited for children utilizing a space that was not approved and we scheduled today's visit to approve the space. A superior sanitation Inspection was completed April 4, 2025 and in addition, a formal letter from Environmental Health regarding approval of space 302 is included. A Fire Inspection was completed August 5, 2025. The facility was approved for daytime care only. An approved building inspection was completed June 16, 2024. All inspections have now been received for the new space, room 302. Room 302 is in the same building are the currently licensed spaces. Room 302 has direct access to the outside via an operable window and direct exit via an adjacent alcove. A space calculation worksheet was completed today for the new space. Space 302 has a capacity of 26 children at 30 square feet per child. Please note, an increase in capacity did not occur as line 27 of the building inspection states that the maximum number of persons allowed by plumbing facilities is 60. The following violation was observed. Violation Number Comment Rule 209 Children used space that was not approved. Children were present and utilizing space 302 which had not yet been approved by our agency. All inspections were received and the space was approved during the visit. Michelle Faison, Director, was unable to assist me during this visit due to another meeting. The program currently operates with a five-star license, issued July 1, 2024, earning 7 points in the education component, 7 points in the program standards component meeting enhanced space and enhanced ratios per .2818(c) and 1 quality point for having a staff benefits package and an infrastructure of parent involvement. Prior to the visit the compliance history score was 100%. The license was posted, and the restrictions were in compliance. A walk-through of the licensed spaces was completed, and all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children were participating in free play in the outdoor learning environment or in large circle time following lunch routines. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Fire and sanitation inspections remain current. Lunch for today was peanut butter and jelly sandwiches, cucumbers with ranch dressing, string cheese, bananas, graham crackers, and milk. A staff member's qualification letter expired October 10, 2024 and the required forms to complete a criminal background check were not submitted prior to the expiration. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. A staff members qualification letter expired October 10, 2024. G.S. 110-90.2(b) & (d) & .2703(e) For your facility to remain in compliance with NC Child Care Requirements, the violations must be corrected immediately. A signed and dated letter of compliance must be received by November 27, 2024, describing how the violation was corrected. The time frame is established to allow you time for submitting your compliance letter. This timeframe is not intended to be used as a guide for correcting violations, as they should be corrected immediately. This violation must be corrected within fifteen (15) days, by November 28, 2024, and a copy of the qualification letter must be sent with the required compliance letter. If the qualification letter is not on file within the 15 days, the employee cannot be at the facility any longer until they receive their qualification letter. Please review child care rules regarding storage and accessibility of potentially hazardous items. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. Today I suggested reviewing these requirements with staff and checking behind them to ensure all hazardous items are stowed correctly. A monthly playground inspection shall be conducted by an individual trained in playground safety requirements. A trained administrator or staff person shall make a record of each inspection using a playground inspection checklist provided by the Division. The checklist shall be signed by the person who conducts the inspection and shall be maintained for 12 months in the center's files for review by a representative of the Division. The playground inspection checklist may be found online at https://ncchildcare.ncdhhs.gov/providers/credent.asp. The playground inspection includes a checklist of items related to safety, surfacing, and equipment quality. These inspections should be stowed in the black box in the office as soon as it is completed. Fire drills shall be practiced monthly, and records shall be maintained as required. Records of monthly fire drills documenting the date and time of each drill, the length of time taken to evacuate the building, and the signature of the person who conducted the drill shall be available for review. Today I suggested marking a date on your calendar so that you can ensure that a fire drill is performed monthly. It may also help to plan these drills for different times of the day so children and staff can be prepared to evacuate no matter activity or routine is taking place. Reminders: Lockdown or shelter-in-place drills should be practiced at least every three months as part of your facility's EPR plan. These drills should be documented on the Emergency Drill Record. Frequent practice will allow children and staff members to be familiar with the steps to take to keep everyone safe in an emergency. Today I suggested you plan the dates and times of your drills in advance and use calendar reminders to ensure they are completed at least every three months. You must schedule and obtain a fire inspection within 12 months of your facility’s previous fire inspection. I suggest you notify the local fire inspector when it is time for the center's annual fire inspection. The original approved annual fire inspection report should be submitted to me within one week of the inspection visit on the form provided by the Division. I was able to obtain the most current inspection report during my visit today.
