You, Crystal Bowles, Interim Director, assisted with the visit. Your program currently operates with a Notice of Compliance, issued November 28, 2023. The last annual compliance visit was conducted on August 5, 2025. The sanitation inspection was completed March 5, 2026, and received a “Superior” classification. The last fire inspection was conducted February 24, 2026, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two (92) percent as of July 6, 2026. The NC Secretary of State website was reviewed today, and Winter Park Baptist, was listed as current- active. The program is currently operating their summer camp program. There are only three (3) spaces being used. Summer Camp operates from July 6-August 14, 2026. The typical school year will start September 8, 2026. You visited licensed spaces with me. Preschool aged children in spaces 119, 122, and 127 participated in free play indoors. Age-appropriate materials and activities were observed for all age groups. Routine toileting and handwashing procedures were monitored, and requirements were met. Staff/child ratios were in compliance in all classrooms. All children were adequately supervised. Caregivers interacted with children. All meals are brought from home. Staff and Training Worksheets- There were eight (8) staff members employed at this facility for summer day camp . There was one (1) new staff; new staff and a percentage of veteran staff’s files were reviewed. The following violations were observed. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member hired in February 2026 has not completed first aid. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Accurate attendance records are kept and monitored today. Children were observed arriving to the program, washing their hands, then engaging in free play with age appropriate materials such as fine motor, puzzles, blocks, and dramatic play. All meals are brought from home daily. During today's visit child care requirements monitored for compliance included supervision, staff/child ratio, CPR and First Aid, Criminal Record Checks, storage of hazardous substances and medications, approved space, posting of the license, and other applicable requirements. The following violations were observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was from January 2025. Today I suggested using the form from the DCDEE website to ensure all information needed is obtained and present on the permission form. Expired medications must be returned to the parents and new medication received must be accompanied by a new permission to administer form as this form will contain the medication expiration date. Designated emergency medications shall be stored out of reach of children at least five feet high but are not required to be in locked storage. For purposes of this Rule, designated emergency medications are those that are used or needed for the immediate recovery from a life-threatening event and include Glucagon, epinephrine auto-injector, diazepam rectal installation and albuterol. Technical assistance was also provided today regarding file organization. I suggested that you use the revised file checklists under “Provider Documents and Forms” on the DCDEE website to assist with organization and ensure all requirements are met. I also encouraged you to review the chart in Child Care Rule 10A NCAC 09 .2318 for more information about records retention. You, Robin Donahue, Director, assisted me with the visit. Your program currently operates with a Notice of Compliance. Sanitation and fire inspections remain current. Prior to the visit the compliance history score was 94%. The NC Secretary of State website was reviewed today, and Winter Park Baptist Church of Wilmington was listed as current- active. The permit was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in indoor learning environments and found supervision to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Meals are brought from home. The following violations were observed. An epi-pen room 127 was not in the original labeled container. .0803(2)(a) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. A staff member had a Chick-Fil-A cup on a shelf in classroom 122. .0901(i) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. A new staff member did not have proof of tuberculosis test or screening on file. .0701(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. The allegation is as follows: -There are concerns that a child wasn’t adequately supervised while outdoors on the playground. The program operates with a Notice of Compliance, issued October 23, 2023. Prior to the visit the compliance history score was 95%. Director, Robin Donahue, assisted me with the visit today. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. Files for new staff were reviewed and contained all required information. Additionally, I observed the indoor and outdoor spaces used by the children. Children were observed in spaces 2-5, and 7. Positive and appropriate staff/child interactions were observed as well as active supervision in all spaces. Staff/child ratios were as follows: Space 2 – two year olds– 2:10 Space 3 – four & five year olds – 2:13 Space 4 – three & four year olds – 2:10 Space 5 – three & four year olds – 2:9 Space 7 – two & three year olds – 2:10 Space 8 – one & two year olds – 2:8 Allegation: There are concerns that a child wasn’t adequately supervised while outdoors on the playground. **On Monday, February 19, 2024, Director, Robin Donahue telephoned DCDEE intake to self-report regarding a supervision violation. On Friday, February 16, 2024, at approximately 11:45 am a child 4 years of age was left unattended on the fenced in playground for approximately 2 minutes unsupervised. The child was reported to be okay; however, he was crying when he was found by one of the teachers responsible for him. During my investigation, Ms. Donahue provided me with written statements from both teachers detailing the incident. Per staff, around 11:30 am on Friday, February 16, 2024, two classrooms were lining up at the door to the building at the same time to return indoors and prepare for departure. Both teachers stated they counted the children multiple times and then headed indoors. However, there was no mention of face to name recognition. Once the group entered the building one teacher started diapering and toileting routines while the other teacher prepared the children to go home. After the teacher finished changing a child’s diaper she asked where the child was, and the other teacher stated she didn’t know. At that time the teacher ran to the playground door and found the child outside crying, not injured. She immediately consoled the child, reported the incident to the director, and told the parent at pick up moments later. Both teachers responsible for this child were immediately terminated from employment. I reviewed the programs Staff Policies and Procedures during the visit. Staff are required to position themselves throughout the playground to ensure safety and are required to do head counts throughout the day. The child is still enrolled in the program. Based on the observation, investigation, and staff report, the allegation for supervision was substantiated. Ms. Donahue stated that she has an all staff meeting today at 12:00 pm to discuss the incident and supervision requirements with staff. Ms Donahue has emailed a letter to all parents about the incident and ensured parents that the program takes safety of the children seriously. The following violation was observed. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On February 16, 2024, a child four years of age was left unsupervised on the playground for approximately 2 minutes. .1801(a)(1-5) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Teachers should position themselves so that they can observe all of the children: watching, counting, and listening at all times. Transitions are times when heightened observations need to take place as we have received many reports lately of children being left in a classroom or in the outdoor play space. Teachers should account for all children with name-to-face recognition by visually identifying each child. They should also use their knowledge of each child’s development and abilities to anticipate what they will do, then get involved and redirect them when necessary. This constant vigilance helps children learn safely. Good supervision practice and routines should be discussed during your Orientation process. Today we discussed assigning zones for the teachers while on the playground to ensure all children can be seen and are accounted for. We also discussed using tracking logs with teachers to ensure that face-to-name recognition is occurring when transitioning children to and from the playground. This may be something you choose to add to your policies and procedures. A violation was documented for failure to maintain compliance with supervision requirements. Violations of this nature directly impact the safety of the children while in your care. In addition, these types of violations have the greatest negative impact on your compliance history. During the visit, we discussed that repeated violations of this nature during consecutive visits may lead to an administrative action against your license. A follow-up visit will be conducted in the near future to monitor compliance with childcare requirements.
