Name of Operation: WILMINGTON CHRISTIAN ACADEMY PRESCHOOL Facility ID: 6559006 Consultant: SHEREEN PICKETT Operation Type: Center Case Number: Visit Date: 2/11/2026 Number Present: 232 Completed Date: 2/11/2026 Age: From 2 To 5 Total Minutes: 255 Time In: 09:00 AM Time Out: 01:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during an annual compliance visit. A checklist was used to complete this visit. This program currently operates with a Notice of Compliance issued September 27, 2017, with a capacity of 285 children ages 2-12 years old. Restrictions on the Notice include daytime care only and no cooking allowed in building. The last annual compliance visit was conducted by Child Care Consultant, Shereen Pickett on February 18, 2025. Prior to this visit, the facility’s eighteen-month compliance history was 95%. According to the North Carolina Secretary of State’s website, your corporation, Grace Baptist Church, Inc is current and active. Sanitation Inspection – 10/14/2025 - Superior classification Fire Inspection – 11/12/2025 - Facility was approved for daytime only A walkthrough of all indoor and outdoor spaces approved for use with children was conducted. Children in space 101 were engaged in morning group time. Children in spaces 109 and 106 were observed playing in the indoor jungle gym. Children in spaces 103, 105 and 110 were engaged in outdoor play. Children in spaces 104, 108, 113, 107 and 111 were seated down for morning snack, which consisted of oatmeal cookies, apple slices and water. Children in space 102 were engaged in free play. Lunch planned for today consisted of turkey slices, roll, baked beans, pineapples and milk. Staff/child ratios were in compliance. All children were adequately supervised. Staff in all classrooms were observed interacting with children in a positive and nurturing manner. Staff and Training Worksheets- There are fifty staff members employed at this facility. All new staff and a percentage of veteran staff’s files were reviewed. The following violations were observed and must be corrected immediately. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. In space 102, a diaper cream used on a child expired in December 2025. 10A NCAC 09 .0803(1)(d) 847 Parent's medication authorization did not include required information. In space 102, a diaper cream parent permission form did not include expiration date of cream and description of how to apply the cream. 10A NCAC 09 .0803(4)(6-9) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space 101, wipes stored in soft plastic labeled "keep out of reach of children" were observed stored on top of a shelf in the bathroom, less than five feet above ground and accessible to children. .0604(q) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. the last quarterly drill was practiced on September 4, 2025. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 20, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If you email the compliance letter, it must be sent from the email address registered with the DCDEE and the following information must be included: name, position, facility name, and facility ID number. Technical Assistance for Violations Cited: Medications should be checked consistently to ensure they do not expire while staff members are using them on children. Today, a diaper cream that expired in December 2025 was still in use. I encourage you to come up with a checks and balances system that ensures medications are checked often and either replaced or sent home prior to expiration. To ensure the health and safety of children in care, all questions on parent permission forms should be completed. Leaving lines blank or answered with “as needed” leaves the interpretation open to staff members as to how to use the medication with the children – which is unacceptable. Again, having a system to check permission forms prior to placing them in the classroom for use is necessary. Any products labeled with “keep out of reach of children” must be stored at least five feet above ground. Any products with more than one warning, including aerosol sprays should be locked at all times. Child care rules require that a shelter-in-place or lockdown drill is practiced with the staff and children every three months. This is to ensure that in case a true emergency occurs, everyone knows what precautionary steps to follow. According to documentation on file, the last drill was practiced on September 04, 2025. Moving forward, I would encourage that drills are schedules on the calendar as a reminder. Reminders: A fire drill is due for the month of February. S. Keener’s CPR/FA certification expires in August 2026. Ensure new certification is completed and card is on file prior to expiration of current one. Provider Portal for ABCMS (the criminal records check database): Ms. Liles stated she has completed the required training, and access has been given, and current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired within five business days. Should you need assistance please contact the Criminal Background Check Unit at (919) 814- 8401 and someone will assist you. 2025-2026. Respiratory Season: To help North Carolinians understand how to defend themselves against serious illness from the flu, COVID-19 and RSV, NCDHHS has released a bilingual Respiratory Illness Communications Toolkit for the 2025–2026 respiratory virus season. The toolkit offers ready-to-share fact sheets, social media content, videos, posters and other materials to help communities, health departments, schools and employers share respiratory illness information. The materials explain how these viruses spread, symptoms to watch for and the benefits of getting updated vaccines. They also provide information on where to get vaccinated. NCDHHS encourages North Carolinians to get vaccinated for the strongest protection and follow everyday steps like handwashing and staying home when sick. For questions or concerns, talk to a health care provider. Resources and vaccinefinder information are available at MySpot.nc.gov. According to the website, your water was last tested on January 19, 2024. Criminal Background Check Information – DCDEE launched the Provider Portal for ABCMS on February 1, 2024. This access will allow providers to see applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster, see the real-time background check status of staff/household members, and run a printable report of the staff roster to assist with compliance visits. I confirmed today that Ms. Horsley has completed the training and the roster for all current staff members.
