    Name of Operation: RISING STARS OF WILMINGTON Facility ID: 65000981 Consultant: ALLISON DAVIS Operation Type: Center Case Number: 0525-266A Visit Date: 5/23/2025 Number Present: 46 Completed Date: 5/23/2025 Age: From 0 To 4 Total Minutes: 140 Time In: 12:40 PM Time Out: 03:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of this unannounced visit was to investigate allegations of violations of child care requirements at this child care facility. LaShanna Harris, Executive Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Harris, Portia Jackson, Regional Director, and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were confirmed during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On May 20, 2025, a staff member failed to provide adequate supervision resulting in a one-year-old child choking on a piece of food and requiring medical attention. .1801(a)(1-5) 405 A child's hands were not washed after each diaper change. On May 20, 2025, a staff member failed to have multiple one-year-old children wash their hands after being diapered. 15A NCAC 18A .2803(c)(2) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. On May 20, 2025, a staff member failed to have one-year-old children wash their hands before eating. 15A NCAC 18A .2803(c) 609 Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. On May 20, 2025, a staff member failed to wash their hands after diaper changing and before handling food multiple times. 15A NCAC 18A .2803(a) 807 A safe indoor and outdoor environment was not provided for the children. On May 20, 2025, a staff member left two different children unattended on the changing table for approximately three minutes placing them at risk of falling and injury. This same staff member left plastic bags on a table accessible to one-year-old children which placed the children at risk of harm. On this same date, snacks were served to one-year-old children that were not age-appropriate in size. 10A NCAC 09 .0601(a) 1810 There was a substantiation of child maltreatment. Pursuant to its investigation, the Division has confirmed sufficient information to determine child maltreatment. Portia Jackson, Administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Jackson and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were confirmed during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On August 23, 2023, a one-year-old child was placed in an unsafe environment when the child accessed acorn seeds on the playground and consumed several seeds, thus placing the child at serious risk of harm and/or injury. As a result of the incident, the child received medical treatment. Staff members were unaware the acorn seeds were on the playground. 10A NCAC 09 .0601(a) Violations must be corrected immediately.
