    Name of Operation: REYNOLDS HOME DAY CARE Facility ID: 65000227 Consultant: APRIL LESTER Operation Type: Family CC Home Case Number: Visit Date: 11/12/2025 Number Present: 5 Completed Date: 11/12/2025 Age: From 0 To 4 Total Minutes: 75 Time In: 09:30 AM Time Out: 10:45 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance with applicable child care requirements. Limited child care requirements pertinent to this visit type were monitored. The program operates with a one (1) star license issued August 17, 2020. Restrictions on the permit include: daytime care only; maximum of 5 preschool children at any time; 12 hours of operation; max 5 preschool and 3 school age. As of November 11, 2025, this facility has an 18-month compliance history of 100%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The last annual compliance visit was completed June 3, 2025. There are five (5) children ages infant through 4 years of age enrolled and all five (5) were present today. A speech therapist was present and worked with a child in the kitchen area. The infant was put down for rest time in an adjacent room with the door open. Safe sleep checks were monitored and found in compliance. The other three (3) preschool children were observed in the playroom/living room participating in free play with age appropriate materials and activities. You had informal conversations with the children as they played. All children were cared for in a nurturing and caring manner. All children bring meals from home daily. During today's visit child care requirements monitored for compliance included supervision, staff/child ratio, CPR and First Aid, Criminal Record Checks, storage of hazardous substances and medications, approved space, posting of the license, and other applicable requirements. One (1) violation was observed and corrected during the visit. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Aerosol air refresher was stored on a high shelf rather than in locked storage in the bathroom. .1719 (a)(7) Continued compliance with all applicable child care requirements is always required. Technical Assistance: Today you stated you need to take some additional on-going training hours. Please refer to the last verification form for your required hours needed before you next annual visit. You may also visit the DCDEE website to access several free, online trainings through Moodle using your NCID. Other Information: Criminal Background Check Information – You have completed the required ABCMS training and is currently using the ABCMS Provider Portal to update information at facilities by verifying the roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. CLEAN CLASSROOMS FOR CAROLINA KIDS - Your facility’s status, according to the Clean Classrooms for Carolina Kids website, shows the following: • Water Testing-Completed on 08/06/24. Water testing must be completed every three (3) years. • Lead-Based Paint-No hazards identified • Asbestos-No hazards identified Ensure you keep all paperwork that you receive regarding this matter, and it may be requested to verify compliance. Wanda Reynolds, Owner/Operator, assisted me with the visit. The program currently operates with a 1-star license issued August 17, 2023. Restrictions on the permit include: daytime care only; maximum of 5 preschool children at any time; 12 hours of operation; max 5 preschool and 3 school age. The last annual compliance visit was conducted July 25, 2022. Prior to the visit the compliance history score was 100%. All licensed spaces were monitored today. Children are cared for in the main living room. Children use the bathroom at the end of the hallway. Children ages 0 to 2 were observed present today. During the visit children engaged in free play with age-appropriate materials and activities. An activity plans was observed and aligned with NC Early Childhood Foundations. Safe sleep checks were monitored and found in compliance. All children bring meals from home and nutrition opt out forms were in each child’s file. The following violation was observed today and corrected during the visit. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Hand sanitizer and disinfecting wipes were not stored a minimum of 5 feet off the floor or in locked storage. The operator properly stored these items during the visit. .1719 (a)(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Technical Assistance: Your CPR and First Aid certification will expire at the end of October 2023. CPR and First Aid training for child care must be taught in-person by an approved training organization, and only certain courses meet requirements.
