This facility was issued a One Star license on April 10, 2024. Prior to this visit, the facility’s eighteen-month compliance history was 93%. According to the Secretary of State’s website, the corporation’s status, Ace Learning Two, LLC, is current and active. The last annual compliance visit was conducted by Child Care Consultant Shereen Pickett on October 03, 2024. Sanitation Inspection – 07/01/2025 - Superior classification Fire Inspection – 09/02/2025 - Facility was approved for daytime care only M. Biddle, Administrator, was present and available for consultation. All indoor and outdoor spaces approved for use with children were visited today. Infants in spaces 1 and 2 received care based on individual needs, including diapering, feeding and napping routines. Infants not involved in routine care were observed being held by caregivers or moving about the room, playing freely. Infant feeding schedules and safe sleep checks were monitored and found to be in compliance. Children in spaces were observed 3, and 5-6 were engaged in outdoor gross motor play. Children in spaces 4, 9-11 were seated down for morning group time. Children in space 7 were changing clothes in preparation for water play on the splash pad, located on the premises. Children in space 8 were engaged in free play, accessing a variety of age-appropriate materials. Positive interactions were observed. Morning snack consisted of Belvita cookies, peaches, and water. Lunch planned for the day consisted of lasagna, green beans, blue/black berry mix, and milk. The following violations were observed and must be corrected immediately. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In space 3, a Nystatin diaper cream was not stored in the original container with written instructions. .0803(2)(a) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member, hired on September 2, 2025, did not have a medical report on file prior to employment. 10A NCAC 09 .0701(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. A staff member, hired as a cook on 9/25/2023, did not complete the criminal background check process. G.S. 110-90.2(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member, hired on 6/12/2024, did not complete FA trainiing within 90 days of employment. One staff member's FA certification expired in June 2024. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Ms. Sessions was present today and was asked to leave the premises and not return until the criminal background check process is completed through DCDEE and a valid qualification letter is on file. CPR/FA - For the health and safety of the children, all staff members are required to obtain CPR and First Aid certification and keep it current. Renewing this certification is crucial for staff to be refreshed in first aid and other lifesaving techniques as well as receive the most up to date information. I suggest you set a reminder on your calendar to keep track of expiration dates and/or seek out training from additional approved agencies when staff is not able to attend courses offered locally or at the facility. Maltreatment Training - Remember, the Recognizing and Responding to Suspicions of Child Maltreatment training must be taken at www.preventchildabusenc.org, within 90 days of employment. I encourage you to put a reminder on your calendar upon hiring a new employee to ensure this training is successfully completed within the required timeframe. REMINDERS: A monthly fire drill and playground inspection are due in October. As discussed today, the trained staff member should review the Emergency Preparedness and Response Plan annually, or when information in the plan changes, to ensure all information is current. Regarding the use of sound machines in infant classrooms: There can be concerns that staff members may not be able to hear and respond to sleeping infants and supervise appropriately. This facility was issued a One Star license issued on April 10, 2024. Prior to this visit, the facility’s eighteen-month compliance history was 94%. According to the Secretary of State’s website, the corporation’s status, Ace Learning Two, LLC, is current and active. This is the first annual compliance visit since the program became licensed. Sanitation Inspection – 6/11/2024 - Superior classification Fire Inspection – 10/2/2024 - Facility was approved for daytime care only K. Gragg, Administrator, was present and available for consultation. All indoor and outdoor spaces approved for use with children were visited today. Infants in spaces 1 and 2 received care based on individual needs, including diapering, feeding and napping routines. Infants not involved in routine care were observed being held by caregivers or moving about the room, playing freely. Children in spaces 3-4, 6, 7-8 were observed during outdoor play. Children in space 5 were engaged in free play, accessing a variety of age-appropriate materials. Children in spaces 10 and 11 were seated on the carpet and engaged in morning group time. Space 9 is not currently in use. Morning snack consisted of cheese toast, blackberries and water. The following violations were observed and mut be corrected immediately. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A staff member, hired as a cook on 9/25/2023, did not complete the criminal background check process. G.S. 110-90.2(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member, hired on 6/12/2024, did not complete FA trainiing within 90 days of employment. One staff member's FA certification expired in June 2024. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member, hired on 6/12/2024, did not complete CPR trainiing within 90 days of employment. A staff member, employed since 9/25/2023, did not have a valid criminal background quailification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A staff member, hired on 7/1/2024, did not complet the maltreatment training as required. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Infant feeding schedules and safe sleep checks were monitored and found to be in compliance. One year old children in Spaces 2 and 5 were engaged in activity areas with blocks, cars, and soft toys or engaged on the playground with swings, balls, and tunnels. Two year old children in Spaces 3 and 4 were outside in the play area climbing on the structure. The facility’s Temporary License was issued October 9, 2023 and is valid through April 9, 2023. Restrictions on the permit include a first shift capacity of 200 children ages 0 to 12 years old. According to the North Carolina Secretary of State website, your corporation Ace Learning Two, LLC, is current and active. Child Care Consultant April Bosse was present and assisted with today’s visit. Facility administrators Meg Biddle and Kayla Gragg were present today and accompanied Ms. Bosse during the facility walk-through. The center had the following items posted/located in a prominent place in the center for parents and available for me to review: the Temporary License, current menu, Safe Arrival and Departure Procedure, fire drill log, and the Summary of the NC Child Care Law. The indoor and outdoor learning environments were monitored for all pertinent safety and equipment related requirements. There are seventy-six children enrolled and fifty-six children ages 0-4 were present today. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for children. Activity plans were observed posted in the classroom. A variety of age-appropriate learning materials was observed in the classroom. Infants in Space 1 received care based on individual need, including diapering, napping, and feeding routines. Infants not engaged in routine care were observed playing on the carpeted area with soft toys, balls, and scarves while caregivers sat close by. Infant feeding schedules and safe sleep checks were monitored and found to be in compliance. One year old children in Spaces 2 and 5 were engaged in activity areas with blocks, cars, and soft toys or engaged on the playground with swings, balls, and tunnels. Two year old children in Spaces 3 and 4 were outside in the play area climbing on the structure. Preschool children in Spaces 6 and 7 were engaged in activity areas gluing feathers, lining up animals, and engaged in group time talking about what they did this past weekend. Lunch today consisted of turkey and cheese sandwiches, green beans, sliced apples, and milk. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Medication authorization did not include the required information for diaper cream in Space 4 and an Epi-Pen in Space 6. 10A NCAC 09 .0803(4)(6-9) The following violations were observed today. A compliance letter including detailed information about how all violations have been corrected must be received by November 20, 2023. Include any supporting documentation with your response (if applicable). Your letter will be used as verification that violations have been corrected and compliance is maintained; your written response is considered a legal document so must be accurate and truthful. If you email the compliance letter, it must be sent using the email address registered with DCDEE and include your facility name and ID number. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to maintain an eighteen-month compliance history score of at least seventy-five percent. Technical assistance regarding violations cited: All medications, topical and oral must have written permission to administer to a child. For medications such as epi-pens or Benadryl, medication administration forms will be valid for six months and then will require a new signature but the parents. Topical ointments such as diaper creams must have permission to administer forms completed. These forms for ointments are valid for at least one year if the parent so chooses. Designated emergency medications shall be stored out of reach of children at least five feet high but are not required to be in locked storage. For purposes of this Rule, designated emergency medications are those that are used or needed for the immediate recovery from a life-threatening event and include Glucagon, epinephrine auto-injector, diazepam rectal installation and albuterol. Today we discussed ensuring that these medications were brought to the playground and kept on the caregivers or stored five feet off the ground and out of reach of children. RATED LICENSE: At this time the facility is not planning to apply for a 2-5 star rated license. The facility does not currently participate in the North Carolina subsidy program. They understand they will not be eligible to participate in the subsidy program in the future if they do not have a minimum of a three star license. You may choose to apply at any time. Education Standards: WORKS is a self-service applicant portal that will be used by an individual applicant to register and maintain their workforce information to support education requirements. The WORKS portal is available online and requires that the user maintain an individual North Carolina Identity Management (NCID) username and password. Only official unopened transcripts will be submitted to the Workforce Education Unit, all other documents will be uploaded by the applicant. All staff need to ensure current education information has been submitted to WORKS and they have requested the appropriate evaluation for their position.
