The facility’s Four-Star License was issued September 25, 2024. Restrictions on the permit include a first shift capacity of 120 children ages 0 to 12 years old; meeting enhanced ratios. According to the North Carolina Secretary of State website, your corporation Palmetto Preschool & Learning Center, LLC is current and active. Prior to this visit, the program’s eighteen-month compliance history was 98%. The last annual compliance visit was conducted by Child Care Consultant Shereen Pickett on March 18, 2025. Sanitation – 4/9/2025 – Superior classification Fire – 8/29/2024 – Approved for daytime care only Rebecca Hinman, Administrator was present and available for consultation. Infants in space 1 received care based on individual needs, including napping, feeding and diapering routines. Infants not engaged in routine care were placed on the rug and allowed to play/move freely. Safe sleep checks and feeding schedules were monitored and found to be in compliance. Toddlers and preschool-aged children in spaces 3 – 5a were observed during outdoor play. Two-year-old children in space 2 were engaged in music/movement activities. Spaces 4 and 5b are not currently in use. Breakfast today consisted of English muffin, bananas, and milk. Lunch planned for the day included cheese pizza, salad, green beans and milk. Three staff members were hired since the last visit. All new staff members’ files were monitored and found to be in compliance. One violation was observed and must be corrected immediately. The facility’s Temporary License was issued March 25, 2024 and is valid through September 24, 2024. Restrictions on the permit include a first shift capacity of 120 children ages 0 to 12 years old; meeting enhanced ratios. Restrictions were in compliance today. Childcare Consultant April Bosse was present and assisted with today’s visit. According to the North Carolina Secretary of State website, your corporation Palmetto Preschool & Learning Center, LLC is current and active. Your last fire drill was completed April 5, 2024 and your last outdoor inspection was completed April 6, 2024. As this is a change of ownership, a new building, fire and inspections must be completed prior to September 24, 2024. You will request your building and sanitation inspection through the COAST portal. You will contact your local fire marshal to request your fire inspection. Failure to obtain the required approved inspections prior to September 24, 2024 may result in administrative action against your license. You must also complete the two-day pre-licensing workshop for centers prior to September 24, 2024. The center had the following items posted/located in a prominent place in the center for parents and available for me to review: the Temporary License, Safe Arrival and Departure Procedure, transportation, and emergency medical care plan. Five classrooms are operating with forty-five children enrolled and thirty-two children present. All children were appropriately supervised. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. During today’s visit, infants in Space 1 were outside in the outdoor area in bouncy chair and on a carpet exploring the area. Toddlers in space 3 were engaged in group time with the caregiver reading the book “Animal Babies” and making the animal sounds. Two-year-old children in Space 2 were outside playing on see-saws, kicking balls, and sliding on the slide. Preschool children in Space 4 were outside in the play area swinging on swings, throwing balls and picking flowers. Preschool children in Space 5a were in the block center building with large cardboard blocks and animal figures. Lunch served today consisted of chicken sandwiches, sweet potato tots, corn, and milk. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Authorization for one diaper cream in Space 1 expired on January 25, 2024 and the medication was discarded or returned to the parent within 72 hours. .0803(12) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. A medical action plan was not available for review for the Epi-Pen in Space 2. .0801(b) The above violations were observed today. Send your compliance letter to me by Friday, March 10, 2024. Your compliance letter should clearly state how the violations have been corrected as well as a plan to ensure this violation is not cited in the future. If you submit your compliance letter electronically, it will need to be submitted via the email address on file with our agency. Technical Assistance regarding violations cited: Emergency Medications: For any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be attached to the application. The medical action plan shall be completed by the child's parent or a health care professional and may include the following: (1) a list of the child's diagnosis or diagnoses including dietary, environmental, and activity considerations that are applicable; (2) contact information for the child's health care professional(s); (3) medications to be administered on a scheduled basis; and (4) medications to be administered on an emergency basis with symptoms, and instructions. This plan is only needed for those children with the previously listed conditions. Today we discussed what forms are needed for all emergency medications in the facility. We also discussed keeping those medications unlocked, and in the space the child is enrolled. You stated today that you will be obtaining bags for these emergency medications so that they can easily be brought with the child as they transition to other spaces or outside. Topical Ointments: Today there was one diaper cream form in Space 1 that was expired. Topical ointment forms are valid for one year if the parent prefers that. We discussed that the forms being available in the classroom might help the staff see when they expire. I suggested making a list that is easier to read instead of shifting through the forms. On the playground adjacent to space 2 and 3 garbage was observed on the playground. There were several used fork, food wrappers and paper trash scattered throughout the playground. It’s time to enroll your facility in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to have tap water tested for lead and to have inspections for lead paint and asbestos if applicable. Please see steps below: 1.
