Rachel Shaffer, Director, was present and available for consultation. Your program currently operates with a 5 Star License, issued March 7, 2022, earning 5 points in Education, 7 points in Program Standards and 1 Quality Point in the Programmatic Option. Restrictions on the permit include: a capacity of 100 during first shift with an age range of 0-12 years old, daytime care only, meets enhanced space, and meets enhanced ratios per .2818(c). The last annual compliance visit was completed October 15, 2025. The last sanitation inspection was completed November 10, 2025, with a “Superior” classification. The last fire inspection was conducted October 24, 2025, and your facility was approved for daytime care only. Prior to the visit the 18-month compliance history score was 93%. The NC Secretary of State website was reviewed today, and Masonboro Baptist Church, Inc. was listed as current- active. All approved indoor and outdoor spaces were monitored today. Daily schedules and activity plans were available for review and were being followed. A variety of age-appropriate learning materials were observed in all classrooms. Infants in Spaces 1 transitioned to outdoor play. Infant feeding schedules and safe sleep checks were monitored and found to be in compliance. Toddlers in space 2 participated in group story time. Two-year-olds in space 3 participated in group music activities. Three-year-old children in Space 4 were observed in toileting routines. While waiting, books were read to them. Preschool children in spaces 5, 6, and 7 all engaged in circle time. Activity areas consisted of language, manipulative toys, dramatic play, art, and blocks. Meals are brought from home daily. Staff and Training Worksheets- There were twenty-two (22) staff members employed at this facility. There was one new staff; all new staff and a percentage of veteran staff’s files were reviewed. Health & Safety Training – All staff members who have been employed for at least one year, have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. New staff are in the process of completing these training courses. Health and safety trainings are required to be completed during the first year of employment and renewed every 5 years. Remember, the Recognizing and Responding to Suspicions of Child Maltreatment training must be taken at www.preventchildabusenc.org, within the first 90 days of employment. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. Your program currently operates with a 5 Star License, issued March 7, 2022, earning 5 points in Education, 7 points in Program Standards and 1 Quality Point in the Programmatic Option. Restrictions on the permit include: a capacity of 100 during first shift with an age range of 0-12 years old, daytime care only, meets enhanced space, and meets enhanced ratios per .2818(c). The last annual compliance visit was conducted at this facility on October 28, 2024. As of October 14, 2025, your facility has an 18-month compliance history for the facility was 100%. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Fire Inspection – October 24, 2025 – Approved for day time care only - Satisfactory Sanitation – 04/29/2025 – 10 Demerits – Superior Director, Rachel Shaffer, was present and available for consultation. There are seven (7) approved indoor spaces and four (4) approved outdoor spaces. Ms. Shaffer accompanied us as we visited each classroom and outdoor play areas. All approved indoor and outdoor spaces were observed today. There were eighty-three (83) children enrolled and seventy-nine (79) children, ages 0-5, present today. Daily schedules were available for each group of children. Activity plans were observed posted in six (6) of the seven (7) classrooms. A variety of age-appropriate learning materials were observed in each classroom. In space #1, infants received care based on their individual needs. Infant feeding schedules were observed, and safe sleep charts were being maintained. Caregivers were engaged in play with infants, recorded safe sleep checks, and bottle fed children. Toddlers and preschool children in spaces 2-7 were observing participating in free choice activities, circle time, morning snack, and personal care routines such as toileting. Meals are brought home daily. Per the facilities “lunch and snack requirements” parents must follow the USDA meal guidelines. The program provides drinking milk for lunch daily and supplemental food groups for any child who is lacking a component. Routine toileting and handwashing procedures were monitored, and requirements were met. Staff/child ratios were in compliance in all classrooms. All children were adequately supervised. Your program currently operates with a 5 Star License, issued March 7, 2022, earning 5 points in Education, 7 points in Program Standards and 1 Quality Point in the Programmatic Option. Restrictions on the permit include: a capacity of 100 during first shift with an age range of 0-12 years old, daytime care only, meets enhanced space, and meets enhanced ratios per .2818(c). The last annual compliance visit was conducted at this facility on October 28, 2024. As of October 14, 2025, your facility has an 18-month compliance history for the facility was 100%. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Fire Inspection – October 24, 2025 – Approved for day time care only - Satisfactory Sanitation – 04/29/2025 – 10 Demerits – Superior Director, Rachel Shaffer, was present and available for consultation. There are seven (7) approved indoor spaces and four (4) approved outdoor spaces. Ms. Shaffer accompanied us as we visited each classroom and outdoor play areas. All approved indoor and outdoor spaces were observed today. There were eighty-three (83) children enrolled and seventy-nine (79) children, ages 0-5, present today. Daily schedules were available for each group of children. Activity plans were observed posted in six (6) of the seven (7) classrooms. A variety of age-appropriate learning materials were observed in each classroom. In space #1, infants received care based on their individual needs. Infant feeding schedules were observed, and safe sleep charts were being maintained. Caregivers were engaged in play with infants, recorded safe sleep checks, and bottle fed children. Toddlers and preschool children in spaces 2-7 were observing participating in free choice activities, circle time, morning snack, and personal care routines such as toileting. Meals are brought home daily. Per the facilities “lunch and snack requirements” parents must follow the USDA meal guidelines. The program provides drinking milk for lunch daily and supplemental food groups for any child who is lacking a component. Routine toileting and handwashing procedures were monitored, and requirements were met. Staff/child ratios were in compliance in all classrooms. All children were adequately supervised. Caregivers and children interacted in a positive and appropriate manner. Staff and Training Worksheets- There were twenty-one (21) staff members employed at this facility. There were four (4) new staff; all new staff and a percentage of veteran staff’s files were reviewed. Health & Safety Training – All staff members who have been employed for at least one year, have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. New staff are in the process of completing these training courses. Health and safety trainings are required to be completed during the first year of employment and renewed every 5 years. A checklist was used to note the requirements I monitored today, including staff and children’s files. Child Care Consultant, Heather Elliott, accompanied me on today’s visit. Your program currently operates with a 5 Star License, issued March 7, 2022, earning 5 points in Education, 7 points in Program Standards and 1 Quality Point in the Programmatic Option. Restrictions on the permit include: a capacity of 100 during first shift with an age range of 0-12 years old, daytime care only, meets enhanced space, and meets enhanced ratios per .2818(c). The last annual compliance visit was conducted at this facility on October 28, 2024. As of October 14, 2025, your facility has an 18-month compliance history for the facility was 100%. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Fire Inspection – October 24, 2025 – Approved for day time care only - Satisfactory Sanitation – 04/29/2025 – 10 Demerits – Superior Director, Rachel Shaffer, was present and available for consultation. There are seven (7) approved indoor spaces and four (4) approved outdoor spaces. Ms. Shaffer accompanied us as we visited each classroom and outdoor play areas. All approved indoor and outdoor spaces were observed today. There were eighty-three (83) children enrolled and seventy-nine (79) children, ages 0-5, present today. Daily schedules were available for each group of children. Activity plans were observed posted in six (6) of the seven (7) classrooms. A variety of age-appropriate learning materials were observed in each classroom. In space #1, infants received care based on their individual needs. Infant feeding schedules were observed, and safe sleep charts were being maintained. Caregivers were engaged in play with infants, recorded safe sleep checks, and bottle fed children. Toddlers and preschool children in spaces 2-7 were observing participating in free choice activities, circle time, morning snack, and personal care routines such as toileting. Meals are brought home daily. Per the facilities “lunch and snack requirements” parents must follow the USDA meal guidelines. The program provides drinking milk for lunch daily and supplemental food groups for any child who is lacking a component. Routine toileting and handwashing procedures were monitored, and requirements were met. Staff/child ratios were in compliance in all classrooms. All children were adequately supervised. Caregivers and children interacted in a positive and appropriate manner. Staff and Training Worksheets- There were twenty-one (21) staff members employed at this facility. There were four (4) new staff; all new staff and a percentage of veteran staff’s files were reviewed. Health & Safety Training – All staff members who have been employed for at least one year, have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. New staff are in the process of completing these training courses. Health and safety trainings are required to be completed during the first year of employment and renewed every 5 years. Remember, the Recognizing and Responding to Suspicions of Child Maltreatment training must be taken at www.preventchildabusenc.org, within the first 90 days of employment. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. Ensure this information in completed, recorded on the required form, and on file for review. The following violations were observed and documented today. All violations must be corrected immediately. Please review child care rules regarding storage and accessibility of potentially hazardous items. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. You, Rachel Shaffer, Director, and Cyndi Meyer, Assistant Director, assisted me with the visit. Your program currently operates with a 5-star license, issued March 7, 2022, earning 5 points in the education component, 7 points in the program standards component meeting enhanced ratios, enhanced space, and reducing ratios per .2818(c), and 1 quality point for having a staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Lynette Robbins, Licensing Supervisor, assisted me on today’s visit. The last annual compliance visit was conducted December 1, 2022. The sanitation inspection was completed November 3, 2023, with a “Superior” classification. The last fire inspection was conducted November 17, 2022, and your facility was approved for daytime care only. You were reminded via email that your fire inspection is due this month. You replied that you would call and request the inspection. The center's compliance history was reviewed with the operator. The program’s compliance history was 93% as of today’s date. The NC Secretary of State website was reviewed on November 3, 2023, and Masonboro Baptist Church, Inc. was listed as current- active. Mrs. Meyer visited each indoor and outdoor space with me. Children were enrolled in all licensed indoor spaces. Infants in space 1 were observed in free play with age appropriate materials and activities. Safe sleep and feeding schedules were monitored and found in compliance. Toddlers in space 2 ate snack (all meals are brought from home). Preschool aged children in spaces 3-6 either participated in group circle time or free play. All classrooms had current activity plans posted and all aligned with NC Early Childhood Foundations. The following violations were documented. Technical Assistance: For the safety of the children, ensure any plastic bags used for storage are kept inaccessible to children under the age of three years old by keeping them at a minimum height of five feet off the floor. N. Nelson did not have her Recognizing and Responding to Child Maltreatment certification on file. According to Health and Safety training record this was completed on 1/21/22; however, this certification was not in her file. An infant who started September 5, 2023 did not have a medical exam on file. All medical exams must be obtained within the child’s first 30 days. Rated License: The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year won’t begin until July 1, 2024, the preparation year for cohort one begins July 1, 2023. This is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. Use staff meetings to discuss topics related to the Environment Rating Scales or other quality initiatives. For example, if transitions are challenging, ask staff what strategies help, or which transitions including a contact hour earning webinar and Thinking More worksheets for every rating scale. Engage to brainstorm, offer support and clarification, and set goals. Allocate resources so they can focus on self-study as part of the workday to help everyone become more familiar and confident with the assessment process. Also, be reminded that NCRLAP outreach assessments are still being offered if you wish to “practice.” You may also wish to contact technical assistance specialists with One Place to help you with mock assessments. Ms. Simmons advised she participated in an outreach assessment (ITERRS-R) and plans to participate in other assessments prior to completion year of your reassessment. Related to education, ensure you are uploading education has staff completed classwork. The sanitation inspection was completed November 3, 2023, with a “Superior” classification. You replied that you would call and request the inspection. The center's compliance history was reviewed with the operator. The program’s compliance history was 93% as of today’s date. You visited each indoor and outdoor space with me. Children were enrolled in all licensed indoor spaces. Daily schedules were available for children. Infants and toddlers enrolled in spaces 1 and 2 were observed on their designated playground engaging in free play activities such as pretending to have a cookout, riding on ride on toys and playing with baby dolls. Children ages 2-3 enrolled in spaces 3-4 and preschool aged children enrolled in space 6 were also observed on their playground in free play with water play, constructions trucks, balance beams, balls, and slides. Children ages 3-5 in spaces 5 and 7 were observed in their classrooms participating in free play with age-appropriate materials and activities that coincide with their activity plans and aligned with NC Early Childhood Foundations such as pretending to camp, finger painting, and letter magnets. All lunches are brought from home. There was one violation observed today and must be corrected immediately: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A Diaper Cream Form in Space #2 and a Nebulizer with Medical Action Form in Space #5 were expired and medications were not returned to the parents after the course of treatment was completed or authorization had expired. .0803(12) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. For each diaper cream or topical medication in the classroom, there should be a form filled out with permission to administer the cream from the parent. Permission to administer medication forms can be filled out for the length of time a parent will want you to administer the ointment for up to one year. When diaper cream forms are filled out by parents, you should check that the correct expiration date is noted on the topical ointment permission form and that it matches the expiration date on the medication and that the expiration date for the form is filled in correctly for the correct duration the medication is to be given. If a parent brings in a diaper cream to replace another cream, a new form must be filled out for that cream. A medical action form in Space # 5 was expired and medication was not returned to the parent. For any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be attached to the application. The medical action plan shall be completed by the child's parent or a health care professional and may include the following: (1) a list of the child's diagnosis or diagnoses including dietary, environmental, and activity considerations that are applicable; (2) contact information for the child's health care professional(s); (3) medications to be administered on a scheduled basis; and (4) medications to be administered on an emergency basis with symptoms, and instructions. This plan is only needed for those children with the previously listed conditions and expires after one year. Technical Assistance: Today technical assistance was given on what qualifies as plastic that is in reach for children under the age of three. Today we discussed continuing to store any plastic bags used for storage, even the wrapping on the diapers, inaccessible to children under the age of three years old by keeping them at a minimum height of five feet off the floor. Reminders: At the Monday, February 6, 2023, meeting of the Child Care Commission, Commissioners agreed to form a committee to modernize NC star rated system. The North Carolina Child Care Commission and the NCDHHS Division of Child Development and Early Education (DCDEE) are collaborating to modernize North Carolina's Quality Rating and Improvement System (QRIS) commonly known as the Star Rated License. During the next few months, the Division is collecting information from parents, teachers, administrators, operators, and partners to consider in a plan that will be reviewed by the North Carolina General Assembly in Spring 2024. We invite you to participate in open discussion opportunities for Commission members and Division staff to listen and learn about your experiences and challenges with the Star Rated Child Care License System.
