Mary Courtney Warren, Director, assisted me with the visit. Your program currently operates with a five-star license, issued May 19, 2022, earning 7 points in the education component, 7 points in the program standards component meeting enhanced space and ratios and 1 quality point for having enhanced policies and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted October 10, 2024. The sanitation inspection was completed May 14, 2025, with a “Superior” classification. The last fire inspection was conducted January 28, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator and prior to today’s visit the compliance history score was 93%. The NC Secretary of State website was reviewed today and Milestones Learning Center, LLC was listed as current- active. You visited indoor and outdoor spaces with me. Infants in spaces 1 and 2 received care according to individual needs including transitions, bottle feeding, and free play. Safe sleep requirements were monitored and found in compliance. Preschool aged children in all other spaces participated in free play with age-appropriate materials and activities both indoors and outdoors or they ate morning snack that consisted of biscuits and milk. Appropriate handwashing practices were observed. All activity plans were posted, and evidence of NC Early Childhood Foundations was present. Staff engaged in informal conversations with the children and children were tended to in a nurturing and caring manner. The following violation was observed and corrected during the visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Hand sanitizer in space 9 (turtles) was not stored a minimum of 5 feet out of reach or in a locked cabinet/room. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance: Please review child care rules regarding storage and accessibility of potentially hazardous items. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. During today’s visit, children were observed participating in free play indoor and outdoors with age-appropriate materials and activities or they were having routine care needs met such as feedings, and toileting/diapering. Snack served today consisted of French toast sticks and milk. Eight (8) new staff member have been hired since the last annual compliance. Files were monitored and found in compliance. The following violation was observed and must be corrected immediately: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. An emergency inhaler permission form in space 9 (sea turtles) expired in February 2025. .0803(12) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. During today’s visit, infants in Spaces 1 were observed sleeping in appropriate cribs, exploring the environment, and completing routine tasks with a caregiver. Toddlers in Spaces 2 and 3 were engaged in gross motor play in the classroom, throwing balls, and playing musical instruments. Two-year-old children in Spaces 4 and 5 were engaged in activity areas with puzzles, dancing to music, and shaking rattles. Three-year-old children in Space 6 were engaged in activity areas building blocks, pretending to cook, and washing baby dolls in a water sensory table. Children in Space 7 were engaged in group time reading books and playing search and find with books. Preschool children in Space 8 were engaged with caregivers drawing with markers and playing a ball toss game together. Preschool children in Space 9 were engaged in group time discussing letter bags. Preschool children in Space 10 were engaged in activity areas pretending to be on an egg hunt, drawing with crayons on large egg cut outs, and engaged in a spring sensory table. Lunch served today consisted of chicken nuggets, French fries, green beans, roll, and milk. One new staff member was hired since the last annual compliance. Files were monitored and found in compliance. The following violations were observed and must be corrected immediately: Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. An epi-pen in Space 10 was not in the original labeled container. .0803(2)(a) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. The epi-pen medication in Space 10 was not stored in the prescription box with a pharmacy label on the original container. Today we discussed that all prescription medications must be stored in the prescription box or have the pharmacy label on them so that we can confirm that the medication belongs to the child enrolled. You stated today that you would have the parents obtain the label for the medication. For any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be submitted. The medical action plan shall be completed by the child's parent or a health care professional. As we discussed today, this plan is valid for one year. The action plan in Space 10 was signed on 12/5/22 and therefore expired on 12/5/23. The medication administration form is valid for six months. We discussed today that this form gives you permission to give the medication from the parents or guardian for six months. Technical Assistance: Today there was a fan in the Preschool room that was not in use. We discussed reminding staff if they did use the fan that it will need to be 5 feet off the floor at all times or there will need to be a net added to the outside to ensure that children cannot access the blades. Reminders: Childcare licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. Your program currently operates with a five-star license, issued May 19, 2022, earning 7 points in the education component, 7 points in the program standards component meeting enhanced space and ratios and 1 quality point for having enhanced policies and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Today’s visit was conducted by Child Care Consultant April Bosse on behalf of your Child Care Consultant April Lester. The last annual compliance visit was conducted October 16, 2023. The sanitation inspection was completed September 18, 2023, with a “Superior” classification. The last fire inspection was conducted January 9, 2024, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator and prior to today’s visit the compliance history score was 94%. The NC Secretary of State website was reviewed today and Milestones Learning Center, LLC was listed as current- active. You, Kendra Murphy, Director, were available for consultation for this visit. Ten classrooms are operating with one hundred and forty-nine children enrolled and one hundred thirty-four children present. All children were appropriately supervised. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. During today’s visit, infants in Spaces 1 were observed sleeping in appropriate cribs, exploring the environment, and completing routine tasks with a caregiver. Toddlers in Spaces 2 and 3 were engaged in gross motor play in the classroom, throwing balls, and playing musical instruments. Two-year-old children in Spaces 4 and 5 were engaged in activity areas with puzzles, dancing to music, and shaking rattles. Three-year-old children in Space 6 were engaged in activity areas building blocks, pretending to cook, and washing baby dolls in a water sensory table. Children in Space 7 were engaged in group time reading books and playing search and find with books. Preschool children in Space 8 were engaged with caregivers drawing with markers and playing a ball toss game together. Preschool children in Space 9 were engaged in group time discussing letter bags. Preschool children in Space 10 were engaged in activity areas pretending to be on an egg hunt, drawing with crayons on large egg cut outs, and engaged in a spring sensory table. Lunch served today consisted of chicken nuggets, French fries, green beans, roll, and milk. One new staff member was hired since the last annual compliance. Files were monitored and found in compliance. The following violations were observed and must be corrected immediately: Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. An epi-pen in Space 10 was not in the original labeled container. .0803(2)(a) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. The medical action plan for an epi-pen in Space 10 was not updated on an annual basis. .0801(b) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 11, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. The epi-pen medication in Space 10 was not stored in the prescription box with a pharmacy label on the original container. Today we discussed that all prescription medications must be stored in the prescription box or have the pharmacy label on them so that we can confirm that the medication belongs to the child enrolled. You stated today that you would have the parents obtain the label for the medication. For any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be submitted. The medical action plan shall be completed by the child's parent or a health care professional. As we discussed today, this plan is valid for one year. The action plan in Space 10 was signed on 12/5/22 and therefore expired on 12/5/23. The medication administration form is valid for six months. We discussed today that this form gives you permission to give the medication from the parents or guardian for six months. Technical Assistance: Today there was a fan in the Preschool room that was not in use. We discussed reminding staff if they did use the fan that it will need to be 5 feet off the floor at all times or there will need to be a net added to the outside to ensure that children cannot access the blades. Reminders: Childcare licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. Infants in spaces 1 and 2 received care according to individual needs including nap transitions, bottle feeding, and free play. Safe sleep requirements were monitored and found in compliance. Children enrolled in spaces 3, 4, 5, and 8 were observed in outdoor free play. Staff actively supervised children during play. Children in spaces 6, 7, and 9 participated in free play with age-appropriate materials and activities. Preschool aged children in space 10 ate morning snack. Appropriate handwashing practices were observed. All activity plans were posted, and evidence of NC Early Childhood Foundations was present. Lunch consisted of Salisbury steak, mashed potatoes, peas, whole wheat rolls, and milk. The following violations were documented. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. The cabinet in space 8 that housed aerosol was not locked. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Diaper creams stored in a drawer below 5 feet in height was not locked in space 3. 15A NCAC 18A .2820(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Since the violation was corrected immediately, a compliance letter is not due to me at this time. Technical Assistance: Today, cabinets housing aerosol spray and drawers housing diaper creams were not remained locked at all times. Staff must ensure these cabinets and drawers are locked at all times. You have recently had the mulch replenished with triple shredded mulch. Today we discussed how this type of mulch decomposes more quickly than playground mulch which is going to require you to fluff the mulch more frequently to maintain it’s integrity.
