Blair Elementary School Facility ID: 65001068 Consultant: SHEREEN PICKETT Operation Type: Center Case Number: Visit Date: 5/13/2026 Number Present: 35 Completed Date: 5/13/2026 Age: From 3 To 5 Total Minutes: 135 Time In: 09:00 AM Time Out: 11:15 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of my annual compliance visit was to monitor compliance with applicable child care requirements pertinent to this type of visit. A checklist was used to monitor requirements. This facility currently operates with a Five Star license issued March 04, 2025, earning 7 points in Education, 7 points in Program, and 1 quality point for choosing a Programmatic Option. Restrictions on the permit include a first shift capacity of 36 children ages 3 to 5 years old; daytime care only, meeting enhanced space and ratios minus one. Restrictions were in compliance during today’s visit. This facility is operated through New Hanover County Schools. Prior to this visit, the facility’s eighteen-month compliance history was 100%. The last annual compliance visit was conducted by Child Care Consultant Shereen Pickett on September 11, 2025. Sanitation Inspection – 04/15/2026 - Superior classification. Fire Inspection – 10/08/2025 - Facility was approved for daytime only A. Harrison, Assistant Principal, was present and available for consultation. Two indoor spaces and one outdoor space is approved for use with the children. Children in space 175 were engaged in free play, accessing a variety of age-appropriate materials. Children in space 173 were observed during gross motor play outdoors. Adequate supervision and positive interactions were observed. Breakfast consisted of pancakes, oranges, and milk. Regarding the QRIS System and Pathways to the Stars – Currently, this program is county funded and licensed to care for children three to five-years old. The program completed the ECERS-R assessment in December 2024, and is not required to meet the deadline of transitioning to the new QRIS permit until December 2027. I spoke with J. Mullins, Interim Supervisor of the Early Childhood Education Department via telephone. Ms. Mullins stated the plan is for the program to follow Pathway #1 – Program Assessment for Child Care Centers and apply for the transition in the Fall of 2027, with a completion deadline of December 2027. Ms. Mullins will reach out to me for next steps as time progresses. The Following violations were observed and must be corrected immediately: Violation Number Comment Rule 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. In space 175, a bottle of Pure Leaf Sweet Tea, a bottle of flavored sparkling water and a Twix chocolate bar were observed stored on the teacher desk, within arm's reach of children in care. .0901(i) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The depth of the mulch on the playground used with the preschool children measured less than two inches, rather than the required six inches around the large climber and slide. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 26, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification. TECHNICAL ASSISTANCE PROVIDED FOR VIOLATIONS CITED: Staff members should serve as positive role models when caring for children. Foods and beverages not allowed to be served to children such as sweet tea and candy should not be consumed in front of children. If staff members choose to consume beverages other than water, remember to use a covered cup. For the health and safety of the children in care, and in hope of reducing injuries, additional mulch is required around and under the large climber/slide used with the preschool children. Currently, the mulch measures approximately two inches and less in some areas. OTHER: According to the Clean Classrooms for Carolina Kids website, information for your facility is as follows: • Water Testing-Completed in September 2024. Water testing must be completed every three (3) years. • Lead-Based Paint-Exempt • Asbestos-Exempt REMINDERS: A monthly fire drill and playground inspections are due in May. J. Shaw and M. The facility’s Temporary License was issued September 3, 2024 and is valid through March 3, 2025. Restrictions on the permit include a first shift capacity of 36 children ages 3 to 5 years old; no children under 3; daytime care only. Child Care Consultant April Lester was present and assisted with today’s visit. The facility had the following items posted/located in a prominent place in the center for parents and available for me to review: the Temporary License, Safe Arrival and Departure Procedure, fire drill log, emergency medical care plan and the Summary of the NC Child Care Law. Five (5) children’s records and all staff files were reviewed today. The last fire drill was completed September 9, 2024. Thirty-five children ages 3 to 5 years old were enrolled; 27 children were present today. All approved indoor and outdoor spaces were monitored today. Each space is arranged into activity areas to include but are not limited to art, blocks, dramatic play, music, sand and water, cozy, science, manipulatives, writing and language. A variety of age-appropriate learning materials were accessible in each activity area. Today children were observed building wild animal enclosures for a zoo, putting on make-up and feeing babies, drawing, gluing collage materials, playing with plastic bug in the sensory table and moving objects with magnetics. Outside children were swinging, running, climbing, riding scooters, balancing on balancing steps, playing in the sandbox. Both indoors and outdoors staff moved about the space interacting with children and ensuring visual supervision. Lunch served today was peanut butter and jelly sandwiches, oranges, cucumber slices and milk. Violation Number Comment Rule 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. The written permission to administer medication did not contain the required information. 10A NCAC 09 .0803(2)(b)(i-v) The above violation was observed today and must be corrected immediately. A compliance letter including detailed information about how the violation has been corrected must be received by October 11, 2024. I strongly recommend you review North Carolina Child Care Rule 10A NCAC 09. 0803 to ensure medication is documented and administered appropriately. Sample medication forms and medical action plans are available on the DCDEE website under provider documents. All medications must be accompanied by written authorizations from parents with complete instructions for administration. Prescription medications must also be in the original container with the pharmacy label attached. For any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be attached to the application. The medical action plan shall be completed by the child's parent or a health care professional and may include the following: (1) a list of the child's diagnosis or diagnoses including dietary, environmental, and activity considerations that are applicable; (2) contact information for the child's health care professional(s); (3) medications to be administered on a scheduled basis; and (4) medications to be administered on an emergency basis with symptoms, and instructions. Child Care rule 10A NCAC 09 .0803 (6) states a parent may give a caregiver standing authorization for up to six months to administer prescription or over-the-counter medication to a child, when needed, for chronic medical conditions, such as asthma, and for allergic reactions. Only official unopened transcripts will be submitted to the Workforce Education Unit, all other documents will be uploaded by the applicant. All staff need to ensure current education information has been submitted to WORKS and they have requested the appropriate evaluation for their position. We suggest you store the sheets in the children’s personal cubbies and teach them out to make their beds each day, this is a great self-help skill to learn and it will reduce the spread of germs. Trainings: EPR: At least one staff must complete Emergency Preparedness and Response in Child Care training within one year of the effective date of the temporary license. You may contact Region 4 Child Care Resource & Referral for training availability. Maintain documentation of this training. Once the EPR training is completed, you must complete an EPR Plan on a template at the following website https://rmp.nc.gov/portal/portal.aspx within four (4) months. After you complete the Emergency Preparedness and Response in Child Care training, you must conduct either a shelter-in-place or a lockdown drill every three months. Use the Emergency Drill Log to document these additional drills as well as your monthly fire drills. The facility’s Temporary License was issued September 3, 2024 and is valid through March 3, 2025. Restrictions on the permit include a first shift capacity of 36 children ages 3 to 5 years old; no children under 3; daytime care only. Child Care Consultant April Lester was present and assisted with today’s visit. The facility had the following items posted/located in a prominent place in the center for parents and available for me to review: the Temporary License, Safe Arrival and Departure Procedure, fire drill log, emergency medical care plan and the Summary of the NC Child Care Law. Five (5) children’s records and all staff files were reviewed today. The last fire drill was completed September 9, 2024. Thirty-five children ages 3 to 5 years old were enrolled; 27 children were present today. All approved indoor and outdoor spaces were monitored today. Each space is arranged into activity areas to include but are not limited to art, blocks, dramatic play, music, sand and water, cozy, science, manipulatives, writing and language. A variety of age-appropriate learning materials were accessible in each activity area. Today children were observed building wild animal enclosures for a zoo, putting on make-up and feeing babies, drawing, gluing collage materials, playing with plastic bug in the sensory table and moving objects with magnetics. Outside children were swinging, running, climbing, riding scooters, balancing on balancing steps, playing in the sandbox. Both indoors and outdoors staff moved about the space interacting with children and ensuring visual supervision. Lunch served today was peanut butter and jelly sandwiches, oranges, cucumber slices and milk. Violation Number Comment Rule 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. I strongly recommend you review North Carolina Child Care Rule 10A NCAC 09. 0803 to ensure medication is documented and administered appropriately. Sample medication forms and medical action plans are available on the DCDEE website under provider documents. All medications must be accompanied by written authorizations from parents with complete instructions for administration. Prescription medications must also be in the original container with the pharmacy label attached. For any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be attached to the application. The medical action plan shall be completed by the child's parent or a health care professional and may include the following: (1) a list of the child's diagnosis or diagnoses including dietary, environmental, and activity considerations that are applicable; (2) contact information for the child's health care professional(s); (3) medications to be administered on a scheduled basis; and (4) medications to be administered on an emergency basis with symptoms, and instructions. Child Care rule 10A NCAC 09 .0803 (6) states a parent may give a caregiver standing authorization for up to six months to administer prescription or over-the-counter medication to a child, when needed, for chronic medical conditions, such as asthma, and for allergic reactions. Only official unopened transcripts will be submitted to the Workforce Education Unit, all other documents will be uploaded by the applicant. All staff need to ensure current education information has been submitted to WORKS and they have requested the appropriate evaluation for their position.
