Dr. Karen McCarty, Administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Dr. McCarty and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were confirmed during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On March 14, 2024, a staff member used a harsh tone and threatened retaliation to a five-year-old child. G.S. 110-91(10) 904 Child was handled roughly. On March 14, 2024, a staff member handled a five-year-old child roughly by grabbing and holding the child's wrist and making the child hit themselves in the face and then kicked the child on the leg. .1803(a)(1) 1810 There was a substantiation of child maltreatment. Pursuant to its investigation, the Division has confirmed sufficient information to determine child maltreatment. GS 110-105.6(a) Violations must be corrected immediately. This program is operated by New Hanover County Schools. April Bosse, Child Care Consultant, assisted with today’s visit. Sanitation Inspection – 11/08/2023 - Superior classification Fire Inspection – 11/30/2023 - Facility approved for daytime care only Karen McCarty, Director, and Kristin Zamorski, Assistant Director, were present and available for consultation. All indoor and outdoor spaces approved for use by the children were visited. Preschool children in spaces K1, K3, K4, B1, B2, D2, D3 and C4 were observed outdoors in free play with age-appropriate materials such as balls, hoops, jump ropes (supervised), and gross motor stationary equipment. Preschool children in spaces K2, D1, D4, C3, C1, C2, A1, A2, and A3 were observed indoors eating snack, participating in circle time, or engaged in free play with age-appropriate materials and activities. Lunch for today is peanut butter and jelly sandwiches or baked chicken, cheese sticks, broccoli, applesauce, and milk. The NC Pre-Kindergarten Program Site Monitoring Tool was reviewed today. The tool has been completed by the contract administrator. The program follows the New Hanover County school calendar, the length of the day meets requirements, and all staff meet education requirements. E program uses the most current Creative Curriculum for Preschool, has been implemented and the assessment tool, GOLD, Teaching Strategies are all in place. Three formal assessments with parent conferences are scheduled through the school year. Portfolios are maintained on Teacher Strategies, GOLD by taking pictures of the children’s work and uploading them to the program. The facility uses the developmental screening tool, Brigance Early Childhood Screen III (3-5 Years). NC Pre-K requirements were met today. The following violation was observed; however, it was corrected during the visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Lysol wipes were observed in an unlocked filing cabinet in space K1. They were removed by administration during the visit. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance: Remember that any product labeled "keep out of reach of children" without any other warnings, shall be inaccessible to children when not in use, but is not required to be kept in locked storage. A variety of age-appropriate learning materials was observed in the classroom. Preschool children in K-1, K-2, K-3, K-4, B-1, B-2 and C-4 were outdoors playing on slides and climbing equipment, playing in sandboxes, playing basketball, swinging on swings, having races, and riding on scooters. Children in D-3, and C-1 were participating in group time with teachers while reading books, and dancing to the freeze dance song. Groups D-1, D-2, and D-4 were observed transitioning from personal routines to outside or back to the classroom. Preschool children in A-1, A-2, C-2, and C-3 were engaged in activity areas with dot markers, painting on an easel, using watercolors to paint pumpkin pictures, dressing baby dolls, or eating a snack of crackers and apple juice. Lunch planned for today consists of pasta with meat sauce, broccoli, mandarin oranges, rolls, and milk. NC Pre-K Requirements were monitored today. The NC Pre-K Monitoring Tool was reviewed. The tool was not yet signed by the NC Pre-K Site administrator but is scheduled to be completed soon. The NC Pre-K Monitoring Tool will be reviewed in its entirety during your next Routine Unannounced visit to ensure that all requirements were met. The program follows the New Hanover County School calendar, the length of the day meets requirements, and all staff meet education requirements. The Creative Curriculum 6th Addition has been implemented and the assessment tool, Teaching Strategies Gold, is in place. Three formal assessments with parent conferences are scheduled throughout the school year. Portfolios are maintained on Teacher Strategies Gold by taking pictures of the children’s work and uploading them to the program and were observed today. All required screenings were completed at the beginning of the year. NC Pre-K requirements were met today. One violation was observed and corrected during the visit. Violation Number Comment Rule 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. Toothpaste in spaces K-4 and C-2 was stored in a low drawer and on top of a low shelf and was not made inaccessible to children. This is a repeat violation. .0604(a) Due to the fact that this is a repeat violation, a compliance letter, explaining the program's plan of action to ensure a similar violation does not occur is due before or by October 06, 2023. This program was issued a Five Star License on January 18, 2022, earning 7 points in Education, 7 points in Program, and 1 quality point for choosing a Programmatic Option. Restrictions on the permit include: daytime care only, meets enhanced space, meets enhanced ratios as outlined in 10A NCAC 09 .2818(c). Prior to the visit the compliance history score was 97%. This program is operated by New Hanover County Schools. Child Care Consultant April Bosse assisted with this visit. Sanitation Inspection – 05/01/2023 - Superior classification Fire Inspection – 05/04/2023 - Facility approved for daytime care only K. McCarty, Principal/Administrator was present and available for consultation. M. Mantis, Administrator, accompanied April Bosse during the walkthrough. All indoor and outdoor spaces approved for use by the children were visited. Daily schedules were available for children. Activity plans were observed posted in the classroom. A variety of age-appropriate learning materials was observed in the classroom. Preschool children in K-1, K-2, K-3, K-4, B-1, B-2 and C-4 were outdoors playing on slides and climbing equipment, playing in sandboxes, playing basketball, swinging on swings, having races, and riding on scooters. Children in D-3, and C-1 were participating in group time with teachers while reading books, and dancing to the freeze dance song. Groups D-1, D-2, and D-4 were observed transitioning from personal routines to outside or back to the classroom. Preschool children in A-1, A-2, C-2, and C-3 were engaged in activity areas with dot markers, painting on an easel, using watercolors to paint pumpkin pictures, dressing baby dolls, or eating a snack of crackers and apple juice. Lunch planned for today consists of pasta with meat sauce, broccoli, mandarin oranges, rolls, and milk. NC Pre-K Requirements were monitored today. The NC Pre-K Monitoring Tool was reviewed. The tool was not yet signed by the NC Pre-K Site administrator but is scheduled to be completed soon. The NC Pre-K Monitoring Tool will be reviewed in its entirety during your next Routine Unannounced visit to ensure that all requirements were met. The program follows the New Hanover County School calendar, the length of the day meets requirements, and all staff meet education requirements. The Creative Curriculum 6th Addition has been implemented and the assessment tool, Teaching Strategies Gold, is in place. Three formal assessments with parent conferences are scheduled throughout the school year. Portfolios are maintained on Teacher Strategies Gold by taking pictures of the children’s work and uploading them to the program and were observed today. All required screenings were completed at the beginning of the year. NC Pre-K requirements were met today. One violation was observed and corrected during the visit. Violation Number Comment Rule 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. Toothpaste in spaces K-4 and C-2 was stored in a low drawer and on top of a low shelf and was not made inaccessible to children. This is a repeat violation. .0604(a) Due to the fact that this is a repeat violation, a compliance letter, explaining the program's plan of action to ensure a similar violation does not occur is due before or by October 06, 2023. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Please be aware any information submitted by you is considered legal documentation. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Technical assistance related to violation cited: In spaces K-4 and C-2, toothpaste was observed on top of counter and inside a low cabinet drawer that was accessible to children. Any product which is labeled "keep out of reach of children" without any other warnings, shall be inaccessible to children when not in use, but is not required to be kept in locked storage. The product shall be considered inaccessible to children when stored on a shelf or in an unlocked cabinet that is mounted a minimum vertical distance of five feet above the finished floor.
