    Name of Operation: CHILDREN'S COTTAGE OF WILMINGTON 2 Facility ID: 65001063 Consultant: SHEREEN PICKETT Operation Type: Center Case Number: Visit Date: 3/12/2026 Number Present: 12 Completed Date: 3/12/2026 Age: From 4 To 5 Total Minutes: 170 Time In: 11:30 AM Time Out: 02:20 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s annual compliance with rated license visit was to monitor compliance with applicable child care requirements pertinent to this visit type and to begin the reassessment process. A checklist was used to document requirements monitored today. This facility currently operates with a Four-Star license issued December 01, 2022, earning 7 points in Education, 2 points in Program, and 1 quality point for choosing a Programmatic Option. The last annua compliance visit was conducted on March 20, 2025. Prior to the visit, the facility had an eighteen-month compliance history score of 99%. Sanitation – 9/26/2025 – Superior Classification Fire – 2/21/2025 – Approved for daytime care only M. Wooten, Administrator, and K. Sidbury, Assistant Administrator, were present and available for consultation. According to the North Carolina Secretary of State website, your corporation Premier NC Wilmington LLC is current and active. All approved indoor and outdoor spaces were monitored. Infants are not currently enrolled in this building. There are thirty-one (31) children enrolled and twelve children, four and five years old present today. Preschool-aged children in space 6 were observed during outdoor gross motor play, followed by handwashing and lunch routines. After lunch, children completed toileting routines and prepared for rest time. Individual mats and linens were provided. Mats were spaced out to meet compliance. Spaces 4 and 5 are not currently in use. Spaces 1-3 are used with school-aged children. Due to the timing of the visit, school-age children were not present. Lunch is provided from home daily. Regarding the QRIS and the program’s intent to transition to the Pathways: Today, the conversation template was completed. The program is considering Pathway #1 -Program Assessment or Pathway #2 – Classroom and Instructional Quality. Today, I discussed the requirements for meeting Pathway #1 and Pathway #2 with Ms. Wooten. I also emailed the latest version of the application to her for review. Ms. Wooten stated she will review all the forms and begin working towards completion of requirements. Once all require paperwork is completed, the program intends to submit paperwork, along with the application to me for review. The program is hoping to be transitioned to the new permit by September/October 2026. Curriculum - The program understands that an approved curriculum must be used since four-year-old children are enrolled. A copy of the approved list was emailed to Ms. Wooten today. Education - All staff need to ensure that current education information has been submitted to WORKS and they have requested an appropriate evaluation for their position. Staff education can only be verified through WORKS status letters. It typically takes WORKS 4-6 weeks to complete an evaluation. The facility will need to complete and submit the Staff Information and Education Worksheet to me no later than August 28, 2026. Once all your staff’s education has been evaluated, your facility will earn their Star Rated License based on the following: 50% of Lead Teachers and 50% of other educators. (“Educator” means all classroom teaching staff and administrative positions, including education coordinators, curriculum specialists and any staff who have responsibility for planning, caregiving, mentoring or training. To count as an educator meeting 50% of the education standards, an individual 2must meet the requirements of a position and work on-site, full-time.) The following violations were observed and must be corrected immediately. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed on February 21, 2025. 10A NCAC 09 .0304(a) 1898 Staff did not complete the health and safety training within one year of employment. One staff member, hired on April 24, 2023 did not complete all health and safety training topics within one year of employment as required. .1102(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before March 23, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification. Technical Assistance Provided for Violations Cited: M. Mathews, hired on April 24, 2023, did not complete all health and safety training topics within one year of employment as required. Ms. Mathews must complete and log all the required topics to come into compliance. The last fire inspection was completed on February 21, 2025. I reached out to Rachel Fleshman with the fire marshal’s office today and requested that a new inspection is completed soon. Ms. Fleshman stated she understood and will try to accommodate the program’s need for a new inspection as soon as possible. Ensure a copy of the new inspection is submitted to me upon completion. Other Information: Staff and Training Worksheets- There were four staff members employed at this facility. There was one new staff member hired since the last visit. All new staff and a percentage of veteran staff’s files were reviewed today. Required information and trainings for all existing staff were also monitored.
