You, Wendy Lindquist, Assistant Director, assisted with the visit. The program currently operates with a five-star license, issued September 2, 2024, earning 6 points in the education component, 6 points in the program standards component meeting enhanced ratios and enhanced space, and 1 quality point. The program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on February 17, 2025. The last sanitation inspection was conducted on September 9, 2025, and the program received a supervisor classification. The last fire inspection was conducted on January 8, 2026, and the program was approved for day time care only. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eight (88) percent as of February 8, 2026. The NC Secretary of State website was reviewed today, and Child Development Schools of North Carolina, LLC was listed as current- active. You visited each licensed space with me. Preschool children enrolled in space 1 participated in group circle time. New Hanover County Schools had a teacher workday today so the school-agers enrolled in space 3 were combined with the older preschoolers in space 4. These children participated in free play with age-appropriate materials and activities. The toddlers enrolled in space 6 were combined with the infants in space 5. These children participated in free play with books, soft toys, music, and fine motor materials. Infant feeding schedules were observed, and safe sleep charts were being maintained. Children 2 years of age in space 7 prepared for an art activity that consisted of paper and crayons. Two therapists worked with a child in space 2 that is currently being used as a therapy space. Qualification letters were on file for both therapists. Routine toileting and handwashing procedures were monitored, and requirements were met. Staff/child ratios were in compliance in all classrooms. All children were adequately supervised. Caregivers and children interacted with children. Lunch for today consisted of tuna salad, whole grain crackers, mashed potatoes, oranges, and milk. Staff and Training Worksheets- There were fourteen staff members employed at this facility. There were four new staff; all new staff and a percentage of veteran staff’s files were reviewed. Health & Safety Training – All staff members who have been employed for at least one year, have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. New staff are in the process of completing these training courses. Health and safety trainings are required to be completed during the first year of employment and renewed every 5 years. Remember, the Recognizing and Responding to Suspicions of Child Maltreatment training must be taken at www.preventchildabusenc.org, within the first 90 days of employment. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. Ensure this information in completed, recorded on the required form, and on file for review. The facility no longer offers NC Pre-Kindergarten services. The following violations were observed. Violation Number Comment Rule 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. The program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Prior to the visit the compliance history score was 89%. The NC Secretary of State website was reviewed today and Child Development Schools North Carolina, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play in activity areas, transitions, and personal care routines. Infants were engaged in story time and rest time. Sleep checks were monitored. The caregivers were interacting and meeting the developmental needs for each of the children. All activity plans aligned with NC Early Childhood Foundations and the activities observed matched the activities listed on the activity plans. Lunch for today consisted of chicken wraps, pineapple, peas, and milk. A classroom from another Childcare Network location is utilizing space 2 while the remodeling their classroom occurs. Children’s files for these children were on-site; however, the staff files for the two (2) staff members were not. A staff member from the other location was able to bring them over and make copies during the visit. Files for one (1) new staff and two (2) visiting staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with the current program and education components as well as the quality point. Fire and sanitation inspections remain current. ABCMS was monitored today and found in compliance. The following violations were observed. You, Wendy Lindquist, Assistant Director, assisted me with the visit. The program currently operates with a five-star license, issued September 2, 2024, earning 6 points in the education component, 6 points in the program standards component meeting enhanced ratios and enhanced space, and 1 quality point. The program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Prior to the visit the compliance history score was 89%. The NC Secretary of State website was reviewed today and Child Development Schools North Carolina, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play in activity areas, transitions, and personal care routines. Infants were engaged in story time and rest time. Sleep checks were monitored. The caregivers were interacting and meeting the developmental needs for each of the children. All activity plans aligned with NC Early Childhood Foundations and the activities observed matched the activities listed on the activity plans. Lunch for today consisted of chicken wraps, pineapple, peas, and milk. A classroom from another Childcare Network location is utilizing space 2 while the remodeling their classroom occurs. Children’s files for these children were on-site; however, the staff files for the two (2) staff members were not. A staff member from the other location was able to bring them over and make copies during the visit. Files for one (1) new staff and two (2) visiting staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with the current program and education components as well as the quality point. Fire and sanitation inspections remain current. ABCMS was monitored today and found in compliance. The following violations were observed. The last fire inspection was conducted February 6, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 91% as of February 16, 2025. The NC Secretary of State website was reviewed today and Child Development Schools North Carolina, LLC was listed as current- active. All licensed indoor and outdoor spaces were monitored for compliance. Children were observed in free play in spaces 1, 2, 5 and 6. Current activity plans were posted and aligned with NC Early Childhood Foundations. Evidence of activities listed on activity plans were observed in each space. Proper handwashing was observed before, after meals, and after toileting. Informal conversations were observed between staff and children. Breakfast consisted of cereal, mandarin oranges, and milk. Lunch scheduled for today was chicken patty, diced potatoes with cheese, chuckwagon corn, whole wheat bread, and milk. The NC Pre-Kindergarten Program Site Monitoring Tool was reviewed today. The tool has been completed by the contract administrator. The program follows the New Hanover County school calendar, the length of the day meets requirements, and all staff meet education requirements. The Creative Curriculum for Preschool, 6th Edition, has been implemented and the assessment tool, GOLD, Teaching Strategies are all in place. Three formal assessments with parent conferences are scheduled through the school year. Portfolios are maintained on Teacher Strategies, GOLD by taking pictures of the children’s work and uploading them to the program. The facility uses the developmental screening tool, Brigance Early Childhood Screen III (3-5 Years). NC Pre-K requirements were met today. The following violations were documented. The facility’s Temporary License was issued March 1, 2024 and is valid through September 1, 2024. Restrictions on the permit include a first shift capacity of 144 children ages 0 to 12 years old; meeting enhanced space and ratios. Restrictions were in compliance today. Facility administrator Elisabeth Weckenmann was present and available for consultation. The status of the corporation which owns the facility, Child Development Schools North Carolina, LLC, was verified as current and active with the NC Secretary of State’s office on July 15, 2024. Prior to the visit, the facility had an eighteen-month compliance history score of 90%. As this is a change of ownership, a new building, fire and inspections must be completed prior to September 1, 2024. You will need to request your building inspection through the Coast portal. Your fire inspection was completed March 7, 2024. An approved sanitation inspection was completed March 20, 2024. Failure to obtain an approved building inspection prior to September 1, 2024 may result in administrative action against your license. Seven spaces are approved for use; space 4 was not in use today. Sixty-two children ages 0-9 years old were present today. All approved indoor and outdoor spaces were monitored today. Preschool children were observed having lunch. Lunch served today consisted of cheese pizza, pineapple chunks, green beans and milk. After lunch children completed personal care routines and prepared for rest time. Each child had an individual cot with individual linens. School age children were having a picnic lunch outside on the fenced in playground. Infants and toddlers were observed napping/resting. Safe sleep practices including documentation were monitored and in compliance today. Infant feeding charts were current and up to date. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan in space 1, space 2 and space 5 were dated July 8, 2024-July 12, 2024 and not for the week of July 15,2024-July 19,2024. This is a repeat violation. GS 110-91(12); .0508(a) 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. Used tissues, used popsicle tubes, broken and crushed plastic cups, discarded paper towels and other trash was observed on the ground of each fenced playground. Grass along the fences and sandbox has not been cut. The grass is bushy, tall and growing through the fence. 15A NCAC 18A .2832(a) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Used tissues, used popsicle tubes, broken and crushed plastic cups, discarded paper towels and other trash was observed on the ground of each fenced playground. Some of the trash had yard debris on top or weeds growing over it indicating it had been there longer than overnight. This is a repeat violation. 10A NCAC 09 .0604(p) The above violations were observed and cited. Two of the violations cited are repeat violations. Having repeat violations may result in administrative action against your license. Technical Assistance regarding violations cited: Today the activity plan in space 1, space 2 and space 5 were dated July 8, 2024-July 12, 2024. for April Activity plans are detailed plans which outline activities that will encourage children’s growth and development. The activities outlined on the activity plan are activities that will typically occur during the “Activity Center” times indicated on the daily schedule. Having a plan for the classroom allows caregivers and children the opportunity to introduce new skills or reinforce learned skills. They must be posted for easy reference by caregivers and parents. Consider having closing staff on Friday post the following week’s activity plan to ensure they are current on Monday. Outdoor premises: As we have discussed during each visit, North Carolina Child Care Rule 10A NCAC .0604(p) requires that the outdoor premises be checked each day prior to use and any debris be removed. Used tissues, used popsicle tubes, broken and crushed plastic cups, discarded paper towels and other trash was observed on the ground of each fenced playground. Some of the trash had yard debris on top or weeds growing over it indicating it had been there longer than overnight. As we discussed during the temporary time period visit on March 12, 2024, May 5, 2007 and today, at the beginning of each day, staff should walk the playground and remove any trash and debris. Staff should pick up any trash or debris they observed when outside on the playgrounds. Today we again discussed using plastic grabbers to pick up the garbage. The trash cans you purchased for the playgrounds were not observed. A large, clear trash bag was tied to the corner of a cart. The bag was overflowing with trash. Grass along the fences and sandbox has not been cut. The grass is bushy, tall and growing through the fence. These tall grasses encourage vermin which are a potential hazard to the children in care. When your grass is cut, ensure all grasses including grass along edges are cut. RATED LICENSE: The facility has chosen to continue with the points awarded during the last rated license assessment under the previous ownership. Under the previous ownership the facility earned 1 quality point for having a staff benefits package and infrastructure for parent involvement. The facility earned six points in program for meeting minimum requirements, as well as enhanced space and ratios. The facility earned an average score of 5.13 across their three assessments. The facility earned seven points in education. The facility earned a total of 14 points which is a 5-star license. Once all approved inspections have been received your license will be issued. Lead and Asbestos: It’s time to enroll your facility in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to have tap water tested for lead and to have inspections for lead paint and asbestos if applicable. You MUST enroll prior to November 1, 2024. Please see steps below: 1. Prior to the visit, the facility had an eighteen-month compliance history score of 90%. As this is a change of ownership, a new building, fire and inspections must be completed prior to September 1, 2024. You will need to request your building inspection through the Coast portal. Your fire inspection was completed March 7, 2024. An approved sanitation inspection was completed March 20, 2024. Failure to obtain an approved building inspection prior to September 1, 2024 may result in administrative action against your license. Seven spaces are approved for use; space 4 was not in use today. Sixty-two children ages 0-9 years old were present today. All approved indoor and outdoor spaces were monitored today. Preschool children were observed having lunch. The trash cans you purchased for the playgrounds were not observed. A large, clear trash bag was tied to the corner of a cart. The bag was overflowing with trash. Grass along the fences and sandbox has not been cut. The grass is bushy, tall and growing through the fence. These tall grasses encourage vermin which are a potential hazard to the children in care. When your grass is cut, ensure all grasses including grass along edges are cut. RATED LICENSE: The facility has chosen to continue with the points awarded during the last rated license assessment under the previous ownership. Under the previous ownership the facility earned 1 quality point for having a staff benefits package and infrastructure for parent involvement. The facility earned six points in program for meeting minimum requirements, as well as enhanced space and ratios. The facility earned an average score of 5.13 across their three assessments. The facility earned seven points in education. The facility earned a total of 14 points which is a 5-star license. Once all approved inspections have been received your license will be issued. The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you will not be required to have a reassessment according to timelines discussed previously, however you may request one voluntarily. Today I encouraged you to review information about the new standards as it becomes available and to plan on training your staff on new assessment tools if you are still interested in Environment Rating Scales assessments. The new tools are the “3” versions, and I will share more information about training opportunities as they become available. Education Standards: All staff need to ensure current education information has been submitted to WORKS and they have requested the appropriate evaluation for their position. Infant feeding charts were current and up to date and safe sleep charts were being maintained. During today’s visit, infants in Space 5 were observed sitting at the table eating breakfast as a caregiver stood close by. Toddlers in Space 6 were engaged with manipulative toys at the table and walking around the environment. Two-year-old children in Space 7 were outside sliding on the slide and running in the area. Preschool children in Space 1 and 2 were engaged in activity areas with peg toys, bean bags and reading books. Breakfast served today consisted of waffles, pears, and milk. A caregiver from space #7 was drinking iced coffee on the playground while supervising children. .0901(i) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Medication administration permission form for the epi-pen in space 2 expired March 18, 2024. The medication administration permission form for the Benadryl in space 2 was not on file. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) The above violations were observed today. Technical Assistance regarding violations cited: For licensing, it is required to obtain written parental permission to give prescription and over-the-counter medication. Providers must obtain written permission and instructions for giving OTC and prescription medication from a health care and the child’s parent. In Space 6 there is Benadryl and an epi-pen for one child. The action form for the epi-pen was completed and in compliance. With each medication and action plan, you must also obtain permission from the parent to give the child the medication. This form is available on the DCDEE website and is valid for six months. The epi-pen did have a permission to administer medication form, but it was dated for 9/18/23 and therefore expired on 3/18/24 (6 months after original signature). The Benadryl did not have a Medication Administration Permission form to accompany it. You will need to fill out new Medication Administration Permission forms for both medications. Today I suggested that you keep a reminder on the calendar for 6 months from now so that you can have parents fill out new forms. Today a Lysol aerosol can was in an unlocked cabinet in Space 7. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. The program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled; this program uses High Reach Learning. NC Pre-K program requirements were also monitored. According to the North Carolina Secretary of State website, your corporation Child Development Schools Noth Carolina, LLC is current and active. Your last fire drill was complete February 15, 2024 and your last outdoor inspection was completed December 11, 2023. As this is a change of ownership a new building, fire and inspections must be completed prior to September 1, 2024. You will need to contact each inspector to request the inspection. Lunch served today was beef ravioli, peas, applesauce, corn muffin and milk. NC Pre-K Site Monitoring: Space #2 is operated as an NC Pre-K classroom. The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children in this program attend from 8:00am to 2:30pm, meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Brigance Developmental screening, and Teaching Strategies Gold for their ongoing assessments. Developmental screenings and health assessments including vision, hearing, and dental screenings were on file for all NC Pre-K children’s files reviewed. Evidence of on-going assessments was observed on printed progress reports. Ms. Gore contacts parents and documents when children are absent more than three consecutive days. A Site Monitoring visit was conducted December 1, 2023 by Shannon Smiles, representative of the contracting agency New Hanover County School. Ms. Smiles signed the monitoring tool noting that all requirements were in compliance. NC Pre-K staff information was verified in the NC Pre-K Plan and all required staff members meet requirements. The Early Childhood Environment Rating Scale-Revised (ECERS-R) have not ever been completed for the NC Pre-K classroom. The facility is aware the classroom must earn a score of at least 5.0 on the ECERS-R when the next rated license reassessment is conducted. Your last fire drill was complete February 15, 2024 and your last outdoor inspection was completed December 11, 2023. As this is a change of ownership a new building, fire and inspections must be completed prior to September 1, 2024. You will need to contact each inspector to request the inspection.
