You, LaTawnya Kneebone, Director, assisted with the visit. The program currently operates with a five-star license, issued September 13, 2024, earning 6 points in the education component, 6 points in the program standards component meeting enhanced ratios and enhanced space, and 1 quality point. The program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on April 15, 2025. The last sanitation inspection was conducted on July 23, 2025, and the program received a supervisor classification. The last fire inspection was conducted on February 10, 2026, and the program was approved for day time care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety (90) percent as of March 23, 2026. The NC Secretary of State website was reviewed today, and Child Development Schools of North Carolina, LLC was listed as current- active. You visited each licensed space with me. Preschool children in space 4 ate breakfast that consisted of cereal, tropical fruit, and milk. Staff had informal conversations with the children while they ate. Children two years of age in space 2 either ate breakfast or engaged in free play with age-appropriate materials and activities. The toddlers enrolled in space 3 also participated in free play with dramatic play materials and books. Staff participated in playful banter with a child as he ran to and away from her smiling and laughing. Infants in space 1b were observed drinking bottles or engaged in free play. Infant feeding schedules were observed, and safe sleep charts were being maintained. Routine toileting and handwashing procedures were monitored, and requirements were met. Staff/child ratios were in compliance in all classrooms. All children were adequately supervised. Lunch for today consisted of turkey, broccoli, pears, whole grain pasta, cheese sauce, and milk. Staff and Training Worksheets- There were twelve staff members employed at this facility. There were two new staff; all new staff and a percentage of veteran staff’s files were reviewed. Health & Safety Training – All staff members who have been employed for at least one year, have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. New staff are in the process of completing these training courses. Health and safety trainings are required to be completed during the first year of employment and renewed every 5 years. Remember, the Recognizing and Responding to Suspicions of Child Maltreatment training must be taken at www.preventchildabusenc.org, within the first 90 days of employment. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules. Technical Assistance: String lights are hung all around space 2. These lights had a series of “female gilbert plugs” which allows you to connect multiple strands of lights to one another. You must keep all of these plugs covered just as you would in a regular electrical outlet. Each infant shall be served only formula, breast milk, and bottles labeled with their individual name. Before feeding a bottle to an infant, use name to face recognition to ensure you are giving the correct bottle to the correct child. Today I suggested staff checking all bottles for labeling before they are placed in the refrigerator. Other Information: The facility has completed the training and has entered staff. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The program currently operates with a five-star license, issued September 13, 2024, earning 6 points in the education component, 6 points in the program standards component meeting enhanced ratios and enhanced space, and 1 quality point. The program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Prior to the visit the compliance history score was 96%. The NC Secretary of State website was reviewed today and Child Development Schools North Carolina LLC, was listed as current- active. The restrictions for their license were in compliance. Fifty-two (52) children are enrolled and thirty-four (34) children ages 1-5 were present today. A walk-through of the facility was completed today and all approved spaces were monitored for compliance. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play. The caregivers were interacting and meeting the developmental needs for each of the children. Staff were engaged in free play with children in the dramatic play area, reading books, and assisting in routine care needs such as bottle feedings and toileting/diapering. One new staff member has been hired, and all required paperwork was observed. Fire and sanitation inspections remain current. Today Ms. Kneebone and I discussed their Pathways to Stars. The program plans to move forward with Pathway 1 - Program Assessment. Today we discussed next steps and that documentation is included in the packet for today. The following violation was observed and corrected during the visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. The program currently operates with a five-star license, issued September 13, 2024, earning 6 points in the education component, 6 points in the program standards component meeting enhanced ratios and enhanced space, and 1 quality point. The program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Prior to the visit the compliance history score was 96%. The NC Secretary of State website was reviewed today and Child Development Schools North Carolina LLC, was listed as current- active. The restrictions for their license were in compliance. Fifty-two (52) children are enrolled and thirty-four (34) children ages 1-5 were present today. A walk-through of the facility was completed today and all approved spaces were monitored for compliance. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play. The caregivers were interacting and meeting the developmental needs for each of the children. Staff were engaged in free play with children in the dramatic play area, reading books, and assisting in routine care needs such as bottle feedings and toileting/diapering. One new staff member has been hired, and all required paperwork was observed. Fire and sanitation inspections remain current. Today Ms. Kneebone and I discussed their Pathways to Stars. The program plans to move forward with Pathway 1 - Program Assessment. Today we discussed next steps and that documentation is included in the packet for today. The following violation was observed and corrected during the visit. Allegation 1: There is a concern that a staff member is working with a disqualification letter. *The administrator was interviewed today and reported Kourtney Caldwell as being employed and currently working at the program. The staff member initially provided a qualifying letter upon being hired (August 26, 2025); however, she failed to notify the facility of her pending charges. I interviewed Ms. Caldwell today and she stated that she was aware she needed to submit a response to our agency and failed to do so. I explained to Ms. Caldwell that her failing to submit a response to our agency resulted in an immediate disqualification and she was no longer permitted to work in child care and must leave immediately. Ms. Caldwell gathered her belonging and left the premises. Based on the observation, investigation, and staff report, the allegation was confirmed. Violations were not observed today. Violation Number Comment Rule 1802 The child care provider and/or household member who incurred pending charges, indictments, or convictions since their last qualification letter was issued by the Division, did not notify the operator of these charges within five business days or before returning to work. Tawny Kneebone, Director, assisted me with the visit. The program currently operates with a five-star license, issued September 13, 2024, earning 6 points in the education component, 6 points in the program standards component meeting enhanced ratios and enhanced space, and 1 quality point. The program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. This is the first annual compliance visit after the change of ownership that occurred May 1, 2024. The sanitation inspection was completed December 11, 2024, with an “Superior” classification. The last fire inspection was conducted on May 22, 2024, and your facility was approved for daytime care only. Prior to the visit the 18-month compliance history score was 100%. The NC Secretary of State website was reviewed today and Child Development Schools North Carolina, LLC was listed as current- active. All licensed indoor and outdoor spaces were monitored for compliance. Children enrolled in spaces 1a, 2, 3a, and 4-6 were observed in free play indoors and outdoors. Current activity plans were posted and aligned with NC Early Childhood Foundations. Evidence of activities listed on activity plans were observed in each space. Proper handwashing was observed upon arrival and after toileting/diapering routines. Informal conversations were observed between staff and children. Lunch scheduled for today was turkey with gravy, brown rice, peas, peaches, and milk. The following violations were documented and corrected during the visit. Violation Number Comment Rule 617 All openings to the outer air were not protected against the entrance of flying pest. The window in space 2 was open without the screen pulled down to prevent the entrance of flying pests. 15A NCAC 18A .2831(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space 1b, a mop reservoir with Mr. Clean in it was stored on a high shelf rather than in locked storage. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Leftover sunscreen in space 6 was not returned to the parent or discarded after the course of the treatment. .0803(12) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Technical Assistance: Please review child care rules regarding storage and accessibility of potentially hazardous items. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. Today I suggested administration do a walk through each morning to ensure all items are stowed correctly. Reminders: C. Russell must complete 1 on-going training hour by May 1, 2025. You must email me completion of this training to avoid a future violation. Rated License: The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you will not be required to have a reassessment according to the timelines discussed previously; however, you may request one voluntarily. Today I encouraged you to review information about the new standards as it becomes available and to plan on training your staff on the new assessment tools if you are interested in Environment Rating Scales assessments. I will share more information as I am able. North Carolina Rated License Assessment Project (NCRLAP): As of February 1, 2025, the new and improved Environment Rating Scales – Third Additions will be implemented.
