Upon arrival you were not present. L. James, Educational teacher was present and assisted. You (M. Rodgers), arrived shortly after and assisted. , administrator, assisted me with the visit. Your program currently operates with a five-star license, issued on February 20, 2020, earning 7 points in the education component, 7 points in the program standards component (meeting enhanced space and enhanced ratios) and 1 quality point the program having a combined turnover rate of 20 percent or less for the administrator, program coordinator, lead teacher, teacher, and group leader positions over the last 12 months. The last annual compliance visit was conducted on December 9, 2024. The sanitation inspection was completed on June 8, 2025, with a “Superior” classification. The last fire inspection was conducted on September 00, 2025 .and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of November 14, 2025. The NC Secretary of State website was reviewed on November 14, 2025 and Smiling Faces Childcare Center, Inc. was listed as current- active. The contact information for the facility was reviewed with Ms. Rodgers, and all information was verified. No changes are required. I visited each indoor and outdoor space during my walk-through. Children in each space were observed engaged in free-choice play, completing toileting, diapering, and hand washing routines and interacting with caregivers. Children in Space #2 completed an art activity making turkeys. Children in Space #5 were observed in outdoor play. Children under twelve months old received care according to individual needs including diapering and bottle feeding. They were also observed participating in floor-play activities. Proper hand washing techniques and diaper changing procedures were observed. Breakfast was observed and consisted of cheese biscuits, strawberries, bread, and milk. James, assistant director, left seven (7) children ranging from ages three to five in the office unattended for approximately two to three minutes. .1801(a)(1-5) 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. Bolts were protruding from the black border enclosing the play equipment on preschool and infant/toddler playgrounds posing tripping hazards. Documentation verifying completion of recognizing and responding to suspicions of child maltreatment training was not on file for one employee hired on June 2, 2024. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The individual responsible for supervising the children left them unattended in the office while she walked down the hall. I informed you that children cannot be left unattended at any time. I explained that the caregiver could have taken the children with her down the hall to ensure supervision was maintained. You stated that office staff receive children in the morning and are responsible for walking them to their designated classrooms. You added that you will implement a procedure to ensure children are taken to their classrooms immediately upon arrival, and parents would be asked to wait until their child can be escorted to the classroom. I explained that continuous supervision is essential to ensure children always remain in a safe environment. Any lapse in supervision could result in children engaging in activities that could result in sustaining injuries or something far worse. An unannounced follow up visit will be conducted to monitor supervision requirements. MEDICATION REQUIREMENTS: A tube of prescription cream and asthma inhaler observed were not in the original containers with the pharmacy labels and instructions. In addition, an EpiPen was also observed; however, no authorization form was available onsite to verify that the parent had provided permission for the medication to be administered. We discussed the importance of ensuring all medication requirements are met, including the need for medication to be provided in its original container so it can be verified as belonging to the individual child. You stated moving forward that all medication will accept through the main office and approved to ensure documentation is received prior to being taken to the classroom. SAFE OUTDOOR ENVIRONMENT: The items identified on the playgrounds were corrected during the visit. Maintenance reinserted the bolts into the barrier enclosing the play equipment. The two nails were hammered back into the wood to prevent them from protruding. I encouraged you to have maintenance and classroom staff to conduct safety check of the outdoor play area prior to allowing children access. This will help ensure that children do not encounter any potential hazards. QRIS DISCUSSION: WE started to review the QRIS documentation, but you requested to schedule another visit designated just to review and discuss the QRIS transition plan for your program. We scheduled a technical assistant visit for December 18, 2025, to review the information. You shared the Education Coordinator was not present and you would like her to be included in the conversation. REMINDERS: Sex Offender Registry: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. B. Rodgers, Administrator, assisted me with today’s visit. Your program currently operates with a five-star license, issued 2/20/20, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for 75% of lead teachers and teachers have at least 10 years EC work experience. The last annual compliance visit was conducted 12/9/24. The sanitation inspection was completed on 12/18/24 with a “Superior” classification. The last fire inspection was conducted on 9/23/24. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 5/19/25. The NC Secretary of State website was reviewed on 5/19/25 and Smiling Faces Child Care Center, LLC was listed as current- active. You visited each indoor and outdoor space with me. Children throughout the facility were engaged in group time, free play, art activities, and transitions. Infants were engaged in tummy time, feeding, and personal care routines. Outdoors included stationary climbers, swings, slides, ride-on toys, and balls. Proper handwashing was observed. Breakfast was observed and consisted of whole grain bran muffin, banana, and milk. The following violations were observed: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the school age playground and the toddler playground there was grass growing in the mulch and fire ants around boarder. 10A NCAC 09 .0601(a) Compliance Verification: Violations cited during today’s visit have been corrected. No further action is required at this time. Technical Assistance: Outdoor Maintenance- Proper maintenance is a key factor when trying to ensure a safe play environment for children. Each playground is unique and requires a routine maintenance check program developed specifically for that setting. Outdoor areas should be kept free of excessive high grass. Located on the school age and toddler playground grass was growing in the mulch and fire ants were present around the border. Maintenance personnel treated the fire ants and removed the grass while on site. We discussed the maintenance personnel checking the playground daily for safety issues. Additional Comments: We discussed Clean Classrooms for Carolina Kids and you have completed Lead in Water testing. Your enrollment has been started for Lead-Based Paint and Asbestos testing, you are to follow up and complete your enrollment. The new QRIS has been approved by the Child Care Commission. The Division is working on implementation and trainings for the new system. We discussed the QRIS Modernization.
