T. Biggs, Legal Designee, assisted me with the visit. Your program operates with a four-star license, issued March 5, 2021, earning 7 points in the education component, 2 points in the program standards component (meeting enhanced ratio) and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. The last annual compliance visit was conducted on May 6, 2025. The sanitation inspection was completed December 16, 2025, with a “Superior” classification. The last fire inspection was conducted on May 7, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty- eight percent as of April 7, 2026. The NC Secretary of State website was reviewed on February 19, 2026, and Guiding One's Direction Child Care & Enrichment Center, LLC was listed as current- active. I visited the indoor and outdoor spaces used by children. Children were eating lunch, completing toileting and hand-washing routines and interacting with the group leader. Lunch was observed and consisted of chicken nuggets, broccoli and carrots, peaches and milk. Proper hand-washing techniques were observed. The following violations were documented: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Departure times were not documented for three children between the dates of March 6, 2026, and March 20, 2026. 10A NCAC 09 .0302(d)(4) 807 A safe indoor and outdoor environment was not provided for the children. Large tree branches and sticks were on the outdoor playground posing a tripping hazard. 10A NCAC 09 .0601(a) 1850 Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. Signage regarding smoking and tobacco was not posted in the facility. .0604(i) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. On or before April 21, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. SAFE INDOOR/OUTDOOR ENVIRONMENT: A Large tree branches and stick were observed throughout the playground. I explained the large branches poses tripping hazards to children and could lead to potential injuries if stepped incorrectly. I advise you to have the tree branches and sticks removed from the play area. STAFF/TRAINING WROKSHEET: You tried to submit the staff/training worksheet to me electronically; however, technical difficulties were encountered. I requested that you complete a handwritten staff/training worksheet and scan to me by tomorrow April 8, 2026. I reviewed the staff file today and all requirements were met. ARRIVAL/DEPARTURE TIMES: We talked about ensuring departure times are documented when children leave at the end of the day. Departure times were not recorded for three children between March 6, 2026, to March 20, 2026. You stated you would remind parents to sign their children out daily. I informed you that the times could document by staff but they need to add their signature or initials to reflect who recorded the times. LEAD PAINT AND ABESTOS TESTING: During the visit, we discussed the Clean Classroom for Carolina Kids requirements to obtain asbestos and lead paint testing. Based on my review of the portal, your program has not completed the required tests. I informed you of the appropriate steps that must be followed to ensure the testing is completed and submitted through the Clean Classroom for Carolina Kids portal. If you have questions or need technical support, contact Clean Classroom for Carolina Kids support line for assistance. This task must be completed immediately. ABCMS REQUIRMENTS: Although a certificate was provided verifying the administrator completed the ABCMS training, my review of the ABCMS system revealed that staff were not linked to your program in the ABCMS portal. You were encouraged to retrieve the code sent after the training, and provide it to staff so they can enter it into their criminal background check portal. Additionally, the administrator will need to log back into the ABCMS portal to update and complete the process to ensure all staff are properly linked to the program. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. You, Talaya Biggs, Group Leader assisted me with the visit. Your program currently operates with a four-star license, issued March 5, 2021, earning 7 points in the education component, 2 points in the program standards component (meeting enhanced ratios) and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety percent as of January 20, 2026. The NC Secretary of State website was reviewed on January 20, 2026, and Guiding One's Direction Child Care & Enrichment Center, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play utilizing board games and other materials and completing toileting and hand-washing routine. The caregiver was interacting and meeting the developmental needs for each of the children. She participated in board games with children. The fire inspection has not occurred since the last visit and remains current. The sanitation inspection was completed on December 16, 2026. You provided me with a copy of the sanitation report during the visit. Lunch was observed and consisted of turkey sandwiches, green peas, mixed fruits and milk. We also discussed Three violations were observed today. The following violations were documented. Wooden boards with protruding screws were on the side of the building inside the fenced in playground used by children. 10A NCAC 09 .0601(a) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I suggested ensuring damaged materials be placed on the outside of the fenced area and be inaccessible to children to ensure a safe environment is always maintained. ARRIVAL/DEPARTURE DOCUMENTS: Arrival and departure logs were not available for January 5th through 8th, 2026 and for January 16, 2026. The group leader stated those were possibly the dates the children were carried over to the other program for care. Arrival and departure times must be recorded for all children while in the programs care and logs must be maintained for review during the visit. Departure times were also recorded for majority of the children on the logs reviewed. I explained when parents do not document the departure times, the staff can do so and add their signature or initials indicating to reflect the person adding the information. LICENSED/APPROVED SPACE: It was reported by the teacher that five children are taken over to another child care center owned by you for care after 5:00 p.m. due to participation in sports. It was further shared that these children are not enrolled at that facility. I explained to the teacher that child care may not be provided to children in a location where they are not enrolled or approved. I contacted you by telephone and requested that you come over to the center so that the information could be discussed with you in person. Upon arrival, I shared the information given by the teacher. During our conversation, you confirmed that the children are taken to the other facility some afternoons for care. You reported only one of the children received subsidized funding through a voucher and that the remaining children are private pay. You also stated that the parents had given permission for them to be transported to the other facility. I explained that a violation was warranted because children were cared for in an unapproved space where they are not enrolled. We also discussed the potential implications that could occur if an incident or injury were to happen while the children were not in their assigned and approved location. You stated moving forward children would no longer transported to the other program for care. You asked whether it would be permissible for your grandchild and the other children to be taken to the other facility if written permission was obtained and the children we enrolled. I informed you further research will be conducted and feedback provided later. I advised you to talk to the Department of Social Services and parents about the matter and what steps will be taken moving forward. CHANGE OF OWNERSHIP: We discussed the steps required to change your program ownership from a limited liability company to a non-profit organization. I explained that this change will require completion of the change of ownership process. In addition, a new application and all required documentation must be submitted. I will follow up with you soon to provide additional information regarding next steps and how to proceed. You stated no children were enrolled for second shift care. QRIS CONVERSATION: You stated the QRIS conversation was not required at this facility because the information was reviewed during the visit to your other facility. You, Talaya Biggs, Group Leader assisted me with the visit. Your program currently operates with a four-star license, issued March 5, 2021, earning 7 points in the education component, 2 points in the program standards component (meeting enhanced ratios) and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety percent as of January 20, 2026. The NC Secretary of State website was reviewed on January 20, 2026, and Guiding One's Direction Child Care & Enrichment Center, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play utilizing board games and other materials and completing toileting and hand-washing routine. The caregiver was interacting and meeting the developmental needs for each of the children. She participated in board games with children. The fire inspection has not occurred since the last visit and remains current. The sanitation inspection was completed on December 16, 2026. You provided me with a copy of the sanitation report during the visit. Lunch was observed and consisted of turkey sandwiches, green peas, mixed fruits and milk. We also discussed Three violations were observed today. The following violations were documented. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Record verifying arrival and departure times for children on the dates of January 5th through 8th, 2026 and January 16, 2026 were not available for review. 10A NCAC 09 .0302(d)(4) 209 Children used space that was not approved. It was reported that five school-age children were cared for in unapproved space on the dates on January 5th through 8th, 2026, and January 16, 2026. Additionally, the children were cared for in unapproved space occurred on other dates not given. GS 110-91(1)&(4-5) 807 A safe indoor and outdoor environment was not provided for the children. Wooden boards with protruding screws were on the side of the building inside the fenced in playground used by children. 10A NCAC 09 .0601(a) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 3, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. You stated the wooden boards were from a broken picnic table. I explained that the picnic table with protruding bolts posed a hazard to children and should be removed immediately. You stated you would have the picnic table removed as requested. I suggested ensuring damaged materials be placed on the outside of the fenced area and be inaccessible to children to ensure a safe environment is always maintained. ARRIVAL/DEPARTURE DOCUMENTS: Arrival and departure logs were not available for January 5th through 8th, 2026 and for January 16, 2026. The group leader stated those were possibly the dates the children were carried over to the other program for care. Arrival and departure times must be recorded for all children while in the programs care and logs must be maintained for review during the visit. Departure times were also recorded for majority of the children on the logs reviewed. I explained when parents do not document the departure times, the staff can do so and add their signature or initials indicating to reflect the person adding the information. LICENSED/APPROVED SPACE: It was reported by the teacher that five children are taken over to another child care center owned by you for care after 5:00 p.m. due to participation in sports. It was further shared that these children are not enrolled at that facility. I explained to the teacher that child care may not be provided to children in a location where they are not enrolled or approved. I contacted you by telephone and requested that you come over to the center so that the information could be discussed with you in person. Upon arrival, I shared the information given by the teacher. During our conversation, you confirmed that the children are taken to the other facility some afternoons for care. You reported only one of the children received subsidized funding through a voucher and that the remaining children are private pay. You also stated that the parents had given permission for them to be transported to the other facility. I explained that a violation was warranted because children were cared for in an unapproved space where they are not enrolled. We also discussed the potential implications that could occur if an incident or injury were to happen while the children were not in their assigned and approved location. You stated moving forward children would no longer transported to the other program for care. You asked whether it would be permissible for your grandchild and the other children to be taken to the other facility if written permission was obtained and the children we enrolled. I informed you further research will be conducted and feedback provided later. I advised you to talk to the Department of Social Services and parents about the matter and what steps will be taken moving forward. CHANGE OF OWNERSHIP: We discussed the steps required to change your program ownership from a limited liability company to a non-profit organization. I explained that this change will require completion of the change of ownership process. In addition, a new application and all required documentation must be submitted. I will follow up with you soon to provide additional information regarding next steps and how to proceed. You stated no children were enrolled for second shift care. QRIS CONVERSATION: You stated the QRIS conversation was not required at this facility because the information was reviewed during the visit to your other facility. You shared the same pathway would be follow at this program as well which is Pathway #2. I informed you the application would be sent to you via email so you would have access to it when you are ready to apply and the documents imbedded. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. (§14-208.19) To register for the e-mail notification, go to http://sexoffender.ncsbi.gov. If you have any questions, please contact your local sheriff's department. A container of Zep wipes was located on the counter in the hallway bathroom. .2820(b) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Ensure that you are keeping items such as disinfectant wipes, any aerosol spray cans, and any items that have more warnings than just “Keep out of reach of children” in locked storage. Take a fresh look at where and how these items are stored to make them quick and easy to access when needed, but still maintain the safety of the children in care. Refer to the Center for Disease Control (CDC) website for more information on best practice. This violation was corrected during the visit. Electrical Outlets - Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets (2,3). Most these injuries involve children under the age of six. An outlet in the space 2 was not protected with a safety plug. All electrical outlets not in use that are in areas accessible to children must be protected with an outlet cover. This includes unused outlets on power strips and outlets over counters in the kitchen.
